REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

Reports to unit manager. Works under supervision of a Registered Nurse and in collaboration with other health care professionals.

POSITION SUMMARY

One on one nursing preceptorship to enhance nursing knowledge, awareness of nursing role, enhancing confidence, acclimation to environment, and socialization to the team. Under supervision of RN preceptor and in collaboration with other care professionals, assists in performing a variety of nursing care services for patients.

PROGRAM DETAILS

Work one on one with a registered nurse preceptor under direct supervision in the clinical setting. Engage in required didactic educational assignments as scheduled. Participation in progress evaluations required.

Upon completion of the orientation requirements, the nursing apprentice is required to work a minimum of 24 hours a month as a nursing apprentice until transition into the nursing residency in addition to ongoing educational requirements. If a nursing apprentice does not accept or receive a residency position during the designated hiring period for their anticipated graduation date, their position as a nursing apprentice will be transitioned out of the nursing apprentice program and into another appropriate position as available.

Must maintain enrollment in accredited nursing program during Nursing Apprenticeship with exception of graduation from the program and awaiting to take NCLEX exam.

ACCOUNTABILITIES

All duties listed below are essential unless noted otherwise

  1. Nursing Apprentice works under direct supervision of an RN preceptor

  2. Perform duties related to nursing care and nursing process as noted in Table A: Role Functions of the Nursing Apprentice.

  3. Other duties as assigned.

    ROLE FUNCTIONS OF NURSING APPRENTICE

    ASSESS

    - Data collection & documentation of patient health status

    - Participate in patient rounds

    PLAN

    - Reports observations about patient's condition to RN & physician

    - Communicate with RN regarding Plan of Care & Patient/Family Education

    - Communicates with RN regarding the organization of assignments based on priorities

    IMPLEMENT

    - Perform skills, treatments or interventions provided they have performed successful educational validation and delegation of RN preceptor

    - Observes & discusses medication administration, IV therapy by the RN & other skills/interventions not authorized for Nursing Apprentice to perform

    - May not receive physician orders

    - May not delegate or supervise care

    EVALUATE

    - Reports noted changes to the registered nurse.

    - Participate in personal clinical and didactic evaluation process

    REQUIRED QUALIFICATIONS

    Education: Enrollment in an accredited nursing program and successful completion of first clinical semester.

    Certification: Basic Life Support (BLS) completed within orientation period.

    WORKING CONDITIONS

    Personal Protective Equipment: Must Demonstrate knowledge and use of standard precautions and personal protective equipment.

    Physical Demands:

    - Must be able to move about hospital and between workstations, and prolonged periods of standing.

    - Must be able to understand directions and be able to communicate and respond to inquiries with effective people skills.

POSITION SUMMARY

ProMedica Toledo hospital is seeking a highly motivated Antimicrobial Stewardship and Infectious Diseases Pharmacist to join an established Antimicrobial Stewardship Team. The Antimicrobial Stewardship and Infectious Diseases Pharmacists works collaboratively with members of the ProMedica and University of Toledo Infectious Diseases teams through daily stewardship activities and key responsibilities listed below.

ACCOUNTABILITIES

  • Assists and supports the development, implementation and management of clinical pharmacy services.

  • Provides clinical consultation and clarification to emergency department practitioners; recommends evidence-based medication therapy regimens and monitoring plans; suggests appropriate cost-effective therapeutic alternatives to medical staff as needed.

  • Supports the development and implementation of treatment guidelines, policies, protocols, formulary reviews and critical pathways as approved by appropriate committees.

  • Monitors medication therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory interactions, adverse drug reactions, appropriateness of drug and dose and therapeutic duplications.

  • Provides accurate, adequate and timely drug information to professional staff. Works collaboratively with clinical pharmacy specialists, clinical staff pharmacists, emergency department physicians, advanced practice providers, medical residents, nurses and case management.

  • Supervises and instructs pharmacy students and pharmacy residents, if applicable.

  • Participates in antimicrobial stewardship program.

  • Prepares and presents medication use evaluations, adverse drug reaction reports, medication errors, and pharmacy interventions summary to the P&T committee.

  • Supports and participates in the pharmacy's performance improvement activities including customer service and patient safety initiatives and documents departmental compliance.

  • Establishes productive, collaborative relationships with staff and community members.

  • Performs duties of Staff Pharmacist as needed.

  • Performs other duties as directed.

  • Performs all duties and responsibilities in an ethical and responsible manner.

  • Supports the department in providing a vision for its direction and embraces the change process to achieve the vision.

  • Fosters the culture of patient safety by using the error prevention techniques.

  • Coaches and facilitates the growth and development of employees within the department.

  • Communicates effectively orally and in writing within department as well as to all external customers.

  • Strives for continuous improvement by focusing on key department systems.

  • Participates in creating an environment for success through team participation.

  • Education: Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy.

  • Skills: PGY2 and/or five years of experience as a pharmacist in acute care hospital. Toledo and Flower Hospitals require residency as they precept residents at those locations.

  • Years of Experience: 5 years of experience in Antimicrobial Stewardship or Infectious Diseases.

  • License: Pharmacist licensure in current state of practice.

  • Certification: As applicable, must complete GCP and CITI training upon hiring and maintain certification.

PREFERRED QUALIFICATIONS

  • Education: Doctor of Pharmacy Degree, Completion of PGY2 Infectious Diseases Pharmacy Residency is preferred

  • Certification: Board Certification (BCIDP), BLS - Basic Life Support

POSITION SUMMARY

  • Practice consists of 2 Physicians and 2 APPs

Patient Population: Orthopedic Trauma patients from our Level 1 Trauma Center ER, Post-Orthopedic Surgeries (Joint Replacements, Fractures, Back Surgeries), Wound Vac, Hemovac, JP Drains, as well as patients with Drips (Insulin, Cardizem, and Heparin)

Specialized with: Traction set-up, Trapeze, set-up, Continuous Passive Motion Machines, and External Fixation Devices

Generations 7 acute care will offer a wide variety of experiences, as they receive patients with a wide range of diagnoses.

**We are working to become certified in Geriatric Hip Fractures.

POSITION SUMMARY The Medical Nutrition Therapy Dietitian of the Bariatric Department assesses plans and implements the nutrition education program as related to Bariatric.

ACCOUNTABILITIES

  1. Serves as a member of the multidisciplinary team. 2. Serves as a case manager for assigned patients. 3. Performs outpatient Medical Nutrition Therapy for patients, as ordered by Physician, both individually and in-group, both confidently and effectively with role model expertise. 4. Provides a variety of education materials and approaches to individualize Medical Nutrition Therapy to accommodate any patient barriers to learning. 5. Completes patient care documentation. 6. Collaborates effectively with both General Surgery and Bariatric staff to provide care. 7. Participates in team quality improvement initiatives. 8. Participates in staff Metabolic and Bariatric Surgery Committee meetings. 9. Participates in team quality improvement initiatives. 10. Adheres to hospital and department compliance plan; assists with documentation auditing. 11. Conducts or assists with as needed: staff orientation, annual competency training programs and as a student preceptor. 12. Participates in planning, researching and implementation of patient education curriculum for Bariatric services. 13. Presents consumer education programs as requested and approved. 14. Completes continuing education hours to maintain licensure and in the non-CDE, completes seventy five (75) hours of continuing education required every five years as per ADA guidelines. 15. Education: A BS or BA in Nutrition or Dietetics License: Must be Licensed and Registered Dietician in the State of Ohio, Ohio licensure (LD) through the Ohio Medical Board and Registration (RDN) through the Commission on Dietetic Registration

ADDITIONAL EXPERIENCE

Registers/pre-registers patients to receive medical services in clinic, laboratory, inpatient, outpatient or emergency department. Obtains demographic information, validates insurance, and receives/post payments. Adheres to necessary regulatory requirements. Answers and/or refers questions received from patients, visitors, and staff to appropriate department. Performs various related clerical functions. Always shows respect and value for our customer and projects an image of professionalism.

Accountabilities/Performance Standards

  • Perform a variety of face-to-face registration, pre-registration, walk in outpatient scheduling, and collection functions for all patients including self-pay patients.

  • Utilize electronic insurance verification system to verify insurance benefits and determine copay and/or coinsurance for the patient's services; ask all eligible patients for copay and/or coinsurance to meet monthly Point of Service goals.

  • Contacts patients via phone for necessary information pertaining to registration, billing and financial responsibility via our pre-registration process.

  • Assists patients at time of service with general benefit and financial responsibility inquires and posts payments to patient accounts as appropriate.

  • Manages time effectively to perform complete registration questionnaires while meeting departmental productivity standards in compliance with system productivity policies. Uses time to complete other departmental duties when patients cannot be registered.

  • Maintains high attention to detail by reviewing all work for completeness and accuracy in compliance with system quality assurance policies. Completes registrations to meet department accuracy standards for error-free work as demonstrated on the monthly KPI (Key Performance Indicators).

  • Understand basic Registration processes and provide feedback to management on issues that impede timeliness or quality of Registration and work with management to resolve.

  • Independently review assigned reports/work queues to ensure goals and deadlines are met.

  • Maintains current knowledge of insurance requirements communicated by email, memorandum, educational matrices and in-services.

  • Seeks assistance from Financial Counselors when needed to maintain patient flow while resolving financial issues. Communicates effectively with service delivery areas when unresolved financial issues threaten appointment schedules.

Position Summary:

The acute care Registered Dietitian works in collaboration with the inpatient heath care team to provide the patient centered nutrition care based on evidence based best practice guidelines.

Accountabilities:

  • Identifies patients that may be at nutrition risk based on hospital protocol.

  • Obtains accurate and timely nutritional history from patient and/or family using appropriate interviewing techniques.

  • Performs age-specific nutrition assessments for our diverse patient population based on historical, clinical and lab data, including nutrition focused physical assessment when indicated to identify patients at risk of malnutrition.

  • Provides appropriate interventions and initiates individualized nutrition care plans utilizing the Academy of Nutrition and Dietetics 'Nutrition Care Process'.

  • Provides on-going monitoring and evaluation of the patient's needs at regular intervals to assure that nutritional goals are being met.

  • Provides appropriate recommendations for enteral and parenteral nutrition support if the patient is unable to maintain adequate oral nutrition. RD may manage enteral nutrition support and oversee parenteral nutrition support when consulted by provider.

  • Attends and participates in inter-disciplinary rounds on assigned units as needed.

  • Provides education on therapeutic diets to patients and/or care givers based on the patient's needs and provider consults. This includes assessment of the patient's learning needs, barriers to learning, preferred learning styles and educational level of the patient.

  • Documents assessments, reassessments and care plans in the EMR using the standardized 'Nutrition Care Process' as per hospital policy and procedure.

  • Participates in performance improvement activities as assigned.

  • Provides nutrition care to out-patients in areas such as diabetes and oncology clinics based on provider referrals.

  • Provides care according to organizational ethics, licensure, and the Standards of Professional Practice for Dietetics.

  • Applies the Joint Commission and/or other regulatory standards in the provision of nutrition services.

  • Participates as an active member of hospital committees; represents department interests and contributes toward hospital goals.

  • Required to provide weekend and holiday coverage either on site and/or remote coverage to provide consistent nutrition care to patients.

  • Other duties as assigned by leadership.

Required Qualifications:

  • Requires a minimum of a Bachelor's degree in nutrition and dietetics or related field.

  • Completion of a dietetic internship and successful completion of the national registration exam (RD status).

Skills:

  • Must be able to use the hospital computer system to input and retrieve data.

  • Must be able to follow direction, communicate effectively (verbal and written), with compassion and empathy, respond to inquiries timely.

  • Have effective interpersonal skills

Years of Experience: One year in an acute care setting.

License: Ohio dietitians must be licensed with the State Medical Board of Ohio.

Certification: Registered Dietitian with the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics.

Working Conditions: Employee will be expected to wear Personal Protective Equipment as required by the hospital. Employee should be able to be standing, moving or walking for long lengths of time, and this may happen for the entirety of the shift.

ProMedica Coldwater Regional Hospital has been providing high-quality health care services to the Branch County community since 1939. Located in Coldwater, Michigan, ProMedica Coldwater has 71 acute medical beds and 16 adult psychiatric beds to care for patients and their families.

Position Summary:

Under the direction of a registered nurse, performs various patient care activities necessary in caring for patient or a group of patients. Performs clerical and receptionist duties as required.

Accountabilities:

  1. Applies the knowledge and skills necessary to provide care appropriate to the age of the patients served (ie: infant, pediatric, adolescent, adult, and geriatric), coordinated and delegated by licensed nursing personnel.

  2. Obtains patient data, including vital signs, height, weight, and I and O measurements and documents on the daily nursing assessment sheet or communicates results to the assigned RN. Notifies RN of abnormal vitals or PGLUs immediately.

  3. Assists physicians and nursing staff with examinations, procedures, and other processes related to direct patient care depending on assigned department, including but not limited to: Venipuncture, point of care testing, patient transport, assist patients with ADLs, assist patients with ambulation, 12 lead EKG's, initiation of cardiac monitoring, CPR, and urinary catherization.

  4. Ensures an adequate stock of supplies and equipment. Ensures proper functioning of equipment, including initial cleaning of instruments.

  5. Performs communication and coordination duties related to scheduling procedures, along with coordinating admission, discharge, transfer, and death. Collects data, photocopies information, answers telephone, takes messages, directs calls, and enters information into the computer.

  6. Picks up and delivers specimens, documents, and other items. Supports department during crisis by functioning as liaison with other departments and the public.

  7. Prepares and maintains patient records according to standards/policy.

  8. Performs any other job related to duties as indicated.

REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

Reports to unit manager. Works under supervision of a Registered Nurse and in collaboration with other health care professionals.

POSITION SUMMARY

One on one nursing preceptorship to enhance nursing knowledge, awareness of nursing role, enhancing confidence, acclimation to environment, and socialization to the team. Under supervision of RN preceptor and in collaboration with other care professionals, assists in performing a variety of nursing care services for patients.

PROGRAM DETAILS

Work one on one with a registered nurse preceptor under direct supervision in the clinical setting. Engage in required didactic educational assignments as scheduled. Participation in progress evaluations required.

Upon completion of the orientation requirements, the nursing apprentice is required to work a minimum of 24 hours a month as a nursing apprentice until transition into the nursing residency in addition to ongoing educational requirements. If a nursing apprentice does not accept or receive a residency position during the designated hiring period for their anticipated graduation date, their position as a nursing apprentice will be transitioned out of the nursing apprentice program and into another appropriate position as available.

Must maintain enrollment in accredited nursing program during Nursing Apprenticeship with exception of graduation from the program and awaiting to take NCLEX exam.

ACCOUNTABILITIES

All duties listed below are essential unless noted otherwise

  1. Nursing Apprentice works under direct supervision of an RN preceptor

  2. Perform duties related to nursing care and nursing process as noted in Table A: Role Functions of the Nursing Apprentice.

  3. Other duties as assigned.

    IMPLEMENT

    - Observes & discusses medication administration, IV therapy by the RN & other skills/interventions not authorized for Nursing Apprentice to perform

    - May not receive physician orders

    - May not delegate or supervise care

    EVALUATE

    - Reports noted changes to the registered nurse.

    - Participate in personal clinical and didactic evaluation process

    REQUIRED QUALIFICATIONS

    Education: Enrollment in an accredited nursing program and successful completion of first clinical semester.

    Certification: Basic Life Support (BLS) completed within orientation period.

    WORKING CONDITIONS

    Personal Protective Equipment: Must Demonstrate knowledge and use of standard precautions and personal protective equipment.

    Physical Demands:
