POSITION SUMMARY

This position provides nursing care under the direction and supervision of a registered nurse and/or licensed physician, and is accountable for the quality of nursing services delivered by self or others who are under his or her direction. This position observes, contributes to the plan of care, implements, reports and documents patient care in accordance with department policies, standard of care and state scope of practice. This position utilizes specialized knowledge, judgment, and nursing skills necessary to contribute to the assessment of data and to plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

CORE FUNCTIONS

1. Observes, compiles and reports the patient's physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews available information obtained by health care team members.

2. Contributes to plan of care under direction of registered nurse, including the discharge plan, utilizing assessment data and patient, family and health team input. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of care. Plans care in collaboration with members of the multidisciplinary team. Reviews plan of care to reflect changing patient needs and provides input for updates as needed.

3. Implements care based on delegated interventions identified in care plan and medical orders, within scope of practice. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. May delegate and provide supervision in the provision of care to non-licensed personnel, in accordance with state scope of practice. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.

4. Reports condition of patient, including signs and symptoms which may be indicative of change in the patient's condition to the registered nurse and/or physician. Observes and reports patient progress towards goals and expected outcomes in collaboration with other health care team members. Observes and reports patient's response and the effectiveness of patient teaching.

5. Documents observation, implementation and data in the patient record. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.

6. May plan, organize, develop and coordinate the completion of documentation and the input of patient assessment data as required by federal regulation. Maintains, updates and revises required documentation according to patient changes in condition, as regulated by federal and/or state guidelines. Effectively communicates and coordinates with members of the interdisciplinary team regarding appropriate functional/care level and transition to another level. Performs audits related to the reimbursement documentation and the quality management process. Reviews audit results and presents summary information for management utilization and review.

7. May coordinate the admission process including new resident evaluations. Provides verbal and written communication to residents, families and referrals sources regarding the process for admission to the center. Facilitates problem solving with residents, families, referral sources, insurance companies and clinical staff.

8. This position works under supervision, prioritizing data from multiple sources to provide quality care and support to the patient and family. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with staff at all levels throughout the facility, as well as patients, families and physicians.

MINIMUM QUALIFICATIONS

Must possess a current, valid LPN or LVN license in state of practice, temporary LPN or LVN license in state of practice, or compact LPN or LVN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice. Completion of a state approved IV therapy program may be required in certain facilities and/or departments in accordance with state scope of practice, regulatory requirements or facility/ department policy.

Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a LPN or LVN in the specialty area. Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

PREFERRED QUALIFICATIONS

Experience in designated clinical area is preferred.

4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.

5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.

6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.

7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.

8. In some roles, this position may supervise staff and work flow of the department.

9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.

MINIMUM QUALIFICATIONS

Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. POSITION SUMMARY

This position plans daily food production for assigned areas and produces all food items based on information obtained from production sheets, caf menus, catering requests, and any other approved source.

CORE FUNCTIONS

1. Prepares all food items according to standardized recipes. Adjusts recipe quantities for correct yields. Notifies supervisor of any problems relating to production.

2. Plans food production timing and quantities in accordance with the daily orders, menu tally sheets and production assignments. In the event of shortage, determines menu substitutions for regular and caf menus. Consults with diet tech or dietician for menu substitution needs for special diets.

3. Minimizes food waste with appropriate planning and tracking. Records food shortages and excess quantities in accordance with department procedure.

4. Checks and logs food and equipment temperatures and reports any problems to the supervisor. Reads thermometers and follows safety requirements.

5. Maintains par levels of food and supplies in assigned areas, orders and/or restocks items in accordance with established routines, rotates food stocks, checks freshness dates.

6. Keeps assigned work areas clean and in an orderly manner. Reports all needed equipment repair.

7. May receive and check in deliveries from vendors and in menu planning. May also oversee production quality when needed.

8. Works under general supervision. Provides leadership and knowledge in cooking and food preparation skills to other members of the kitchen staff. Has primarily inter-department responsibility. May deal with external customers in caf or catering work assignments.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

This position requires the knowledge and abilities in institutional food production normally acquired with two or more years of similar work experience in production cooking, plus a solid understanding of cooking principles, menu substitutions and modified recipes for all commonly prescribed diets. Must have skills and demonstrated abilities in hot food production and the use of all required commercial kitchen equipment. Must be able to learn and follow established policies and procedures, read and understand written and verbal instruction, communicate effectively, perform math calculations regarding recipe weights and amounts plus calculate amounts of food for necessary production. Must be able to learn food handling regulations and pass certification tests as required. POSITION SUMMARY

Locks and unlocks facility doors at prescribed times. Conducts money pick-ups from departments, logs pick-ups and successfully delivers the contents to the appropriate destination department. Releases bodies from the morgue, ensuring that all paperwork is complete. Checks alarms to ensure they are in proper working order. Escorts patients, visitors and employees when required for security purposes. Participates in security lead training designed for staff.

4. Maintains current knowledge of applicable laws and regulations. Performs -citizen arrest- and demonstrate critical thinking skills during extremely stressful situations as needed.

5. Appropriately and effectively utilizes assigned non-lethal weapon according to policy and training.

6. Investigates and completes written reports of complaints, critical security incidents and unusual activities involving patients, employees and visitors. Maintains an accurate log of all relevant security activities during each shift.

7. Professionally testifies in criminal court, state hearings and review boards when required.

8. Based on area of assignment may assist in the maintenance and preservation of confidential health documents including scanning into electronic patient health information systems.

9. Position reports directly to the Security Supervisor, or OIC in their absence. This position interacts internally with staff, physicians, visitors, volunteers and patients. External customers are law enforcement agencies and emergency service agencies. The position is system wide and officers may be assigned to work at any facility as needed. This position will provide guidance and direction to staff, visitors and patients during a security emergency. This position will be required to proactively identify, deter and respond to security incidents.

MINIMUM QUALIFICATIONS

Must maintain required security certifications. Must possess a valid driver's license and be eligible for coverage under the company auto insurance policy.

Must possess the ability to make accurate observations and exercise good independent judgment in a variety of situations (including emergencies). Must demonstrate the ability to write comprehensive security reports. Must successfully complete all required security departmental training, including department specific annual training and mandatory training as assigned. Must successfully complete Banner Health Basic Officer Academy. Must present a professional appearance and demeanor at all times and must possess strong verbal communications skills. Must qualify and maintain proficiency with assigned non-lethal weapons. Must be able to demonstrate the ability to perform physical requirements of the position. Capabilities will be demonstrated through the successful completion of similarly related skills testing. Employees hired on or after January 1, 2019 will need to successfully demonstrate the physical requirements before starting in the role. POSITION SUMMARY

1. Provides comprehensive nutrition assessment of patients' nutritional needs, to include nutrition-focused physical exams as appropriate. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards and established practice guidelines.

2. Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation.

3. Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols.

4. Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Must be a Registered Dietitian or RD Eligible based on academic and professional requirements as defined by the Commission on Dietetic Registration(CDR) of The Academy of Nutrition and Dietetics. Effective January 1, 2024 CDR requires a minimum of Master's degree to be eligible to take the registration examination. Must possess RD credentials within 6 months of hire. POSITION SUMMARY

This position will work under the supervision of the pharmacist performing routine duties in packaging, distribution, non-sterile compounding, and record-keeping of pharmaceutical supplies. Serves as a liaison between the customer and the pharmacist. Becomes proficient in preparing compounded intravenous medications, total parenteral nutrition products, and chemotherapeutic agents within compliance with USP regulations. Effectively uses pharmacy computer systems to investigate and respond to missing medication requests. Consistently demonstrates an ability to assess a situation, consider alternatives, and choose the appropriate course of action. May educate and train peer Pharmacy Technicians, Technician Externs, Technician Trainees, and volunteers. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients.

CORE FUNCTIONS

1. Identifies medications and other items for reorder as necessary to maintain a continuous inventory of medications and supplies. Receives, checks-in, and restocks medications ordered through the wholesaler and other vendors verifying all quantities against invoices. Assesses practical ways to deal with shorted items and short-dated or unusable items.

2. Manages users within the automated dispensing technology database as per policy. Investigates discrepancies and troubleshoots malfunctions in automated dispensing technology.

3. Exhibits excellent interpersonal skills and is customer service oriented in all contacts with patients, visitors and staff. Responds to standard requests from internal customers. Interacts primarily with department personnel, cross-department personnel, internal customers, patients and physicians.

4. May use automated dispensing technology or requisition to restock medications to nursing and ancillary areas after checking by a pharmacist. When allowed by law and policy, and upon completion of proper training/competency assessment, may restock automated dispensing technology using barcode scanning verification devices. Restocks medications to all areas outside automated dispensing technology which may include but not limited to trays, code carts and emergency boxes.

5. Compounds non-sterile medication products. Understands and appropriately follows all product worksheets/recipes for documentation and check processes. Understands and adheres to all applicable USP compounding guidelines. Demonstrates knowledge of and consistently performs calibrations of equipment and accurately measures components for compounding. Repackages bulk medications into unit of use packaging. Completes accurate documentation of medications compounded and repackaged per policy.

6. Maintains a safe and clean working environment by complying with all policies, laws, and regulations. Performs quality control activities such as monthly nursing unit inspections and pharmacy section checks in a timely manner. Completes documentation of refrigerator temperatures, and equipment maintenance activities as required.

7. Handles narcotic medications following all federal and legal requirements as set through policy and procedure. Understands all regulatory requirements for receiving, compounding, repackaging, and distribution for controlled substances. Consistently adheres to all storage and inventory movement documentation requirements for controlled substances.

8. Becomes proficient in performing compounding and labeling of intravenous non-hazardous compounds using proper sterile and aseptic technique as appropriate to the site of care. Understands the different product lines in sterile populations (adult, pediatric, and neonate). Completes all required environmental cleaning procedures. Documents all required tasks and other information per policy.

MINIMUM QUALIFICATIONS

Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice.

Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.

Ability to complete moderate to difficult mathematical calculations using a hand-held calculator is required. Strong customer service focus, exceptional communication skills, strong attention to detail, prioritization capabilities, superior problem-solving skills, and ability to multi-task. Must have knowledge of computer software, particularly medication management information system software, Microsoft applications and automated dispensing systems. Banner Boswell Medical Center is seeking a new Health Unit Coordinator. This position is entry level and responsible for a variety of administrative support functions and customer service for an assigned patient care area which promote an organized, efficient delivery of care to patients. POSITION SUMMARY

This position is responsible for a variety of administrative support functions and customer service for an assigned patient care area which promote an organized, efficient delivery of care to patients. The position facilitates communication between physicians, nurses and physicians, calls in Consults and acts as a liaison with all other internal departments and external contacts.

CORE FUNCTIONS

1. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.

2. Processes patient admissions, records bed assignments in system, makes face sheets and prints patient labels upon admission. Orders outside records from other facilities and Physician's offices.

3. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. May be responsible for QC/validations.

4. Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges.

5. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.

6. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.

7. This position works under local supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with other health unit secretaries and other staff at all levels throughout the facility. Customers are patients, families, physicians, unit staff, case managers, patient service staff and medical records staff. Interacts with a wide variety of external customers.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Requires skills and abilities typically attained with 1 year of medical clerical experience; preferably in a hospital or physician office setting, or prior experience in customer service. Must possess excellent organizational skills, as well as effective human relations and communication skills. POSITION SUMMARY

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

CORE FUNCTIONS

1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient's status.

3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.

4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. POSITION SUMMARY

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. POSITION SUMMARY

This position works in a senior level role in an assigned area of warehouse operations, including stocking, storage rotation, expiration dates, lot number tracking, picking, packing, storage logistics, order filling, shipping, receiving, customer service and computerized item order entry. 4. Stores new stock and assures that all items in assigned areas are stored appropriately. Rotates stock in date order and maintains a clean and orderly storage area.

5. Fills orders accurately and in a timely manner. Records orders filled, notes any out-of-stock items or product substitutions for filled orders.

6. As assigned, assists customers with service and supply needs.

7. As assigned, operates and maintains warehouse equipment such as fork lifts, pallet jacks, lift trucks and cleaning equipment. Must be licensed to operate reach trucks, order pickers, pacers and walkie riders and trained to follow all standard operating procedures related to safe operation of equipment to include tether and belt safety, battery care, and charging.

Banner Lassen Medical Center offers the latest in state-of-the-art equipment for optimal patient care and outcomes. Be part of a multidisciplinary team where the physicians work in close collaboration with the nursing staff. The facility is designated as a Blue Distinction Center for maternity care. This is a low risk birthing center with a Level I nursery. The unit averages 20-25 deliveries per month and is staffed with a minimum of two RN's per shift. POSITION SUMMARY

POSITION SUMMARY

This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional.

5. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.

6. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record.

7. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. MINIMUM QUALIFICATIONS

POSITION SUMMARY

This position provides leadership and training for environmental services associates, while working as a member of the EVS Associate team.

CORE FUNCTIONS

1. Ensures that staffing plans are carried out properly, and that all areas of the hospital/facility are staffed appropriately. With supervisor, plans completion of hospital wide projects, ensures consistent service delivery standards in all aspects of the Environmental Services and Linen operations. Gives input to supervisor on quality and timeliness of work performed by environmental services staff.

2. Performs cleaning functions in patient rooms, specialty areas and common areas.

3. Works closely with supervisor to train and retrain, recruit, retain and develop the Environmental Services team members, fostering a culture that reflects the core values and operational polices of our department and the company.

4. May supervise staff in the absence of the Environmental Services leader. Makes work assignments and responds to immediate needs by moving staff to various areas of facility as needed. Moves furniture and sets up rooms for meetings and special functions.

5. Assists supervisor in performing Quality Assurance Inspections for review and correction of deficiencies.

6. Distributes linens and supplies as needed; takes inventories in assigned areas.

7. Works independently with general supervision. Department responsibility only. Customers include staff, patients, visitors, physicians and vendors. Oversees daily activities of a team of Environmental Services Associates. Makes staffing assignments in absence of supervisor; coordinates cleaning projects as assigned, assists with on-the-job training, provides input to performance evaluations.

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
