POSITION SUMMARY
This position provides nursing care under the direction and supervision of a registered nurse and/or licensed physician, and is accountable for the quality of nursing services delivered by self or others who are under his or her direction. This position observes, contributes to the plan of care, implements, reports and documents patient care in accordance with department policies, standard of care and state scope of practice. This position utilizes specialized knowledge, judgment, and nursing skills necessary to contribute to the assessment of data and to plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
Observes, compiles and reports the patient's physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews available information obtained by health care team members.
Contributes to plan of care under direction of registered nurse, including the discharge plan, utilizing assessment data and patient, family and health team input. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of care. Plans care in collaboration with members of the multidisciplinary team. Reviews plan of care to reflect changing patient needs and provides input for updates as needed.
Implements care based on delegated interventions identified in care plan and medical orders, within scope of practice. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. May delegate and provide supervision in the provision of care to non-licensed personnel, in accordance with state scope of practice. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
Reports condition of patient, including signs and symptoms which may be indicative of change in the patient's condition to the registered nurse and/or physician. Observes and reports patient progress towards goals and expected outcomes in collaboration with other health care team members. Observes and reports patient's response and the effectiveness of patient teaching.
Documents observation, implementation and data in the patient record. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
May plan, organize, develop and coordinate the completion of documentation and the input of patient assessment data as required by federal regulation. Maintains, updates and revises required documentation according to patient changes in condition, as regulated by federal and/or state guidelines. Effectively communicates and coordinates with members of the interdisciplinary team regarding appropriate functional/care level and transition to another level. Performs audits related to the reimbursement documentation and the quality management process. Reviews audit results and presents summary information for management utilization and review.
May coordinate the admission process including new resident evaluations. Provides verbal and written communication to residents, families and referrals sources regarding the process for admission to the center. Facilitates problem solving with residents, families, referral sources, insurance companies and clinical staff.
This position works under supervision, prioritizing data from multiple sources to provide quality care and support to the patient and family. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with staff at all levels throughout the facility, as well as patients, families and physicians.
MINIMUM QUALIFICATIONS
Must possess a current, valid LPN or LVN license in state of practice, temporary LPN or LVN license in state of practice, or compact LPN or LVN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice. Completion of a state approved IV therapy program may be required in certain facilities and/or departments in accordance with state scope of practice, regulatory requirements or facility/ department policy.
Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a LPN or LVN in the specialty area. Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
PREFERRED QUALIFICATIONS
Experience in designated clinical area is preferred.

9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting. Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working at Banner MD Anderson on the Banner University Medical Center Phoenix campus must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. This is a requirement of the Whole Family Counseling Program held at this specific facility. POSITION SUMMARY
This position plans daily food production for assigned areas and produces all food items based on information obtained from production sheets, caf menus, catering requests, and any other approved source.
CORE FUNCTIONS
Prepares all food items according to standardized recipes. Adjusts recipe quantities for correct yields. Notifies supervisor of any problems relating to production.
Plans food production timing and quantities in accordance with the daily orders, menu tally sheets and production assignments. In the event of shortage, determines menu substitutions for regular and caf menus. Consults with diet tech or dietician for menu substitution needs for special diets.
Minimizes food waste with appropriate planning and tracking. Records food shortages and excess quantities in accordance with department procedure.

May receive and check in deliveries from vendors and in menu planning. May also oversee production quality when needed.
Works under general supervision. Provides leadership and knowledge in cooking and food preparation skills to other members of the kitchen staff. Has primarily inter-department responsibility. May deal with external customers in caf or catering work assignments.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.

Banner Lassen Medical Center is looking for a Registered Nurse to work in the Women & Infant Services department in Susanville, California. Banner Lassen Medical Center offers the latest in state-of-the-art equipment for optimal patient care and outcomes. Be part of a multidisciplinary team where the physicians work in close collaboration with the nursing staff. The facility is designated as a Blue Distinction Center for maternity care. This is a low risk birthing center with a Level I nursery. The unit averages 20-25 deliveries per month and is staffed with a minimum of two RN's per shift. Becomes proficient in preparing compounded intravenous medications, total parenteral nutrition products, and chemotherapeutic agents within compliance with USP regulations. Effectively uses pharmacy computer systems to investigate and respond to missing medication requests. May educate and train peer Pharmacy Technicians, Technician Externs, Technician Trainees, and volunteers. Identifies medications and other items for reorder as necessary to maintain a continuous inventory of medications and supplies. Receives, checks-in, and restocks medications ordered through the wholesaler and other vendors verifying all quantities against invoices. Assesses practical ways to deal with shorted items and short-dated or unusable items.
Manages users within the automated dispensing technology database as per policy. Investigates discrepancies and troubleshoots malfunctions in automated dispensing technology.
Exhibits excellent interpersonal skills and is customer service oriented in all contacts with patients, visitors and staff. Responds to standard requests from internal customers. Interacts primarily with department personnel, cross-department personnel, internal customers, patients and physicians.
May use automated dispensing technology or requisition to restock medications to nursing and ancillary areas after checking by a pharmacist. When allowed by law and policy, and upon completion of proper training/competency assessment, may restock automated dispensing technology using barcode scanning verification devices. Restocks medications to all areas outside automated dispensing technology which may include but not limited to trays, code carts and emergency boxes.
Compounds non-sterile medication products. Understands and appropriately follows all product worksheets/recipes for documentation and check processes. Understands and adheres to all applicable USP compounding guidelines. Demonstrates knowledge of and consistently performs calibrations of equipment and accurately measures components for compounding. Repackages bulk medications into unit of use packaging. Completes accurate documentation of medications compounded and repackaged per policy.
Maintains a safe and clean working environment by complying with all policies, laws, and regulations. Performs quality control activities such as monthly nursing unit inspections and pharmacy section checks in a timely manner. Completes documentation of refrigerator temperatures, and equipment maintenance activities as required.
Handles narcotic medications following all federal and legal requirements as set through policy and procedure. Understands all regulatory requirements for receiving, compounding, repackaging, and distribution for controlled substances. Consistently adheres to all storage and inventory movement documentation requirements for controlled substances.
Becomes proficient in performing compounding and labeling of intravenous non-hazardous compounds using proper sterile and aseptic technique as appropriate to the site of care. Understands the different product lines in sterile populations (adult, pediatric, and neonate). Completes all required environmental cleaning procedures. Documents all required tasks and other information per policy.
MINIMUM QUALIFICATIONS
Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice.
Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
Ability to complete moderate to difficult mathematical calculations using a hand-held calculator is required. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.
Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
This position works under local supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with other health unit secretaries and other staff at all levels throughout the facility. Customers are patients, families, physicians, unit staff, case managers, patient service staff and medical records staff. Interacts with a wide variety of external customers.

Provides patient and family nutrition counseling and education. Develops Nutrition Standards of Practice and Education material in collaboration with peers. Serves as a member of the interdisciplinary Health Care team providing input into patient care policies, programs, quality improvement activities and provides in-service education in areas of expertise.
CORE FUNCTIONS
Provides comprehensive nutrition assessment of patients' nutritional needs, to include nutrition-focused physical exams as appropriate. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards and established practice guidelines.
Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation.
Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols.
Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Provides pertinent information in the medical records, patient plans of care and/or clinical pathways while meeting requirements of regulatory agencies (i.e. Joint Commission, CMS, State and Local Agencies).
Provides nutrition education to patients, families, staff and other health care team professionals through the development of effective teaching materials and in-service programs; Evaluates patient and/or family response to education.
Provides leadership in area of coverage, monitors proper patient care procedures and acts as liaison with nursing areas. May participate in process improvement teams. Serves as mentor to new staff and interns where assigned.

MINIMUM QUALIFICATIONS
Must possess knowledge of nutrition and dietetics demonstrated through the completion of a Bachelor's degree in Nutrition and/or Dietetics. Must be a Registered Dietitian or RD Eligible based on academic and professional requirements as defined by the Commission on Dietetic Registration(CDR) of The Academy of Nutrition and Dietetics. Effective January 1, 2024 CDR requires a minimum of Master's degree to be eligible to take the registration examination. Must possess RD credentials within 6 months of hire. POSITION SUMMARY

Banner Lassen Medical Center offers the latest in state-of-the-art equipment for optimal patient care and outcomes. Be part of a multidisciplinary team where the physicians work in close collaboration with the nursing staff. The facility is designated as a Blue Distinction Center for maternity care. This is a low risk birthing center with a Level I nursery. The unit averages 20-25 deliveries per month and is staffed with a minimum of two RN's per shift. This is a per diem position with no benefits or guaranteed hours.

POSITION SUMMARY

POSITION SUMMARY
This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records and provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.

Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.
Processes patient admissions, records bed assignments in system, makes face sheets and prints patient labels upon admission. Orders outside records from other facilities and Physician's offices.
Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. May be responsible for QC/validations.
Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges.
Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery.
Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
This position works under local supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with other health unit secretaries and other staff at all levels throughout the facility. Customers are patients, families, physicians, unit staff, case managers, patient service staff and medical records staff. Interacts with a wide variety of external customers.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.
Requires skills and abilities typically attained with 1 year of medical clerical experience; preferably in a hospital or physician office setting, or prior experience in customer service. Must possess excellent organizational skills, as well as effective human relations and communication skills. Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, Boswell Skilled Nursing Facility or the Whole Health Clinic must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
Working knowledge of medical terminology and computer literacy is required. The position requires knowledge of commonly used office software and keyboarding skills.
PREFERRED QUALIFICATIONS
Successful completion of an approved unit secretary program and BLS certification is preferred.
Additional related education and/or experience preferred.
