Patient Care Assistant

This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records and provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.

POSITION SUMMARY:

This incumbent provides routine patient care to promote patient comfort and contributes to the overall patient plan of care. The position also performs a variety of support functions which contribute to the overall efficient operation of the nursing unit/department.

CORE FUNCTIONS

  1. Assists with daily living activities and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed.

  2. Collects patient specimens and data, including vital signs, input/output and other delegated measurements in accordance with policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.

  3. Recognizes and reports abnormalities and/or changes in the patient's health status and responses to treatment to a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.

  4. Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and according to state scope of practice. Procedures may include blood draws, foley catheter removal, IV removal, EKGs and Health Unit Coordinator duties. Provides necessary supplies, support and assistance to clinical staff and patients for procedures, under the direct supervision of a licensed professional.

  5. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.

  6. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.

  7. Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in room turnover.

  8. Supports a fast-paced environment with a strong focus on customer service. Interacts with staff at all levels, as well as patients, families, and physicians.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge.

Current BLS certification is required.

Must possess excellent organizational skills, as well as effective human relations and communication skills.

Computer literacy and keyboarding skills are required. In California: advanced skills are excluded.

Employees working at Banner Behavioral Health Hospital or BTMC Behavioral must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

Employees working at BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working at Banner MD Anderson on the Banner University Medical Center Phoenix campus must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. This is a requirement of the Whole Family Counseling Program held at this specific facility.

PREFERRED QUALIFICATIONS

Recent healthcare experience preferred. Working knowledge of medical terminology is preferred.

POSITION SUMMARY

This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.

CORE FUNCTIONS

  1. Prepares patient for exam and treatment by taking and recording vital signs, symptoms and other necessary measurements and recording chief complaint; documents pain scores as appropriate. Reports condition of patient which may be indicative of change in the patient's condition to the clinician.

  2. Assists providers with exams and minor in-office procedures which could include taking patients' blood pressure, documenting respiration and heart rate/pulse, taking temperature, as well as performing phlebotomies, ear irrigation and EKG's. Acts as chaperon as needed. Uses universal blood and body fluid precautions at all times and personal protective equipment as needed. Responsible for prioritization of patient flow through clinical process.

  3. Coordinates and manages patient care including pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Responsible for communicating test results to patient. Acts as a resource to clinician in order to provide optimal patient care. May administer medications in accordance with established policies and procedures or as directed.

  4. Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Provides complete and accurate information necessary for billing related to the patient encounter in a timely fashion.

  5. Maintains a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas. Disinfects equipment and instruments using appropriate solutions following sterilization procedures. Follows manufacturer's recommendations and OSHA guidelines in handling of hazardous substances. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.

  6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).

  7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.

  8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.

  9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician's offices in order to report, ask for or clarify information. Prioritizes data from multiple sources to provide support for the response of the patient and family to changes in health status.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge. Completion of an approved medical assistant training program as defined by state regulations.

Active Medical Assistant Certification is required. Certification or additional training must meet the requirements for Meaningful use. BLS certification required. POSITION SUMMARY

Provides various levels of service for the management of sterile surgical instrumentation and disposable patient care supplies. Support may-or-may-not include, decontamination of surgical instruments, assembly/sterilization, sterile instrument storage, partial or complete case cart support, OR core tech support and high-level disinfection.

  2. Responsible for the decontamination, assembly, and packaging of surgical instruments following established protocols, instructions for use (IFU) as well as performs required quality assurance, repairs, reassembly, documentation, sterility handling, and storage of specialized surgical instruments. Maintains required sterilization logs and quality assurance records relating to all equipment associated for the purpose of decontamination, HLD disinfection, and sterilization.

  3. Ability to interpret the surgical schedule and medical terminology as it pertains to scheduled procedures requiring sterile surgical instrumentation and disposable supplies needed. Able to address and communicate special needs to minimize delays to surgery. Responds to emergent calls from the Operating Room and performs preliminary investigation. Determines priority level of instrumentation requests and other hospital needs. Directs services in a competent and professional manner while assuring OR priorities are completed in a timely manner.

  4. Able to differentiate surgical instrumentation that is surgery owned, consignment/loaner, hospital depart, surgeon, ambulatory surgery center, and clinics both on and off campus. Completes necessary tracking of surgical instruments with a paper log sheet or computer process for instrumentation leaving the area. Reports needed replacement of surgical instruments to maintain the inventory and support readiness of sets.

  5. Utilizes specialized equipment for the decontamination, high-level disinfection, and sterilization of surgical instrumentation according to the device and equipment manufacturer's recommendations and hospital policy. Operates steam and hydrogen peroxide sterilizers of various sizes with the potential of operating ethylene oxide sterilizers where available. Operates automated surgical instrument washers, ultrasonics, flush pumps, and cart washers to include high-level disinfectors where supported.

  6. Operates various quality assurance testing equipment associated to flexible endoscopic scopes, transesophageal ultrasound probes, and electro-surgical insulation defect detector for laparoscopic and energy provided surgical instrumentation. Utilizes heat detection devices to allow steam sterilized surgical instrumentation to be released for use without risking burns to staff and patients. Operates demagnetizers of stainless-steel instruments for ophthalmic and delicate cardiac surgical instrumentation for safe use on patients. POSITION SUMMARY

As a Fitter you will be able to use your experiences assisting providers and the orthotist with fitting patients with industry standard bracing and orthotic products on extremities to assist the patient in their recovery to better health. The position is also responsible for performing other processes related to DME fittings and assisting with general operation such as Inventory control, Patient scheduling and Insurance verification. POSITION SUMMARY

This position is responsible for fitting patients with durable medical equipment (DME) and to ensure successful fitting outcomes. The position is also responsible for performing other processes related to DME fittings and assisting with general operation.

CORE FUNCTIONS

  1. Assists patients in properly fitting, measuring and adjusting prefabricated and off-the-shelf orthoses such as cervical orthoses, knee braces, ankle walkers and pressure gradient hose. Educates patients on the use and maintenance of products.

  2. Provides evaluation of selected orthotic needs of patients and educates patients on the application, use, care, and expected outcome for DME products as indicated by the physician instructions and manufacturer recommendations.

  3. Obtains required patient and insurance information for pre-registration, benefit verification and pre-authorization. Verifies that prescription information is complete and accurate. Schedules fitting appointments in system.

  4. Completes assessment notes on patient records and maintains accurate documentation of patient care provided. Posts charges for durable medical equipment (DME) and other applicable medical products in the system.

  5. May assist with purchasing of retail merchandise and durable medical equipment. May assist with sales and general operations including inventory, merchandise display and customer service. May assists in the planning and development of department actions to achieve department/service goals.

  6. Maintains main supply inventory and completes daily/bi-daily purchase orders for products used in clinics that fall below par levels and submit for approval and ordering. Checks in all orders delivered and disburse deliveries to intended departments and/or persons. Pulls product to be transferred to satellite offices and complete daily inventory transfers from main supply to satellite offices.

  7. Reviews surgery schedules the day prior and check for add on surgeries to ensure product has been delivered to patient or to have the courier deliver products to surgery center. Prepares Post Op surgery equipment into bags to be delivered to patients and coordinate delivery of the products to the necessary office.

  8. This position has responsibility for interacting with patients, families, physicians, and staff in various departments throughout the facility and organization. External contacts may include employees of other healthcare organizations, community providers and agencies concerning the health care of the patient. Also interacts with the physician offices in order to report, inquires or clarify information.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge and two years of fitting of soft goods and bracing in a clinical setting.

Certification for BLS is required.

Must complete continuing education to maintain certification and/or current practices. Requires the ability to establish and maintain effective working relationships with volunteers, staff and community members. Requires strong retail customer service skills and knowledge of, and ability to operate, business machines. Must possess the ability to communicate sufficiently to perform duties in a manner consistent with departmental scope of services and standards.

Basic knowledge of medical terminology, computer literacy and keyboarding skills are required.

PREFERRED QUALIFICATIONS

Clinical Pharmacy Services provides collaborative drug therapy management and educational interventions in patient care to optimize medication therapy, improve outcomes, promote wellness, and disease prevention.

POSITION SUMMARY

This position provides support to a team of clinical pharmacists in population health management. The technician will work under the supervision of clinical pharmacists. Primary responsibilities include pre-screening and support functions for Medication Therapy Management and Prior Authorizations. Other support functions include data gathering and entry, electronic document filing and electronic faxing. Additional responsibilities may include scripted phone calls to patients and/or physician offices.

CORE FUNCTIONS

  1. Inputs and maintains data and documents in multiple software application types, both in-house and vended software products. Processes all assigned data sets with the specifications of existing workflows. Using programs to include, but not limited to, SharePoint, Word, Excel, Visio, Acuity, and Fortis. Adheres to all documentation standards to ensure relevant information within assigned work areas is retrievable.

  2. Communicates with health plan members, pharmacists, pharmacy staff, physicians, care managers, and other members of the healthcare team to promote successful medication use and improve quality, as measured by CMS HEDIS and NCQA, AHCCCS, or other regulatory body. Communicates with clarity and professionalism in all settings.

  3. Researches and evaluates clinical data as necessary for pharmacist review and complete a prior authorization or other utilization management function, contact members regarding pharmacotherapy optimization, or conduct prescriber outreach initiatives. This includes work within Microsoft excel, faxing, and/or calling members of the healthcare team. Conducts all data collection and communication in a timely manner consistent with maintain service levels.

  4. Maintains proficiency in the pharmacy standards and workflows within the assigned area. Follows all applicable policies, procedures, and desktop procedures and recommends process improvements as necessary. Collaborates with other team members and network providers to ensure that members have a positive customer service experience.

  5. Works independently under regular supervision. Uses structured work procedures and independent judgment to solve problems and achieve high quality levels. Work output has a significant impact on business goal attainment. Customers include patients, pharmacists, providers, non-physician providers, other health care team members, health plan representatives and medical office staff for the purpose of integrating services, improving patient care and ensuring effective communication systems.

MINIMUM QUALIFICATIONS

Requires pharmacy technician licensure in the state of practice (where applicable). Pharmacy technician certification required.

Must demonstrate 2 or more years of experience performing pharmacy technician duties in a pharmacy setting with managed care emphasis preferably within a healthcare organization or retail pharmacy. Must have good written and verbal communication skills for interfacing with all levels of staff, physicians, patients and other contacts. Must have the ability to learn and master various software programs necessary for job functions.

POSITION SUMMARY

POSITION SUMMARY

This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.

  6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).

  7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.

  8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.

  9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician's offices in order to report, ask for or clarify information. The Senior Patient Access Services Representative's first and foremost responsibility is to educate our patients on their financial responsibility, walk them through their options, and collect their amount due. POSITION SUMMARY

This position provides leadership, precepts, training and offers on-going guidance as needed for an admitting/registration team. This position conducts customer service, registration, point of service collections, may validate and/or obtain authorizations from payers in order to maximize reimbursement. Provides a customer-oriented interaction with each patient in order to maximize customer experience. Obtains all required consents for each registration. Document all facets of the registration process, loads correct payer(s) to each account and meet accuracy goals as determined by management. Collect payments and regular collection targets as determined by management. May perform financial counseling when appropriate. Meets productivity targets as determined by management. Demonstrates the ability to resolve customer issues and provide excellent customer service.

Obtains federally/state required information and all consents and documentation required by the patient's insurance plan(s).   3. Verifies and understands insurance benefits, collects patient responsibility based on estimates at the time of service or during the pre-registration process. As assigned collection attempts may be made at the bedside. Must be able to consistently meet monthly individual collection target as determined by management.

  4. May provide financial counseling to patients and their families. Explains company financial policies and provides information as to available resources, offers and assists patients with applying for Medicaid. Assists patients with completing all financial assistance programs (i.e.: basic financial assistance, enhanced financial assistance, prompt pay discount, loan program).

  5. Acts as a liaison between the patient, the billing department, vendors, physician offices and the payor to enhance account receivables performance, resolve outstanding issues and/or patient concerns, and maximize service excellence.

  6. Communicates with physicians, clinical and hospital staff, nursing and Health Information Management Services to resolve outstanding issues and/or patient concerns. Works to meet the patient's needs in financial services.

  7. Consistently meets monthly individual productivity goal as determined by management. Completes daily assignments/work lists, keeps electronic productivity log up to date and inputs information accurately. Identifies opportunities to improve work process and practices good teamwork.

  8. Provides a variety of patient services and financial services tasks. May be assigned functions such as transporting patients, may precept new hire employees, recapping daily deposits, posting daily deposits or conducting other work assignments of the Patient Financial Services team.

POSITION SUMMARY

This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.

CORE FUNCTIONS

  1. Prepares patient for exam and treatment by taking and recording vital signs, symptoms and other necessary measurements and recording chief complaint; documents pain scores as appropriate. Reports condition of patient which may be indicative of change in the patient's condition to the clinician.

  2. Assists providers with exams and minor in-office procedures which could include taking patients' blood pressure, documenting respiration and heart rate/pulse, taking temperature, as well as performing phlebotomies, ear irrigation and EKG's. Acts as chaperon as needed. Uses universal blood and body fluid precautions at all times and personal protective equipment as needed. Responsible for prioritization of patient flow through clinical process.

  3. Coordinates and manages patient care including pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Responsible for communicating test results to patient. Acts as a resource to clinician in order to provide optimal patient care. May administer medications in accordance with established policies and procedures or as directed.

  4. Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Provides complete and accurate information necessary for billing related to the patient encounter in a timely fashion.

  5. Maintains a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas. Disinfects equipment and instruments using appropriate solutions following sterilization procedures. Follows manufacturer's recommendations and OSHA guidelines in handling of hazardous substances. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.

  6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).

  7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.

  8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.

  9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician's offices in order to report, ask for or clarify information. Prioritizes data from multiple sources to provide support for the response of the patient and family to changes in health status.

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge. Completion of an approved medical assistant training program as defined by state regulations.

Active Medical Assistant Certification is required. Certification or additional training must meet the requirements for Meaningful use. BLS certification required. Active Medical Assistant certification such as: Registered Medical Assistant (RMA) with proof of current membership from American Medical Technologists (AMT) or Certified Medical Assistant (CMA) with proof of current membership from American Association of Medical Assistants (AAMA) or Certified Clinical Medical Assistant (CCMA) with proof of current membership from National Health Career Association (NHA) (Test administered after 7/1/2017 only) or National Certified Medical Assistant (NCMA) with proof of current membership from National Certified Competency Testing (NCCT).

Requires strong computer skills, including the ability to work with medical software. Requires exceptional interpersonal and communication skills. Requires the ability to manage multiple changing priorities in an effective and organized fashion.

Employees working at BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working at Banner MD Anderson on the Banner University Medical Center Phoenix campus must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. This is a requirement of the Whole Family Counseling Program held at this specific facility.

PREFERRED QUALIFICATIONS

Experience with office medical procedures and treatments and knowledge of insurance, managed care operations, EMR and scheduling software preferred.

We are looking for a motivated, experienced Certified Medical Coder | Physician Practice Coder with GI, Hepatology, and/or General Surgery Coding experience to join our talented Team. MUST have CPC and/or CCS-P coding certification in active status. The production expectations are the code 40-50 charges per hour, with a mix of procedure and E/M services. POSITION SUMMARY

This position evaluates medical records, provides clinical abstracts and assigns appropriate clinical diagnosis and procedure codes in accordance with nationally recognized coding guidelines.

CORE FUNCTIONS

  1. Analyzes medical information from medical records. Accurately codes diagnostic and procedural information in accordance with national coding guidelines and appropriate reimbursement requirements. Consults with medical providers to clarify missing or inadequate record information and to determine appropriate diagnostic and procedure codes. Provides thorough, timely and accurate assignments of ICD and/or CPT4 codes, MS-DRGs, APCs, POAs and reconciliation of charges.

  2. Abstracts clinical diagnoses, procedure codes and documents other pertinent information obtained from the medical record into the electronic medical records. Seeks out missing information and creates complete records, including items such as disease and procedure codes, point of origin code, discharge disposition, date of surgery, attending physician, consulting physicians, surgeons and anesthesiologists, and appropriate signatures/authorizations. Refers inconsistent patient treatment information/documentation to coding quality analysis, supervisor or individual department for clarification/additional information for accurate code assignment

  3. Provides quality assurance for medical records. For all assigned records and/or areas assures compliance with coding rules and regulations according to regulatory agencies for state Medicaid plans, Center for Medicare Services (CMS), Office of the Inspector General (OIG) and the Health Care Financing Administration (HCFA), as well as company and applicable professional standards.

  4. As assigned, compiles daily and monthly reports; tabulates data from medical records for research or analysis purposes.

  5. Works independently under regular supervision. Uses specialized knowledge for accurate assignment of ICD/CPT and MS-DRG codes according to national guidelines. May seek guidance for correct interpretation of coding guidelines and LCDs (Local Coverage Determinations).

MINIMUM QUALIFICATIONS

High school diploma/GED or equivalent working knowledge and specialized formal training equivalent to the two year certification course in medical record keeping principles and practices, anatomy, physiology, pathology, medical terminology, standard nomenclature, and classification of diagnoses and operations, or an Associate's degree in a related health care field.

Must demonstrate a level of knowledge and understanding of ICD and CPT coding principles as recommended by the American Health Information Management Association coding competencies, and as normally demonstrated by certification by the American Academy of Professional Coders. Six months providing coding services within a broad range of health care facilities. Must be able to achieve an acceptable accuracy rate on the coding test administered by the hiring facility according to pre-established company standards.

Must be able to work effectively with common office software, coding software, and abstracting systems.

PREFERRED QUALIFICATIONS

Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT), Certified Coding Specialist (CCS), Certified Professional Coder (CPC) in an active status or or Certified Coding Specialist-Physician (CCS-P) with American Health Information Management Association or American Academy of Professional Coders is preferred. Will consider experience in lieu of certification/degree. Additional related education and/or experience preferred.
