Responsible for checking expiration dates, stock rotation and removal of excess inventory in managed inventory locations.
Orders, delivers and stocks supplies for assigned departments as scheduled, maintaining proper par levels and storage space on shelves. Accountable for reporting to supervision needed adjustments to inventory and par levels according to utilization.
Promotes teamwork and collaboration with co-workers, customers and leadership to problem-solve and promote innovative solutions to provide the right product at the right time at the right place.
Processes and delivers supply and equipment requests. Uses good judgment in responding to requests for specific patient items and determines the best method and timing of delivery; i.e., routine vs. stat.
Unloads, inspects and checks material and supplies arriving at the dock; noting special storage or handling instructions. Maintains appropriate documentation for tracked items; i.e., tissue.
As directed, enters patient charges and orders/documents using appropriate vendor websites as applicable.
Collaborates with Purchasing and Accounts Payable to resolve receiving and invoice discrepancies.

Handles all facets of outbound shipments in a cost effective manner, documenting applicable RGA and tracking information, while ensuring that all shipments are timely as required by our customer.

High School Diploma or G.E.D. Must have the ability to work independently and think critically to problem solve. Must have knowledge of Windows-based computer programs and the capacity for learning specialized software programs. Must be able to understand directions, communicate and respond to inquiries. Must have effective interpersonal, oral and/or written communication skills. Must maintain a good driving record to operate company vehicles as required by applicable business unit.
PREFERRED QUALIFICATIONS
Previous experience in materials management preferred.

RN Description:
The Registered Nurse assesses, plans, implements and evaluates the nursing care provided to a group of patients. This nurse leads the nursing team members in the care as directed by the attending physician and in collaboration with other health care providers in accordance with the philosophy and policies of ProMedica Acute Care Hospitals. Actively participates in the creation of an environment that fosters patient, family, physician and employee satisfaction. Ensures decisions made are based on patient and family centered care philosophy utilizing evidenced based practices, and focused on safety, customer satisfaction, and quality outcomes.
Scope of Service:
The Registered Nurse provides nursing care to individuals and groups which require specialized knowledge, judgment, and skill derived from the principles of biological, physical, behavioral, social, spiritual/cultural, and nursing sciences. The Registered Nurse functions within the full scope of nursing practice as noted by the Board of Nursing of their designated state for which they are practicing in addition to compliance with any of the hospitals accrediting bodies.
POSITION SUMMARY Under general supervision and according to the established policies and procedures, coordinates total nursing care for patients, participates in patient and family teaching and provides leadership by working cooperatively with ancillary nursing and other patient care team personnel in maintaining standards for professional nursing practice in the clinical setting. The Wound Care Registered Nurse provides and coordinates patient care, collecting patient history and wound information as needed in the development of the plan of care. Educates both patients and their caregivers regarding the plan of care. Functions as case manager and team member as assigned for a specified group of patients. Education: Degree from an accredited nursing program. License: Must possess current licensure as a Registered Nurse in the state of Ohio Certification: Current BCLS certification. Specialty certifications as required.
Physical Demands: Due to continuously changing patient census/activity and staffing needs, must have the flexibility to work a varying schedule should the need arise, which may require working a varying schedule including shift, overtime (greater than 40 worked hours per week) and/or shift durations that exceed the assigned status for shift length and/or hours per week. Must move about for extended periods of time and must move, position, and transfer patients weighing in excess of 100 pounds.
Participate in usage and maintenance of equipment and instruments for safe patient care. Promote clean and safe physical environment including fire and safety awareness. Participate in performance improvement activities of patient care by monitoring and evaluation.

- The RN in Ohio must complete twenty-four contact hours of continuing education during his/her licensure period. At least 1 hour of contact hours must be related to Chapter 4723 of the Ohio nurse practice code and rules.

POSITION SUMMARY

ACCOUNTABILITIES
Reports for scheduled shifts on time and in a professional manner with regard to behavior, telephone courtesy, dress, identification, and communication with patients' colleagues, and outside agencies.
Works closely with the provider and leadership to continually improve the patient experience though daily patient care, clerical support, ongoing projects and assumes additional duties as needed in a self-directed manner.
Prepares the patient for the visit by taking vital signs, documenting and order entry within the EPIC system, arranging and coordinating support and ancillary services for patient care, preparing patient rooms for visits, stocking exam rooms, preparing schedules, and providing general support to the providers and patients. Performs specific specialized tests (i.e. pulmonary function tests, phlebotomy procedures) as needed.
Provides for a safe patient experience through adherence to clinical protocols, regulatory requirements, manufacturer standards and reports any potential equipment malfunctions or safety violations.
May support a variety of clerical office functions including calls, documentation, scanning, etc.
Within scope of Medical Assistant, demonstrates clinical knowledge and focuses on customer service to answer and triage patient telephone calls. Records complete and accurate messages and obtains necessary instructions from providers and nurses, etc. when appropriate.
Supports internal and external customers, to maintain a superior customer experience across the continuum of care through teamwork. Communicates professionally with patients, providers, and co-workers at all times. Actively participates and is engaged in staff meetings.
Collaborates with leadership and other staff to recommend changes to improve the safety, quality, and efficiency of patient care.

Education: Must be a high school graduate or its equivalent.

A Licensed Practical Nurse functions under the direction of a Registered Nurse to assist in the planning and delivery of the care required for a patient or group of patients, complying with established standards of practice in conjunction with all members of the health care team.
Demonstrates knowledge of depressive symptoms, suicide risk factors, and indications of potential for violence to others. Recognizes signs of hallucinations and delusions. Demonstrates therapeutic verbal and nonverbal communication skills including, but not limited to clarity, proper timing, active listening, attending, paraphrasing, utilizing silence, redirection, kinesics (body movement), paralanguage (voice quality and nonlanguage sounds), proxemics (use of personal and social space), and touch. Demonstrates knowledge of components of therapeutic milieu. Knowledge of potential side effects and adverse effects of psychotropic and anxiolytic medications. Provides supportive and educational counseling related to stress management skills, communication styles, medication management, and activities of daily living. Demonstrates knowledge of all aspects of the use of restraint and seclusion (appropriate use, proper application, monitoring according to policy and accurate/complete documentation.) Demonstrates therapeutic crisis intervention skills with the acting out patient. Has knowledge of and utilizes de-escalation techniques, i.e. appropriate responses to patient behavior level -anxious/supportive; verbally acting out/verbal and directive; physically acting out/physical response appropriate to situation; cooling off/therapeutic alliance and debriefing of incident. Recognizes the importance of hearing, vision and mobility problems in the care of the geriatric patients. Considers the developmental level when providing care to the mentally disabled. Demonstrates ability to recognize positioning needs in physically challenged patients. Uses appropriate methods to communicate with family/significant others including tone of voice, inclusion in patient's care (with patient's permission) as appropriate, and avoiding jargon.
Graduate of an approved school of practical nursing. Currently licensed LPN in the State of Ohio, including required documentation to administer medications. Maintains licensure, including continuing education to possess required CNE's. Basic knowledge of the biological, physical, behavioral, social, and nursing sciences. CPR certification (Healthcare Provider CPR). Demonstrates productive interpersonal and human relationships, service orientation, and guest relations.

Position Summary:
Prepares, seasons, and cooks food for hospital's patients, employees, and visitors. Performs lead role for kitchen service activities and the delivery of food in consultation with the Chef, Managers, Supervisors, and Clinical Dietitians.
Accountabilities:
Applies the knowledge and skills necessary to provide care appropriate to the age of the patients/clients served, i.e. infant, pediatric, adolescent, adult, and geriatric. Prepares, seasons, and cooks food. Cuts, pours, measures, and mixes ingredients according to recipes. Uses a variety of kitchen utensils and equipment, including choppers, slicers, mixers, and blenders. Previews menus and work orders to determine type and quantities of meals, vegetables, soups, salads, and desserts to be obtained and prepared. Responsible for menu selection and completion. Plans and coordinates cooking schedules so that meals will be ready at required times. Assist with care and maintenance of department equipment and supplies. Alerts Manager to problems and needs concerning equipment and food supplies. Detects and reports spoiled or unattractive food, defective supplies/equipment, or other unusual conditions. Maintains established department policies, procedures, objectives, quality assurance programs, and safety standards. Ensures that sanitary levels are maintained throughout the food preparation process as well as throughout the workday. Performs other related duties as assigned or requested.
Education: High school graduate or GED. Culinary degree preferred, but not required.
Skills: Capable of operating kitchen equipment; have good math and communication skills; be able to read and speak English; have the ability to learn how to change and convert recipes as needed and to learn special diet production needs. The Candidate needs to be able to deal effectively with a fast-paced environment and be open to new ideas and be willing to take instruction. Must have a basic understanding of culinary basics: cooking temperatures and proper labeling/rotation of food product.
Years of Experience: 1 year in food production preferred.
Certification: Food Handler Card or Serv Safe preferred, but not required.

POSITION SUMMARY
The Central Sterile Technician performs a variety of cleaning, assembling, and processing duties for reusable instruments and equipment. Performs various patient care activities and related non-professional services to assist nursing personnel. Cleans and disinfects/sterilizes endoscopes according to manufacturer's instructions.
Appropriately and consistently uses proper personal protective equipment while performing duties.

Under immediate supervision, provides counseling, program treatment management and supervision to adult patients. Conducts individual and group counseling. Functions as an active participant in reviewing patients' diagnoses and prognoses. Monitors patients' response(s) to treatment. Records progress notes in charts in a timely manner. Updates treatment plans regularly and makes revisions as needed. Reviews appropriateness of any psychosocial restrictions. Conducts lectures as assigned, leads patient discussions of video presentations, documents progress or revises multi-disciplinary treatment plans to be reviewed separately with patients. Shares expertise with staff as needed and makes appropriate referrals.
Provides information on treatment concepts, goals, and strategies to patients and family/caregivers with proper consent.
Maintains contact with referral sources for patient progress reports.
Maintains confidentiality of all patient information.
May be assigned duties and responsibilities dictated by program leadership consistent with the professional activities of a chemical dependency counselor. Duties may include specialized counseling activities such as family therapy, large group therapeutic sessions on weekends, etc. Provides information to treatment team members and assists them in their functions as appropriate.
May be assigned liaison responsibilities between the department of psychiatry and external agencies.
Performs public presentations, community liaison functions, and other duties as directed by psychiatric leadership.

Position Summary:
The RN Acute Care Navigator is responsible for direct patient care focusing on: Care Progression/Care Coordination, Level of Care, Length of Stay, Readmission Prevention, Value Based Programs, Daily Transition Rounds (DTRs), Discharge Planning and compliance for their assigned caseload to ensure appropriate patient throughput. Compliance requirements include but are not limited to: Maintains working knowledge of Condition Code 44 Intervention, Second IMM, MOON/Observation Status notification, Advanced Directives, Beneficiary notices, and Patient Choice. The RN Acute Care Navigator is responsible for collaborating with patient's care team (Physicians, Nurses, Social Workers, Ancillary Services, Care Navigation Resource Center Coordinators, Contracted Vendors, etc.) and escalating appropriately to ensure their assigned patient receives exceptional care and avoids unnecessary delays in care progression or discharge.

Conducts in person Initial Assessment with patients/caregivers including identification of patient decision maker as appropriate, with goal of Initial Evaluation completion within 24 hours of admission.
Develops Discharge Plan (with associated contingency plan) within 24 hours of admission; updates, as appropriate.
Assesses patients to determine ability for self-care and to identify those most at risk for post discharge adverse health consequences without intensive discharge planning. Provides a discharge planning evaluation to those patients identified as at risk and upon the request of the patient, key stakeholders, members of the interdisciplinary team or the physician.
Conducts a comprehensive assessment of the patient's physical, psychosocial, spiritual, environmental, and caregiver status to identify post-hospitalization needs. Documents all findings in the EMR. Identifies patients most at risk for readmission without intensive discharge planning through information gathered on the admission nursing database, electronic medical record (EMR) predictive analytics tools, and proactive case finding.
Completes Readmission Assessment on readmitted patients.
Shares Readmission Risk score daily during DTRs; collaborates with interdisciplinary team to identify high risk patients whose risk score may not have indicated appropriately; implements interventions according to risk score.
Identifies transitional care barriers and collaborates in comprehensive, patient-centered care plan development. Reassesses patients and revises the plan as applicable.
Implements Discharge Plan; inclusive of Discharge Plan communication and confirmation and includes patients/caregivers in Discharge Plan development to gain participation, agreement, and accountability.
Stratifies assigned patients by Clinical, Financial, and Psychosocial risk factors and submits follow-up referrals, as appropriate.
Consults Social Work for complex discharges and psychosocial/SDOH needs.
Develops, documents, and communicates Care Coordination Plan, updates as appropriate.
Collaborates with UM team, as appropriate and applies clinical understanding of medical necessity criteria, patient status and discharge criteria and assists UM team by relaying potential changes in medical necessity/appropriate patient status/LOC.
Reviews necessary patient information, including lab and other test results and progress notes in patient health record daily.
Collaborates and actively engages patients and key stakeholders throughout interdisciplinary progression and coordination of care along with the discharge planning process to ensure a patient-centered plan and document accordingly.
Partners with physicians as appropriate for care progression, care coordination and appropriate length of stay and collaborates with other key team members to manage transitional care activities and communicate vital information.
Actively participates in DTRs and facilitates discussion of progression and discharge needs.
Establishes initial Estimated Discharge Date (EDD), updates, as appropriate.
Discusses EDD with patient and/or caregiver(s).
Validates EDD with care team (inclusive of Attending Physician).
Validates Discharge Plan with Interdisciplinary Team (Physician, Nursing, etc.); updates as needed.
Escalates issues to appropriate level of Care Navigation leadership and coordinates mitigation activities in a timely manner as needed.
Provides supplementing patient-centered education to patients and key stakeholders regarding disease processes, medications, treatments, diet, and nutrition, expected symptoms and when/how to seek additional help. Utilizes motivational interviewing and teach back techniques as appropriate.
Coordinates patient access to necessary services, including community and public health resources. Assists patients/caregiver in selecting a post-acute care provider by sharing data on quality measures and resource use measures that are relevant and applicable to the patient's goals of care and treatment preferences.
Ensures assigned patients have an identified Primary Care Physician (PCP)/Specialist and follow-up appointment (or appropriate follow-up plan) scheduled prior to discharge; if PCP not identified, exhausts all efforts to assign.
Ensures effective communication through the continuum to support ongoing progress toward identified outcomes.
Communicates necessary medical information to appropriate facilities, agencies or outpatient services for follow-up or ancillary care, including all essential medical information.
Serves as a supportive resource regarding payor information; educates interdisciplinary team and patients/caregivers regarding payor requirements and/or barriers.
Works in partnership with acute and ambulatory care team to follow patient through care continuum and ensures thorough hand-off to ambulatory Care Navigators/Care Managers or post-acute caregivers. Serves as an essential link for patients without a primary physician.
Ensures timely delivery of second IMM notification, as appropriate.
Maintains positive working relationships with all internal and external customers.
Attends applicable conferences, trainings, and meetings. Participate in quality improvement and strategic initiatives.
Completes special projects and other duties, as assigned. Required Qualifications:
Education: Must have either a BSN, or 5 years of applicable nursing experience.
Skills: Must have a history of positive rapport with patients, families, physicians, and interdisciplinary team members. Relevant experience working with diverse populations. Must have exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in a self-directed environment with attention to detail and follow-through. Must be able to function effectively in a critical care environment. Able to establish priorities and respond to inquiries. Critical thinking skills. Works as a team player with the interdisciplinary team to reach care goals. Ability to manage multiple tasks and prioritize levels of importance. Ability to manage assignments with minimal supervision, work as a part of an interdisciplinary team, and focus on optimal patient and department outcomes. Performs duties in a manner to promote quality patient care and customer service/satisfaction, while promoting safety, cost efficiency, and a commitment to ProMedica principles.
