Job Description

The Clinical Supervisor will direct, supervise and coordinate staff and activities at assigned clinics to ensure the delivery of quality patient service. This position ensures staff complies with all regulatory initiatives and policies and procedures and works with senior management to support effective clinic operations. Location: This position will be based primarily out of our Fairlawn office but will also oversee multi-specialty offices in Streetsboro and Hudson.

Schedule: Monday - Friday, 8:00 a.m. - 5:00 p.m.

Primary Responsibilities:

  • Supervises assigned personnel to ensure quality patient care, including forms and procedures as required

  • Monitors clinic workflow and schedule to ensure productivity standards are met

  • Administers clinic policies and procedures in a consistent and timely manner

  • Evaluates performance and recommends merit increases, promotions, and disciplinary actions, assists in interviews, trains, and orients staff

  • Ensures that office space, supplies, assistance are provided and maintained appropriately for medical staff and patient care

  • Works with senior management to ensure operations and relations are effective

  • Attends required meetings and participates in committees as requested

  • Ensures personnel forms and/or records are kept updated and submitted to other departments in a timely manner

  • Ensures that offices for new and incoming physicians are ready and equipped

  • Coordinates staff work activities and serves as resource to medical management staff in various market(s)

  • Collects clinic performance and quality metrics and produces reports for management

  • Orders office and medical supplies and ensures sufficient quantities are maintained at all times. Assists the Clinic Administrator in budget preparation and adherence

  • Coordinates conference calls and assist with preparation for meetings for medical management staff in market

  • Performs all other related duties as assigned

Required Qualifications:

  • High School Diploma or GED

  • Professional healthcare license or certification

  • 2+ years of clinic operations or healthcare experience, including 1+ years of supervisory experience (Comparable leadership/internship experience may be substituted in lieu of supervisory experience)

  • Knowledge of organization policies and procedures and ability to apply principles to solve everyday problems in a variety of situations

  • Knowledge of fiscal management and human resource management techniques

  • Knowledge of office management techniques and practices

  • Working knowledge of Microsoft Office applications, including Excel

  • Computer literacy

  • Proven ability to exercise initiative, problem solving, decision-making

  • Proven ability to establish priorities and coordinate work activities

  • Completion of a medical assistant training program and/or 1+ years of medical assistant / EMT experience in an urgent care, physician, or emergency room setting, or enrolled in an accredited Radiologic Technologist program

  • Must meet all other state requirements by law, including but not limited to all state requirements, and licenses, where applicable

  • Active CPR/BLS certification or ability to obtain within 90 days of hire

Preferred Qualification:

  • Direct experience with: Clinical intake, patient data collection, laboratory specimen collection, performing EKG testing, application of orthopedic devices, performing basic wound care, and/or employer health services

As a member of the broader Home and Community Care team, you'll provide annual clinical assessments to patients in the comfort of their homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being.

This role requires travel across the entire state of New York, approximately 85% of the time. All travel expenses are covered, and there is likewise a base salary plus multiple incentives.

Primary Responsibilities:

  • Conduct in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam

  • Perform evidence-based practice screenings including point of care testing (as appropriate)

  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment

  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment

  • Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care

  • Identify urgent and emergent situations and intervene appropriately

  • Educate members on topics such as disease process, medication, and compliance

Required Qualifications:

  • Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice

  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in state of assignment by start date

  • Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand and kneel to perform physical assessment

  • Reliable transportation to complete home visit assessments

Preferred Qualifications:

  • 1+ years of clinical experience in their highest level of education, clinical setting

  • May be requested to obtain additional licensure in other geographic areas

  • Experience in gerontology, cardiology, internal medicine, or endocrinology

  • Home Health care or home visit experience

  • Ability to travel throughout the state, beyond your assigned region, up to 5% of the time. Based on business needs with advanced notice, eligible for additional compensation incentives

*PLEASE NOTE* Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
