This role requires travel across the entire state of New York, approximately 85% of the time. All travel expenses are covered, and there is likewise a base salary plus multiple incentives.
Primary Responsibilities:
Conduct in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
Perform evidence-based practice screenings including point of care testing (as appropriate)
Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care
Identify urgent and emergent situations and intervene appropriately
Educate members on topics such as disease process, medication, and compliance

Required Qualifications:
Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in state of assignment by start date
Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand and kneel to perform physical assessment
Reliable transportation to complete home visit assessments
Preferred Qualifications:
1+ years of clinical experience in their highest level of education, clinical setting
May be requested to obtain additional licensure in other geographic areas
Experience in gerontology, cardiology, internal medicine, or endocrinology
Home Health care or home visit experience
Ability to travel throughout the state, beyond your assigned region, up to 5% of the time. Based on business needs with advanced notice, eligible for additional compensation incentives
*PLEASE NOTE* Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The Associate Patient Care Coordinator is responsible to the Center Administrator and provides appointment scheduling, patient registration, cashiering, answers incoming calls and performs other related clerical functions. This primary role provides front office support functions.

Primary Responsibilities:
Greets patients as they arrive and manage appropriate standard wait times
Complete check-in and check-out tasks to include patient demographic verification, insurance verification, complete pre-authorizations and/or precertification of procedures, and copy required documents
Collects co-payments, co-insurance, and deductibles and issues receipts
Manages cashier box and daily deposits according to company policies
Answers phones and schedules appointments
Answer telephone calls, re-direct calls as appropriate, assist callers with questions or concerns, and take messages as needed
Manages medical records (maintains, files/scans, prepares for schedule)

We seek a Float Senior P harmacy Technician to support all functions of the Genoa mental health and specialty pharmacy primarily through dispensing medical prescriptions and performing necessary clerical duties while under the direct supervision of a registered pharmacist.

Primary Responsibilities:

The Medical Assistant (MA) participates in providing patient care at the appropriate skill level. They have a duty to provide a standard of care that meets or exceeds that of a reasonably competent and knowledgeable Medical Assistant. Primary Responsibilities:
Performs all duties within the scope of a Medical Assistant's practice. Operates diagnostic equipment (cannot interpret tests), remove staples from superficial wounds, changes wound dressing and obtains cultures, administers non-intravenous medication, performs simple specimen collection via noninvasive techniques and collects blood specimens via venipuncture or via capillary, performs EKGs. Performs quality control checks on equipment. Prepares and sterilizes medical equipment using the autoclave
Rooms patients according to policy and procedures, prepares patient for examination
Records patient care documentation in the medical record accurately and in a timely manner
Coordinates patient care as directed by physicians, company standards and policies
Respects patient confidentiality at all times and treats patients with courtesy and respect
Organizes exam and treatment rooms, stocks and cleans rooms and sterilizes instruments
Practices standard infection control precautions
Telephone and in-person screening limited to intake and gathering of information without requiring the exercise of judgment based on clinical knowledge
Supports and follows Standard Delegation of Orders (SDO)
Performs all other related duties as assigned

Required Qualifications:
High school graduate or GED equivalent
Current, nationally recognized Medical Assistant certification or the ability to obtain the certification within 180 days of employment. Medical Assistants who are hired prior to receiving their certification are expected to perform at the same level as a -certified- Medical Assistant
Current BLS certification for healthcare providers (written exam and in-person assessment) at time of hire or within 30 days of hire
Basic computer literacy, with intermediate Excel skills
Knowledge of medical terminology
Proven ability to react calmly and effectively in emergency situations
Proven good communication and customer service skills
This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
Preferred Qualifications:
1+ years of experience as a Medical Assistant
Knowledge of ICD-10 and CPT coding

Purpose of Job:
