POSITION SUMMARY

This position develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.

CORE FUNCTIONS

  1. Processes and facilitates the assessment with analysis of functional and psychological needs of the patient within the framework of his/her developmental stage, functional abilities, cultural milieu, and support network. Assessment of the relationship of the patient's medical needs to the patient's home situation, financial resources, and availability of community resources. Assessment of the social and emotional factors related to the patient's illness, need for care, response to treatment, and adjustment to care. Assessment includes: initial assessments, behavioral observations, test (MMSE, depression screening, substance abuse screening), resource identification - strength based, collection and analysis of information to determine an individual's treatment needs.

  2. Formulates a plan of intervention acceptable to the patient, family, and health care team. Facilitate adjustments to the plan of care when necessary to promote enhanced outcomes. Collaborates with all members of the healthcare team to develop, manage, and communicate patient needs and discharge plans.

  3. Documents all interventions in the patient medical record both timely and accurately including all elements of the transitional care plan to include the discharge plan.

  4. Provides advocacy, assistance, support, counseling and crisis intervention to patients and families. Facilitates hospital-sponsored support groups.

  5. Functions in a liaison role between the hospital and community in making community resources available to the patient and family.

  6. Maintains knowledge of Medicare, Medicaid and other program benefits to assist patients with discharge planning and choices. Knowledge of contemporary behavioral health and system theories relevant to health care, end of life dynamics, and interventions; grief and bereavement counseling, substance abuse identification and interventions, support of victims of abuse, neglect, or violence. Provides professional education to staff and community.

  7. Gather / assessing information, apply counseling and developmental theories, utilizing diagnostic frameworks, and engage in collaborative treatment planning.

  8. Has freedom to determine how to best accomplish functions within established procedures. Confers with supervisor on any unusual situations. Positions are facility based with no budgetary responsibility. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Home health agencies, nursing homes, insurance providers, volunteer services, county and governmental agencies and medical supply companies.

MINIMUM QUALIFICATIONS

Requires a Master's Degree in Social Work, Counseling or related field (requirement is based on business need and regulatory compliance, all positions may not have this requirement).

Requires a Licensed Master Social Worker (LMSW) (equivalent*) or Licensed Clinical Social Worker (LCSW) or have a MSW with the requirement to become licensed within 6 months of hire date. An equivalent license applies to states that do not recognize an LMSW; therefore, the employee must possess a Master's Degree and be a Licensed Social Worker. For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.

Requires a proficiency level typically achieved with 3 years acute care hospital experience. Banner Registry and Travel positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Social Worker MSW in the specialty area.

Must have knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, human relations skills and time management skills. In acute care, must be able to work flexible hours and take rotating call after hours.

POSITION SUMMARY

CORE FUNCTIONS

  2. Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation.

  3. Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols.

  4. Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes.   6. Provides leadership in area of coverage, monitors proper patient care procedures and acts as liaison with nursing areas. May participate in process improvement teams. Serves as mentor to new staff and interns where assigned.

  7. Participates in continuous quality improvement activities in clinical areas to ensure optimal nutrition care outcomes are achieved. May participate in outcome studies as needed. In some roles, this position may manage assigned staff: recruits, selects, trains, evaluates, manages performance.

  8. This position interacts with patients, families, health care providers and nutrition and dietary staff in support of providing high quality nutrition assessment, care planning, delivery and measurement of outcomes. POSITION SUMMARY

Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.

  6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).

  7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.

  8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.

  9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. POSITION SUMMARY

This position performs all functions related to the receiving, shipping, and distribution of materials and supplies for the hospital central supply department.

CORE FUNCTIONS

  1. Receives incoming orders and verifies orders are accurate and intact, including special orders. Documents and communicates damage, shipping errors, and/or other discrepancies. POSITION SUMMARY

POSITION SUMMARY

This position is responsible for providing patient care and assists health care providers in performing assigned patient procedures within scope of training. Position receives general supervision from RN, APN, PA, or Physician in providing patient care.

CORE FUNCTIONS

  1. Applies orthopedic devices, to include pre-manufactured and plaster splints under physician direction.

  3. Performs specimen collection, ensuring accuracy in identification and integrity by using appropriate techniques for all age criteria as required by Lab. Where state statute permits, performs point of care testing for blood glucose, hemoccult and gastroccult or other tests, and performs quality control checks on point of care equipment as required.

  4. Provides communication and updates to RN, Physician or other providers related to perceived reactions or changes to patients' condition. Accurately records information in the patient record, documents vital signs and procedures completed, as well as the equipment and techniques utilized.

  5. Monitors supply stock levels and reports needs. Notifies housekeeping, maintenance, biomedical engineering, and others to ensure proper equipment and a safe environment is maintained. Maintains appropriate levels of stock in medication room, exam room bedside carts, and specialty carts. Assists with health unit coordination tasks and reception responsibilities as needed.

  6. Attends department meetings and educational sessions; Assists in orienting and training of new ED Technicians.

  7. Position receives general supervision from RN, APN, PA, or Physician and other appropriate licensed health professionals. POSITION SUMMARY

This position works to ensure the efficient and effective operation of the assigned department, including day-to-day administration of non-physician staff and operations. Uses specialized knowledge, judgment and skills necessary to provide excellent patient care.

CORE FUNCTIONS

  1. Supervises the day to day activities of non physician staff, ensuring a high level of productivity. Continuously monitors the functioning of the medical group and recommends changes to operational procedures as necessary.

  2. Selects, trains, coaches, motivates, conducts performance evaluations, and directs the workflow of staff. Schedules non physician staff ensuring that staff is appropriately deployed among offices.

  3. Oversee all functions of the medical group including reception and scheduling activities, patient service areas, accounting, medical records, transcription, business information systems, and appropriate clinical areas.

  4. Functions as a liaison between non physician staff and physicians to ensure effective service for patients. Establishes and maintains effective working relationships with group physicians. Serves as a role model and resource person for providing quality patient care and patient information as needed.

  5. Maintains office policies and procedures in accordance with physician requirements, facility and system policies and procedures, standard practices of the profession, and related regulatory requirements.

  6. Determines physical maintenance and equipment needs and repairs and coordinates internal or external resources. Resolves problems relating to the facility on a day-to-day basis. Ensures all areas are appropriately stocked and supplies are readily accessible.

  7. Covers front and/or back office functions. May also administer medications in accordance with established policies and procedures or as directed, and consistent with demonstrated competencies and state scope of practice.

  8. This position has a strong understanding of the daily workflow of the practice and functions autonomously to provide guidance to ensure quality patient experiences and efficient practice operations. This position is required to have working knowledge of referral and authorization processes and must have the ability to navigate a variety of separate computer programs at any given time and handle multiple demands on time. Internal customers include all patients, staff and clinic providers. External customers include the community, vendors and non-clinic providers and facilities.

MINIMUM QUALIFICATIONS

Requires a level of literacy, communication and math skills as normally demonstrated at the level of an Associate's Degree.

Certain assignments may require current certification as a Medical Assistant, Nursing Assistant or current LPN license.

Requires a level of knowledge and ability normally demonstrated by at least two years experience in a physician's office or medical environment. Requires excellent communication and organizational skills.

POSITION SUMMARY

POSITION SUMMARY

The Acute Patient Access Services Representative registers patients for the main admitting team, obtains and verifies patient demographic and insurance information, verifying eligibility, generating patient estimates based on benefits & services provided, entering data into ms4 and obtaining signatures on consent forms. This position is goal oriented. Metrics measured include Accuracy, Productivity (# of patients registered in an hour) and point of service collections. POSITION SUMMARY

This position is the first point of contact at healthcare facilities and assists patients with the administrative aspect of gaining access to medical treatment. Our inpatient medical oncology unit also incorporates a program that utilizes the electronic surveillance partnership in caring for the patient, where remote nurses have the ability to interact with patients via two-way audio-video to assist the bedside nurse with patient care.

POSITION SUMMARY

This position will work under the supervision of the pharmacist performing routine duties in packaging, distribution, non-sterile compounding, and record-keeping of pharmaceutical supplies. Serves as a liaison between the customer and the pharmacist. Effectively uses pharmacy computer systems to investigate and respond to missing medication requests. Consistently demonstrates an ability to assess a situation, consider alternatives, and choose the appropriate course of action. Demonstrates proficiency in preparing compounded intravenous medications, total parenteral nutrition products, and chemotherapeutic agents within compliance with USP <797>/<800>. Ensures that the sterile compounding environment and all equipment are cleaned and maintained according to policy. May educate and train peer Pharmacy Technicians, Technician Externs, Technician Trainees, and Volunteers. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients.

CORE FUNCTIONS

  1. Identifies medications and other items for reorder as necessary to maintain a continuous inventory of medications and supplies. Receives, checks-in, and restocks medications ordered through the wholesaler and other vendors verifying all quantities against invoices. Assesses practical ways to deal with shorted items and short-dated or unusable items.

  2. Manages users within the automated dispensing technology database as per policy. Investigates discrepancies and troubleshoots malfunctions in automated dispensing technology.

  3. Exhibits excellent interpersonal skills and is customer service oriented in all contacts with patients, visitors and staff. Responds to standard requests from internal customers. Interacts primarily with department personnel, cross-department personnel, internal customers, patients and physicians.

  4. May use automated dispensing technology or requisition to restock medications to nursing and ancillary areas after checking by a pharmacist. When allowed by law and policy, and upon completion of proper training/competency assessment, may restock automated dispensing technology using barcode scanning verification devices. Restocks medications to all areas outside automated dispensing technology which may include but not limited to trays, code carts and emergency boxes.

  5. Compounds non-sterile medication products. Understands and appropriately follows all product worksheets/recipes for documentation and check processes. Understands and adheres to all applicable USP compounding guidelines. Demonstrates knowledge of and consistently performs calibrations of equipment and accurately measures components for compounding. Repackages bulk medications into unit of use packaging. Completes accurate documentation of medications compounded and repackaged per policy.

  6. Maintains a safe and clean working environment by complying with all policies, laws, and regulations. Performs quality control activities such as monthly nursing unit inspections and pharmacy section checks in a timely manner. Completes documentation of refrigerator temperatures, and equipment maintenance activities as required.

  7. Ensures the proper use of garbing and compounding techniques in accordance with policies and has knowledge of basic principles within USP <797> and USP<800>, laws, policies, and procedures that pertain to compounding. Performs compounding and labeling of intravenous compounds and hazardous agents to include chemotherapy using proper sterile and aseptic technique as appropriate to the site of care. Understands the different product lines in sterile compounding (patient specific and non-patient specific batch) and the differences between the various populations (Adult, Pediatric, Neonate). Completes all required environmental cleaning procedures. Documents all required tasks and other information per policy. Understands the safe handling of hazardous drugs and demonstrated the correct receipt, storage, labelling and transport process.

  8. Handles narcotic medications following all federal and legal requirements as set through policy and procedure. Understands all regulatory requirements for receiving, compounding, repackaging, and distribution for controlled substances. Consistently adheres to all storage and inventory movement documentation requirements for controlled substances.

MINIMUM QUALIFICATIONS

Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice.

Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.

Minimum of two years pharmacy technician experience. Ability to complete moderate to difficult mathematical calculations using a hand-held calculator is required. Strong customer service focus, exceptional communication skills, strong attention to detail, prioritization capabilities, superior problem-solving skills, and ability to multi-task. Must have knowledge of computer software, particularly medication management information system software, Microsoft applications and automated dispensing systems. Must demonstrate effective oral and written communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.

Minimum Qualifications:

  • MD or DO with an Arizona license by the start date

  • ABNS-certified or eligible Neurosurgeon

  • Open to both experienced and newly graduating neurosurgeons
