Ogallala Community Hospital combines tradition and technology in the charming small town of Ogallala, Nebraska. POSITION SUMMARY
This position provides leadership and training for environmental services associates, while working as a member of the EVS Associate team.
CORE FUNCTIONS
Ensures that staffing plans are carried out properly, and that all areas of the hospital/facility are staffed appropriately. With supervisor, plans completion of hospital wide projects, ensures consistent service delivery standards in all aspects of the Environmental Services and Linen operations. Gives input to supervisor on quality and timeliness of work performed by environmental services staff.
Performs cleaning functions in patient rooms, specialty areas and common areas.
Works closely with supervisor to train and retrain, recruit, retain and develop the Environmental Services team members, fostering a culture that reflects the core values and operational polices of our department and the company.
May supervise staff in the absence of the Environmental Services leader. Makes work assignments and responds to immediate needs by moving staff to various areas of facility as needed. Moves furniture and sets up rooms for meetings and special functions.
Assists supervisor in performing Quality Assurance Inspections for review and correction of deficiencies.
Distributes linens and supplies as needed; takes inventories in assigned areas.
Works independently with general supervision. Department responsibility only. Customers include staff, patients, visitors, physicians and vendors. Oversees daily activities of a team of Environmental Services Associates. Makes staffing assignments in absence of supervisor; coordinates cleaning projects as assigned, assists with on-the-job training, provides input to performance evaluations.
MINIMUM QUALIFICATIONS
Requires the skills, knowledge and abilities in healthcare environmental services work typically acquired over two or more years of related work experience. Must have leadership skills and be able to communicate effectively verbally and in writing. Requires the ability to work independently with minimal supervision. Must serve as a role model to others in customer service skills, safe work practices, and infection control including hand hygiene. Employees working at Banner Behavioral Health Hospital or BTMC Behavioral must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
PREFERRED QUALIFICATIONS
Basic computer skills in word processing and electronic mail are preferred.

At Banner MD Anderson Cancer Center Radiation Oncology Clinic at Banner Gateway, our physicians use the most up-to-date research standards and radiation technologies to provide the safest, most accurate cancer treatments. Among the technologies used is 4D-computerized tomography that lets physicians see and account for the movement of tumors inside the body. The center also houses highly sophisticated linear accelerators and patient positioning devices that ensure exact radiation delivery.
As a Registered Nurse in Radiation Oncology , you have the opportunity to work alongside a team dedicated to providing patients a positive experience. You'll assist the provider by completing a brief patient assessment, provide education materials related to radiation and medication. You'll perform skin/wound care, IV placement, specimen collection and provide Patient Navigation.

Located near Phoenix in Gilbert, Ariz., Banner Gateway Medical Center offers 286 private rooms, 13 operating suites, a 46-bed emergency department and shares a campus with the Banner MD Anderson Cancer Center. Our WIS and NICU services support an average of 4,000 deliveries per year. Key specialties include oncology, obstetrics, bariatric surgery, emergency and other services that focus on meeting the changing needs of the dynamic and growing community we serve.
Banner MD Anderson Cancer Center Located in Gilbert, Ariz. (the Phoenix Metro area) on the Banner Gateway Campus, the center provides world-class care for oncology patients - both inpatient and outpatient - and has also brought leading oncology programs to the Banner Gateway campus including designation as a Stem Cell Transplant Center of Excellence and comprehensive Head & Neck cancer care. Our capabilities include five linear accelerator vaults, a brachytherapy vault, an advanced diagnostic imaging suite with PET/CT scan, more than 70 infusion bays, a cryopreservation lab and much more. Our inpatient medical oncology unit also incorporates a program that utilizes the electronic surveillance partnership in caring for the patient, where remote nurses have the ability to interact with patients via two-way audio-video to assist the bedside nurse with patient care.
POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.
Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient's status.
Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.
Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.
Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
In some roles, this position may supervise staff and work flow of the department.
9.Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.
In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license. Auto insurance coverage is also required to be active and maintained.
Employees working at Banner Behavioral Health Hospital, BTMC Behavioral, Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. Employees working at Banner MD Anderson on the Banner University Medical Center Phoenix campus must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. This is a requirement of the Whole Family Counseling Program held at this specific facility. POSITION SUMMARY
This position is responsible for scheduling across the campus for physicians, medical facilities, and specialized medical practices by coordinating all aspects of scheduling including, but not limited to, appointments for physicians across the campus, surgical procedures, diagnostic tests, pain management, and other specialized tests and treatment as directed by physicians to ensure an exceptional customer experience at each point of service. POSITION SUMMARY
This position produces all required food items based on information obtained from production sheets, catering requests, prep sheets and any other approved source. Plans and conducts daily work to meet food production and service time requirements.
CORE FUNCTIONS
Prepares and cooks hot and cold food items using standardized recipes and calculates production quantities based on production forecast, tally sheets and catering orders.
May be assigned to work in the retail cafe grill area, and/or cook breakfast items.
Records over production and run outs. Writes directions and calculates production quantities.
Checks and logs food and equipment temperatures and reports any problems to the supervisor. Reports all needed equipment repair.

MINIMUM QUALIFICATIONS
This position requires the knowledge and abilities in institutional food production normally acquired with one or more years of work experience in production cooking. Must be able to learn and follow established policies and procedures, read and understand written and verbal instruction, communicate effectively, and perform simple math calculations. Must be able to learn food handling regulations and pass certification tests as required. Must possess good interpersonal skills and be able to perform tasks within limited time frames. Must be able to follow production and cleaning schedules, use chemicals safely, read thermometers and follow safety requirements.

POSITION SUMMARY
This position actively participates on the healthcare team as a pharmacotherapy expert to provide quality patient care in area of specialty. Participates in the optimal selection of drug therapy and designs a regimen to maximize therapeutic benefit and minimize toxicity. This position is accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients. Conducts educational activities, applies principles of performance improvement, and participates in Pharmacy and Therapeutics Committee/Clinical Consensus Group activities as appropriate. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients being served. Accountable for departmental, and individual patient outcomes by assessing and evaluating data reflective of the patient's status, interpreting the appropriate information needed to identify each patient's requirements, and providing pharmaceutical care based on individual needs. Critically evaluates the management of disease states and adjusts or recommends adjustments to therapy to improve patient medication regimens. Ensures appropriate and cost-effective medication therapy. Areas of focus may include but are not limited to evaluating disease state management, medication management, and adverse drug reactions. Adheres to all clinical practices approved by Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g., collaborative practice agreements). Drug information expert in area of specialty and may act as a system resource. Provides detailed drug information and education to healthcare team, pharmacy, patients, and community. Provides education for pharmacy staff, students, and residents where applicable.
Reviews and evaluates medication orders for appropriateness and completeness with respect to patient data including age, height, weight, diagnosis, allergy, laboratory values and other pertinent patient information. Complies with directives of the Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g. therapeutic substitutions, formulary management).
Applies the principles of continuous quality improvement consistent with job expectations. Incorporates quality improvement principles into other activities and facility/system-based projects (i.e. data collection, documentation, med error and adverse drug event trending). Participates in drug use evaluations and facility level medication safety oversight.
Participates in local and system-based committees (e.g. area of specialty or other related area) and effectively communicates to stakeholders. Participates in the development of innovative drug-related patient care practices in their specialty at both institutional and system level. Serves as a resource and mentor to other pharmacists, students, residents, and other health-care providers at both the facility and system level.
Meets ASHP preceptor qualifications. Participates as primary preceptor to residents where applicable. When needed, serves as the Program Director for the institution's Pharmacy Practice Residency Program. Serves as an educator, resource expert, and mentor to other pharmacists, students, residents, and/or other healthcare providers.
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. MINIMUM QUALIFICATIONS
A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program. Without residency training a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management e.g. thromboembolic, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc).
Additional minimum qualifications include board certification through the Board of Pharmaceutical Specialties (e.g. BCPS, BCOP). Board certification must be active and maintained.
For pharmacists providing care in Investigational Drug Services, the ASHP Investigational Drug Service Certificate will be allowed. Candidates who are not Board Certified will be given 24 months to achieve certification.
Requires pharmacist licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. If not licensed in the state of practice, must have completed the Board of Pharmacy application process within 30 days of job offer. Must be licensed within 90 days of hire unless the delay is due to the State Board of Pharmacy. May be required to possess a valid driver's license and be eligible for coverage under the company's auto insurance policy.

POSITION SUMMARY
This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.

Prepares patient for exam and treatment by taking and recording vital signs, symptoms and other necessary measurements and recording chief complaint; documents pain scores as appropriate. Reports condition of patient which may be indicative of change in the patient's condition to the clinician.

Coordinates and manages patient care including pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Responsible for communicating test results to patient. Acts as a resource to clinician in order to provide optimal patient care. May administer medications in accordance with established policies and procedures or as directed.
Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Provides complete and accurate information necessary for billing related to the patient encounter in a timely fashion.
Maintains a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas. Disinfects equipment and instruments using appropriate solutions following sterilization procedures. Follows manufacturer's recommendations and OSHA guidelines in handling of hazardous substances. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.
Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).

This position works under the direct supervision of the providers. BGMC's Periop department consists of a 30 bed Perianesthesia unit supporting nine operating rooms, interventional radiology, and endoscopy. The OR specializes in oncology, bariatrics, robotics, urology, GYN, orthopedics and general surgery. The Periop department also provides support for Banner MD Anderson Cancer Center patients through IR and endoscopy procedures such as endoscopic bronchoscopy and ultrasound, biopsies and chemoembolization. The surgery department has a 25% same day add on case load with increasing patient acuities/complexities.

BGMC's Periop department consists of a 30 bed Perianesthesia unit supporting nine operating rooms, interventional radiology, and endoscopy. The OR specializes in oncology, bariatrics, robotics, urology, GYN, orthopedics and general surgery. The Periop department also provides support for Banner MD Anderson Cancer Center patients through IR and endoscopy procedures such as endoscopic bronchoscopy and ultrasound, biopsies and chemoembolization. The surgery department has a 25% same day add on case load with increasing patient acuities/complexities.

Banner MD Anderson Cancer Center Located in Gilbert, Ariz. (the Phoenix Metro area) on the Banner Gateway Campus, the center provides world-class care for oncology patients - both inpatient and outpatient - and has also brought leading oncology programs to the Banner Gateway campus including designation as a Stem Cell Transplant Center of Excellence and comprehensive Head & Neck cancer care. Our capabilities include five linear accelerator vaults, a brachytherapy vault, an advanced diagnostic imaging suite with PET/CT scan, more than 70 infusion bays, a cryopreservation lab and much more. Our inpatient medical oncology unit also incorporates a program that utilizes the electronic surveillance partnership in caring for the patient, where remote nurses have the ability to interact with patients via two-way audio-video to assist the bedside nurse with patient care.
POSITION SUMMARY
This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records and provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.
CORE FUNCTIONS
Provides assistance in activities of daily living and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed.
Collects patient specimens and data, including vital signs, input/output and other delegated measurements in accordance with unit/facility policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.
Recognizes and reports abnormalities and/or changes in the patient's health status and responses to treatment to a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.
Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and according to state scope of practice; procedures may include blood draws, foley catheters removal, EKGs, and EKG monitoring. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional.
Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.
Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient's record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record.
Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in OR room turnover.

POSITION SUMMARY
This position manages a complex caseload of patients by providing comprehensive nutritional assessments and nutrition-focused physical exams, developing and managing all aspects of appropriate nutrition care plans and monitoring outcomes of plans. Provides patient and family nutrition counseling and education. Develops Nutrition Standards of Practice and Education material in collaboration with peers. Serves as a member of the interdisciplinary Health Care team providing input into patient care policies, programs, quality improvement activities and provides in-service education in areas of expertise.
CORE FUNCTIONS
Provides comprehensive nutrition assessment of patients' nutritional needs, to include nutrition-focused physical exams as appropriate. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards and established practice guidelines.
Demonstrates and maintains competency in enteral and parenteral clinical practice protocols and oral nutrition management standard of practice as demonstrated by successful completion of initial competency exams and ongoing evaluation.
Writes orders to initiate and manage the nutrition care and of all assigned patients including those receiving nutrition support (parenteral and enteral nutrition) per protocols.
Evaluates effectiveness of the care plan and communicates effectively to the other members of the healthcare team to provide optimal nutrition outcomes. Provides pertinent information in the medical records, patient plans of care and/or clinical pathways while meeting requirements of regulatory agencies (i.e. Joint Commission, CMS, State and Local Agencies).
Provides nutrition education to patients, families, staff and other health care team professionals through the development of effective teaching materials and in-service programs; Evaluates patient and/or family response to education.
Provides leadership in area of coverage, monitors proper patient care procedures and acts as liaison with nursing areas. May participate in process improvement teams. Serves as mentor to new staff and interns where assigned.
Participates in continuous quality improvement activities in clinical areas to ensure optimal nutrition care outcomes are achieved. May participate in outcome studies as needed. In some roles, this position may manage assigned staff: recruits, selects, trains, evaluates, manages performance.
This position interacts with patients, families, health care providers and nutrition and dietary staff in support of providing high quality nutrition assessment, care planning, delivery and measurement of outcomes. Position communicates effectively and provides education to ensure optimum nutritional care. Position works independently and provides guidance to nutrition services staff in the development of nutrition plans. Position may also provide community education.
MINIMUM QUALIFICATIONS
Must possess knowledge of nutrition and dietetics demonstrated through the completion of a Bachelor's degree in Nutrition and/or Dietetics. Must be a Registered Dietitian or RD Eligible based on academic and professional requirements as defined by the Commission on Dietetic Registration(CDR) of The Academy of Nutrition and Dietetics. Effective January 1, 2024 CDR requires a minimum of Master's degree to be eligible to take the registration examination. Must possess RD credentials within 6 months of hire. Must hold current registration through the Commission on Dietetic Registration of The Academy of Nutrition and Dietetics and be able to maintain ongoing registration verification of registration annually. Maintains current licensure as required by State law.
Must be able to communicate effectively and function independently. Must be effective at prioritizing assigned patient case load.
