Summary of Primary Function/General Purpose of Position

Performs various general duties and assists in the provision of patient care of all age groups as directed by the Registered Nurse and Organizational Leadership. Provides direct care for patients, answers call lights, performs clerical/courier/transporter functions. Collects data through a variety of means including observation and through the use of instrumentation. Recognizes and communicates pertinent information to multidisciplinary teams to ensure prompt attention to patient and departmental needs.

Essential Functions

Assist patients with measures of comfort and activities of daily living, including but not limited to bathing, feeding, toileting, dressing, and grooming.

Assist patients with mobility by performing active and passive range of motion, lifting, turning, repositioning, ambulation, and transportation.

Performs routine patient care as delegated by the Registered Nurse including but not limited to obtaining specimens, recording vital signs, applying ice/heat/compress/ dressings, intake/output, point of care testing, 12 lead EKG, and reapplying monitors. May perform CPR. Explains procedures to patients allaying apprehension and eliciting cooperation. May assist in restraining combative patients.

Performs tasks and procedures independently after successful performance with a RN or ONLY under the direct supervision and responsibility of a RN including but not limited to changing peripheral IV site dressings, phlebotomy, removal of peripheral IV's, inserting and/or removing urinary catheters, performing simple trach care, administering non-medicated enemas, performing oral suctioning, changing ostomy appliances and performing ostomy care, and assisting with transferring patients to a higher level of care.

Assists nursing team in the process of admitting, transferring, and discharging patients, including notifying appropriate individuals and departments, assembles patient charts and related paperwork, assisting and performing environmental orientation of patients, preparing, stocking, cleaning of rooms, documenting historical information, and securing belongings and valuables.

Clerical skills/duties including but not limited to courier services, answering phones/call lights and orders, obtains, stocks, places, tests, discontinues, and removes patient equipment and supplies.

Communicates and Documents pertinent patient and general information and services provided.

Identifies, troubleshoots, and works with others as applicable to resolve issues and concerns.

Participates in hand-off/report of pertinent information regarding patient and departmental needs upon arrival and when leaving.

Other duties as assigned.

Job Summary:

Primary Function/General Purpose of Position

Responsible for providing coordination of care for patients to support safe, seamless, timely transitions across the continuum. Essential Job Functions

  • Verifies patient's needs for acute level of care, collaborating with utilization management nurse to prevent potential denials. The care manager will address when patient is not receiving evidenced based care and escalate to care management leader and/or physician champion at the facility level to resolve barriers.

  • Supports and promotes assertive proactive care with Observation stay patients and helps in removing barriers related to achieving timely testing and treatment. Works in collaboration partners to help remove barriers to get patients converted to appropriate classification when appropriate and guided by physician, ensuring CC 44 is delivered when indicated, also closing the loop on CC 44 to ensure tracking and trending.

  • Maintains clear, concise, and timely documentation in each patient record to reflect physical and functional limitations, psychosocial characteristics, educational needs of patient and family/caregiver, financial, economic, and transition needs. Initiates referrals to disciplines as indicated. Documentation will reflect plan of care to address post-hospital care needs and evidence of patient, family/caregiver involvement in planning.

  • Participates in nursing unit and department clinical outcome projects as well as process improvement initiatives within care management. Works collaboratively with peers to achieve department goals and daily work as evidenced by appropriate and timely communication which is respectful and clear. Sensitive to workload of peers and shares responsibilities, fills in, and offers help promoting team-based approach in order to accomplish work. Understands data relative to patient population and utilizes information to improve quality and reduce cost.

  • The care manager shall support and follow compliance rules and regulations and Conditions of Participation for DC Planning and Utilization Review, and address opportunities or potential concerns with leadership. Identifies potential or current patient situations which require referral to other members of the health care team. Keeps leadership abreast of potential issues.

  • The Care Manager will demonstrate commitment to work partners to help each other reach mutual goals and learn from each other. Demonstrates actions and behaviors that consistently promote trust, respect, a positive attitude and promote team morale.

Cardiac Sonographer Registered - The Jewish Hospital

Job Summary:

The Cardiac Sonographer performs diagnostic, non-invasive and invasive cardiac exams on patients using ultrasound testing equipment to evaluate the heart. The testing modalities include Transthoracic Echocardiography, Transesophageal Echocardiography, and Cardiac Stress testing.

Essential Functions:

  • Performs diagnostic echocardiography examinations using ultrasound testing equipment to evaluate the heart during Transthoracic Echocardiography, Transesophageal Echocardiography, and Cardiac Stress testing examinations.

  • Demonstrates ability to proficiently perform non-invasive and invasive cardiac exams.

  • Demonstrate knowledge of cardiac anatomy, exam procedures, and interpretation of results.

  • Prepares and monitors patients before and during procedures.

  • Utilizes the hospital electronic medical record to review patient information and document any critical findings.

  • On call hours required.

Education:

  • Associates degree in an applied health science field with a concentration in cardiovascular technology, ultrasonography or other related study (preferred)

Licensure/Certification:

- Responsible for reprocessing if on Endoscopy team.

  • Identifies and prioritizes patients in need of care management services, using a holistic approach inclusive of biopsychosocial, functional, cultural, spiritual, and financial factors.   • Supports and promotes assertive, proactive care for inpatients and observation patients, assisting in removing barriers related to achieving timely testing and treatment, and ensures resources are utilized appropriately. If continued treatment can potentially be managed in the ambulatory setting, the care manager will collaborate with the physician and care team offering alternatives to acute care, and escalate to care management leader or physician champion at the facility level if warranted to resolve barriers.

  • Works in collaboration with revenue cycle partners to help remove barriers to ensure patients are converted to the appropriate classification as guided by the physician, ensuring CC 44 process is followed.

  • Participates in nursing unit and department clinical outcome projects as well as process improvement initiatives within care management. Works collaboratively with peers to achieve facility and department goals and daily work as evidenced by appropriate and timely communication which is respectful and clear. Summary of Primary Function/General Purpose of Position

They process specimens for diagnostic testing and referral testing and sort specimen drop-offs according to insurance. They ensure accurate registration of specimens as needed.

Unit Secretary - Mary Immaculate Hospital

Job Summary:

Under general supervision, the Unit Secretary provides clerical support to the nursing unit. As a member of the nursing unit, the Unit Secretary conducts themselves to promote an organized and positive culture on the unit.

Essential Functions:

  • Must possess the ability to troubleshoot and resolve caller problems and specific request in a timely manner

  • Provides excellent, customer-focused service by prioritizing and navigating all internal and external calls in a prompt respectful manner from both internal customers and the community

  • Schedules discharge follow up appointments; assists in recording traumas and codes within the facility

  • Performs other administrative or clerical duties and projects as assigned

  • Other duties as assigned

Education:

  • High School Degree or GED

Experience:

  • Prior healthcare clerical work (preferred, not required)

Training:

  • EPIC Electronic Health Record (EHR) (preferred, not required)

Skills & Abilities:

  • Possesses problem-solving skills, basic computer skills, 25 WPM typing skills with excellent communication and interpersonal skills

  • Engage with staff and patients in a professional manner

  • Ability to be proactive in a rapidly changing environment

Harmonized - Sterile Processing Technician - Mary Immaculate Hospital

Job Summary:

The Sterile Processing Technician Non-Certified, under general supervision, will inspect, clean, assemble, test, and processes patient care equipment, supplies, and instruments according to established policies and procedures and according to manufacturer's instructions for use. They will act in accordance to sterility per the Association for the Advancement of Medical Instrumentation (AAMI), The Joint Commission (JCAHO), Center for Disease Control (CDC), and state standards of practice. Primary duties may vary depending on area assigned.

Essential Functions:

  • Performs decontamination functions including manual cleaning and processing of surgical instrumentation and patient care equipment according to manufacturer's instructions for use.

  • Applies proper knowledge of various detergents and chemical disinfectants for use in the decontamination and removal of blood, bone, and other body tissues from contaminated surgical equipment. Adheres to standard precautions while performing cleaning and sterilization duties.

  • Inventories, stocks, and delivers supplies, carts, equipment to all facility areas as indicated per facility guidelines. Orders, restocks, labels, and maintains proper inventory levels. Rotates stock, inspecting for outdates per market requirements.

  • Performs and documents quality control aspects of sterile processing using various information technology systems focusing on instrument tracking, surgery schedules, surgeon preference cards, supply ordering and management. Accurately maintains comprehensive quality assurance standards related to sterilization and infection control.

  • Operates and understands various sterilizers and sterilizing techniques required to produce sterile packages for use in the surgical setting keeping package assembly aseptic protocols in mind. Monitors sterilization regularly ensuring quality assurance through chemical and biological indicators and integrators.

  • Documents all activities as required within the outlined processes and reports variances that may affect the outcome of sterile products or patient care and safety.

Education:

  • High School Diploma or GED (required)

Required Licensing & Certifications:

  • One of the following is required: Certified Registered Central Service Technician (CRCST), Healthcare Sterile Processing Association (HSPA) (certification required within 2 years after hired) or Certification Board for Sterile Processing and Distribution (CBSPD) (certified required within 2 years after hire) (required)

  • Recent experience in surgical setting (preferred)
