Responsibilities
Provides strategic clinical oversight and support of the day-to-day clinical community operations to Executive Directors, Health and Wellness Directors and clinical associates, as well as district and regional team leaders above assigned communities. Supports the company goals and strategies and optimizes resources to ensure quality care and service delivery, training and development of staff, and management of risk and regulatory compliance.
Responsible for the successful implementation and maintenance of community support center clinical policies and procedures.
Conducts regular clinical quality audits through community site visits and audits of all health care areas to ensure compliance with Brookdale standards and state regulations, appropriate resident assessments, appropriate staffing, staff training, and quality assurance. Provides a written report of findings and ensures action planning is in place for areas of risk
Analyzes specific community models to support staffing of clinical positions based on community size and residents receiving services.
Supports the implementation of quality care and services, and encourages the development of medical professional relationships within the community. Encourages alignment with value-based provider groups focused care delivery outcomes.
Oversight of community survey readiness for regulatory compliance.
Monitors compliance and assists communities in root cause and corrective action of community quality metrics and clinical outcomes related to falls, elopements, medication administration and reduction of psychotropic drugs as well as skin integrity. Supports community infection control measures and risk in partnership with Executive Director.
Analyzes resident incident reports and supports the community in corrective action plans as appropriate.
Verifies that a Community Care Conference and Collaborative Care Process is in place in accordance with policy, and makes plans to attend virtually or in person as needed.
Reviews resident clinical assessments to validate accuracy of resident's physical, emotional and mental functioning, and validates that clinical services provided to residents are documented in appropriate system/record.
Ensures the CARE process is being executed appropriately to address controllable resident move outs.
Provides clinical guidance to field staff, when necessary, to determine appropriate level of care for resident move-ins.
Supports the community's overall resident/family satisfaction level as related to clinical care and impact to the overall community.
Promotes the community preferred pharmacy relationship and supports 80% or greater resident participation. Supports any innovative projects or implementations that drive resident wellbeing or satisfaction.
Supports the community-based Personal Solutions program, ensures a community leader is identified, endorses resident participation and actively assists community with managing the affiliated budgeted goal.
Participates in hiring, training, and mentoring of Health and Wellness Directors and other field level clinical associates. Actively supports onboarding of these associates, coaching and providing them an exceptional experience in the first ninety days.
Analyzes resident and medication incident reports and assesses trends and assists with process improvement plans. Reviews various healthcare reports, trends, and move-outs to assist in process improvement efforts.

Qualifications
Education and ExperienceBachelor's degree (B.S.N.) from four-year college or university preferred; RN license required, and minimum of five to seven years related experience and/or training, most of which are in the geriatric field; or equivalent combination of education and experience. Experience with the elderly required and must have a thorough understanding of aging issues related to health.
Certifications, Licenses, and Other Special RequirementsLicensed as a registered nurse. Must have a valid driver's license and access to a private vehicle for business travel.

Requirements to be a Maintenance Technician
Maintenance Technicians will maintain scheduled and unscheduled maintenance on, but not limited to: HVAC, elevator, fire and sprinkler equipment, generators, landscape, kitchen equipment and emergency pull cord equipment. Maintenance Technicians will also perform general maintenance projects as scheduled or assigned and respond to maintenance requests by community directors and residents in both private rooms and public living areas. They will also maintain positive communication with local fire marshal, building inspector and any other jurisdictions governing the community.

Responsibilities
Provides direct supervision of care giving within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community.
Provides direct supervision of resident care staff, which includes monitoring job performance, collaborating with community leadership on associate performance evaluations, coaching and counseling associates and conducting and documenting corrective action as needed.
Coordinates training and education of resident care staff on an ongoing basis.
Verifies all associate hours on time cards and weekly hours report. Completes nursing schedules to assure proper coverage to meet resident needs and according to budget. This includes arranging replacement staffing when necessary and maintaining clear records of all changes, requests and irregularities.
Observes residents health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to Health and Wellness Director (or designee) in a timely manner.
Assists the Health and Wellness Director (or designee) in medication management, ordering medication from pharmacy and maintaining records as dictated in Brookdale Senior Living policies and procedures manual. If within scope of practice, may administer medication to residents; may review capabilities of residents who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation).
Monitors staff in administration of medication, to include frequents audits of medication administration records (MAR).
Observes residents, records significant reactions, and notifies community leadership of resident's condition and reaction to drugs, treatments, and significant incidents. Responds promptly to all emergency calls from residents.
Communicates to families regarding resident's personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments. Assigns vital signs to associates and monitors compliance. Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through.
Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families.
Participates in the maintenance of resident charts including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately.
Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by community leadership.

Job Summary
Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. Participates in or leads meetings relevant to resident care issues, such as Care Plan, Change of Condition, Clinical Stand-Up, and Collaborative Care meetings involving appropriate parties as needed.
Ensures in-house ancillary medical services such as podiatrist visits, doctor visits, and dental visits, psych visits, lab tests, X-ray, ambulance, etc., are scheduled and followed through.
Participates in department quality improvement activities.
Assists in keeping the environment safe for associates to reduce the occurrence of Workers' Compensation claims by appropriately evaluating resident transfer and mobility needs and involving therapy services as indicated.

Facilitates continuity of care for residents receiving home health care, hospice services, and other third party healthcare-related services.
Participates in or leads meetings relevant to resident care issues, such as Care Plan, Change of Condition, Clinical Stand-Up, and Collaborative Care meetings involving appropriate parties as needed.
Ensures in-house ancillary medical services such as podiatrist visits, doctor visits, and dental visits, psych visits, lab tests, X-ray, ambulance, etc., are scheduled and followed through.
Participates in department quality improvement activities.
Assists in keeping the environment safe for associates to reduce the occurrence of Workers' Compensation claims by appropriately evaluating resident transfer and mobility needs and involving therapy services as indicated.

Responsibilities
Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as residents' healthcare needs change. Supervises licensed nurses and other direct care staff within the community. Responsible for overseeing and validating all resident care to ensure needs are met and that care is provided in accordance with physician orders, care plans, policies/procedures, and state and federal regulations. Strengthens clinical processes within community until a replacement is identified, then supports newly assigned Director of Clinical Services, Assistant Director of Clinical Services, and/or MDS Coordinator and others as assigned. Travels within the division as assigned to support management vacancies, start-ups, vacations, etc.
Responsible for the direct supervision of community-based licensed nursing staff (LPN/LVN, RN), CNAs, and Med Techs/CMAs (if allowed/required by State Regulations). Also responsible to supervise nursing administrative associates.
Assigns and directs work of subordinates, appraises performance, rewards and disciplines associates, and addresses complaints and resolves problems.
Provides training, supervision, and monitoring of associates in the administration of medications as described/allowed in Nurse Practice Act, including auditing of medication administration records.
Provides training and education to resident care associates on an ongoing basis with classroom in- services, situation-specific training, and resident-specific training.
Supervises the maintenance of resident charts and reviews documentation performed by caregiving staff.
As described and allowed in the Nurse Practice Act, assesses the health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness, and maximizes the resident's opportunity to remain in their environment.
Manages the health care of residents, including the dissemination of information to families and associates. Ensures that family members are aware of residents' need for scheduled appointments.
Participates in pre-admission screening of prospective new residents. Ensures that required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms. Responsibilities
Provides strategic clinical oversight and support of the day-to-day clinical community operations to Executive Directors, Health and Wellness Directors and clinical associates, as well as district and regional team leaders above assigned communities. Supports the company goals and strategies and optimizes resources to ensure quality care and service delivery, training and development of staff, and management of risk and regulatory compliance.
Responsible for the successful implementation and maintenance of community support center clinical policies and procedures.
Conducts regular clinical quality audits through community site visits and audits of all health care areas to ensure compliance with Brookdale standards and state regulations, appropriate resident assessments, appropriate staffing, staff training, and quality assurance. Provides a written report of findings and ensures action planning is in place for areas of risk
Analyzes specific community models to support staffing of clinical positions based on community size and residents receiving services.
Supports the implementation of quality care and services, and encourages the development of medical professional relationships within the community. Encourages alignment with value-based provider groups focused care delivery outcomes.
Oversight of community survey readiness for regulatory compliance.
Monitors compliance and assists communities in root cause and corrective action of community quality metrics and clinical outcomes related to falls, elopements, medication administration and reduction of psychotropic drugs as well as skin integrity. Supports community infection control measures and risk in partnership with Executive Director.
Analyzes resident incident reports and supports the community in corrective action plans as appropriate.
Verifies that a Community Care Conference and Collaborative Care Process is in place in accordance with policy, and makes plans to attend virtually or in person as needed.
Reviews resident clinical assessments to validate accuracy of resident's physical, emotional and mental functioning, and validates that clinical services provided to residents are documented in appropriate system/record.
Ensures the CARE process is being executed appropriately to address controllable resident move outs.
Provides clinical guidance to field staff, when necessary, to determine appropriate level of care for resident move-ins.
Supports the community's overall resident/family satisfaction level as related to clinical care and impact to the overall community.
Promotes the community preferred pharmacy relationship and supports 80% or greater resident participation. Supports any innovative projects or implementations that drive resident wellbeing or satisfaction.
Supports the community-based Personal Solutions program, ensures a community leader is identified, endorses resident participation and actively assists community with managing the affiliated budgeted goal.
Participates in hiring, training, and mentoring of Health and Wellness Directors and other field level clinical associates. Actively supports onboarding of these associates, coaching and providing them an exceptional experience in the first ninety days.
Analyzes resident and medication incident reports and assesses trends and assists with process improvement plans. Reviews various healthcare reports, trends, and move-outs to assist in process improvement efforts.
