Responsibilities
Job Summary
In alignment with Brookdale's Resident and Family Engagement philosophy, plans, coordinates and facilitates a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. Provides person centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.
Essential Functions
Designs a 7 day a week person centered program that is engaging, creative and meets the needs and interests of the individual residents. Promotes and engages residents in the 7 domains of well-being programs in a dementia friendly structured environment; including sensory focused small group programming for residents who cannot participate in a larger group setting.
Develops a monthly calendar, in partnership with residents, based on residents' shared interests. Ensures person centered special events and daily path programs are scheduled and executed as scheduled.
Ensures dementia care programs are in compliance with Brookdale Excellence Standards Tool (BEST), Divisional Dementia Care Site visit tool, state, federal, and other regulations and meet Brookdale's mission, policies, procedures and dementia care standards as assigned by the Executive Director.
Collaborates with community leadership educating & providing resources to families and associates regarding Alzheimer's disease and other dementias. This includes how to engage residents and lead programs on an ongoing basis. This includes facilitating and coordinating dementia focused in-services and or events for families or computer based and situation specific training for associates including Understanding Alzheimer's & Dementia Care (UADC), as required by Brookdale.
May provide direct supervision of Clare Bridge Program Assistant/s, as required by Executive Director, which includes assisting with hiring, coaching, and counseling program assistant associate and conducting the documenting corrective action, as needed. Completes job performance evaluations. May act in supervisory capacity in absence of Manager or Executive Director.
Collaborates with community leadership in planning, coordinating, appropriately decorating, and participating in special events including holidays, signature programs, National Walk to End Alzheimer's, family socials, educational and marketing events.
Reports changes in residents' overall health to clinical partner, including changes in behavior; takes actions to address concerns in a timely manner and collaborates with the team in exploring approaches and interventions to address behavioral expressions.
Oversees social media content to ensure alignment with Brookdale's culture, brand, and social media guidance and Brookdale standards.
Plans and schedules programming events outside the community, which may include driving a community motor vehicle.
Maintains and adheres to department monthly budget and expense control and may manage volunteer program.
Performs personal care, housekeeping and dietary responsibilities, as needed, to ensure service quality is maintained in accordance with Clare Bridge Program standards. Has completed training and qualifications based on state regulations to ensure service quality is maintained in accordance with Clare Bridge Program standards.

Qualifications
Education and ExperienceBachelor's Degree in therapeutic recreation, gerontology, education or related field preferred. Minimum two years of direct programming experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.
Certifications, Licenses, and other Special RequirementsMust have a valid driver's license and a current 3-year driving abstract with no more than one moving violation as a condition of employment and may be required to obtain a commercial driver's license based on the needs of the community

Responsibilities
Under limited supervision, receives walk-in/call-in visitors and uses software systems to identify and contact leads provided by the Business Development Coordinator and current community residents. Effectively explains and demonstrates to prospective residents and their families the benefits of residing and receiving care at the community. Communicates sales and contractual information and closes contract sales to maintain and improve occupancy levels. Solicits assistance of community staff to promote and explain community services, practices, policies, pricing and other information necessary to close the sale.
Routinely reviews software databases to identify and follow-up on leads from all sources. Calls a lead and obtains additional information on personal residential and care needs. Encourages prospective residents and families to make wait list deposits. Maintains long term contact with prospects to encourage continued familiarity. Encourages and promotes visits and tours of community(s). Meets established goals regarding contacts and tours to achieve desired sales goals. Enters updated prospective resident information and status of relationship into the lead management system.
Maintains positive and understanding relationships with current residents and their family. Encourages their positive promotion of community to friends, acquaintances and family. Identifies religious, civic and social groups to which current residents belong to expand referral network. Cultivate leads with current residents through positive interaction and interest in activities and concerns. Follows-up with resident referrals as identified. Explains and promotes the Resident Referral Program to current residents.
Assists in identifying innovative events and methods to increase visitation of targeted groups. Assists in planning, sponsoring, organizing, and executing special events to inform and attract referrals and prospects to visit community. Presents and demonstrates the value and benefits of the community for its residents as appropriate to stimulate interest and sales.
Obtains and maintains knowledge of competitors. Communicates community features to best compete with competitor's strengths and weaknesses. May visit competitors' communities to understand how to best refer and market services and products.
Conducts personal tours of communities for prospective residents and family. Schedules meetings with staff department heads as requested or as deemed necessary to effectively convince potential residents of abilities to meet their residential and care needs. Understands needs of potential residents to highlight community features that will satisfy them.
Explains fee structure in a manner that will be most meaningful and that will clearly reflect the benefits of prospective resident's choices. Explains the physical and financial assessment processes to prospects and coordinates required staff interviews and credit checks.
Closes contractual arrangements. Communicates availability of units and status of waiting lists. Obtains necessary personal information and prepares standard, legal documents required for signature. Communicates pricing structures and service rates. Obtains signatures, required deposits and authorizations for move-ins.
Requests assistance of marketing assistant when appropriate to coordinate needs relating to logistics of new resident moves including such things as transference of keys, furniture move-in coordination, unit preparation, new resident orientation and welcoming, etc. Maintains continued contact to reflect concern and interest for their continued wellbeing and satisfaction after relocation to the community.
May provide input regarding the development of promotional brochures, videos and mailings as used in networking referrals and marketing to prospective residents.
Prepares written reports and communicates status of current success in a clear and concise manner.

Qualifications
Education and ExperienceA Bachelor's degree or equivalent in coursework and related experience is required. Minimum of five years of progressive experience in the real estate or health care field including experience in such areas as promotion, public relations, or admissions, including a minimum of two years with a proven sales or promotion track record. Leads effort to leverage satisfied residents and families to grow community occupancy; executes renewal program with existing residents. Partners with Resident Council as necessary. Administers resident satisfaction and other surveys and works with department leaders and district team to create plan to address opportunities for improvement in resident satisfaction, experience, and engagement.
For assisted living or memory care communities, ensures collaborative relationship with community clinical leader to ensure community's care and services are appropriate to meet the needs of residents. Oversees resident services including the admission process, healthcare management, and maintenance of resident documentation to ensure high quality services and compliance with Company policy and applicable legal requirements. Empowers department leaders to demonstrate the same for their area. Ensures service plan reviews are conducted, consistent with applicable legal requirements, with appropriate clinical care team members and resident families to maintain the personal dignity of residents.
Demonstrates a high degree of financial acumen as it relates to community operations. Analyzes, develops, and executes annual operating and capital budgets; works to meet or exceed budgeted revenue, profitability, and occupancy goals. Proactively creates plans to overcome unanticipated expenses or revenue shortfalls. Continually explores means of revenue enhancement and expense reduction, while meeting the needs of residents and adhering to Company policies and applicable regulations.
Drives sales and marketing efforts in collaboration with the community sales leader to meet or exceed occupancy or revenue targets by developing new business, generating leads, and building strategic relationships. Engages with prospects who tour the community. Identifies trends and works with sales leader to implement approved sales and marketing activities and strategies to maximize revenue (RevPAR).
Identifies and builds positive relationships with local influencers and professionals to raise community profile. Becomes active in social and civic affairs of the local community that align with the Company's mission. Responsibilities
Directs food service operations within the community including all food preparation, dining room operations and dining delivery services. Purchases all food and manages inventory ensuring effective cost controls and vendor service and quality. Ensures the highest nutrition and food quality for the health and pleasure of the residents. Complies with all federal, state and local regulations to ensure sanitary and safe operations. Plans and executes entertaining dining events and themes in conjunction with Activities Director, Marketing Director and other administrative staff to optimize the life and pleasure of the residents. Consults with chefs and cooks to develop and maintain pleasing and nutritious menus. Responsibilities
The Care Coordinator - LPN serves as a liaison, promoting coordination, communication, and collaboration among Brookdale teams, residents, families, and healthcare providers within Brookdale's HealthPlus communities. Upholds open and collaborative communication channels, aligning with the changing healthcare needs of the residents. Works closely with primary and specialty care providers to formulate resident-specific care plans, ensuring that the delivery of medically necessary services aligns with established care plans. Serving as a resident advocate, they actively contribute to identifying and improving service delivery, closing gaps in care, and understanding key interventions and care protocols relevant to treating residents in place.
Reviews the current population's medical diagnoses to identify care needs. Assists in assessing the health, functional, and psycho-social status of residents.
Coordinates services required to enhance optimum wellness and manage chronic conditions.
Collaborates with hospice, palliative, and home health services/therapy as indicated.
Participates in collaborative care reviews (CCR) and care conferences.
Educates and explains Brookdale's care pathways as determined by individual diagnosis and healthcare needs and provides advanced care planning education to residents and families/responsible parties. Helps ensure each resident has a completed Advanced Care Plan, inclusive of goals of care. Communicate with healthcare providers to ensure care delivery is consistent with the resident's Advanced Care Plan(s).
Provides resources for residents and families to make informed decisions regarding choices in meeting healthcare needs and effectively champions the benefits of residing and receiving care and services in the community. This includes informing them about opportunities to enhance care and benefits through specialized plans like the IE-SNP, specifically designed for residents with chronic medical conditions.
Applies quality improvement methodology to analyze, enhance, and manage outcomes, striving to achieve quality measure goals. Collaborates with HWD and/or RN Care Manager to ensure compliance with service plans, while working with providers to attain value-based outcomes for our residents. This includes coordinating and communicating care for each resident.
Communicates information promptly to the appropriate care provider regarding changes in residents' health/well-being, personal needs, risk management issues, customer service issues, and family, or outside health care provider concerns.
Applies quality performance measures, and assist with training and education of teams to ensure quality outcomes for residents. Responsibilities
Directs food service operations within the community including all food preparation, dining room operations and dining delivery services. Purchases all food and manages inventory ensuring effective cost controls and vendor service and quality. Ensures the highest nutrition and food quality for the health and pleasure of the residents. Complies with all federal, state and local regulations to ensure sanitary and safe operations. Plans and executes entertaining dining events and themes in conjunction with Activities Director, Marketing Director and other administrative staff to optimize the life and pleasure of the residents. May also cater events as requested by residents or staff.
Hires, trains and manages all supervisory staff for the kitchen, dining room, catering and delivery services. Ensures that supervisors maintain a safe and sanitary environment and provide a caring and pleasurable dining experience for the residents. Coaches supervisors in maintaining appropriate staffing levels and effectively motivating, disciplining and training associates.
Consults with chefs and cooks to develop and maintain pleasing and nutritious menus. Assesses and determines the desires and preferences of the residents regarding food selection and presentation through surveys and daily contact with residents. Ensures preferences are incorporated into the menu and events. Understands the dietary needs of residents and coordinates with dieticians, nursing staff, and care associates to ensure proper selection, preparation and delivery of meals.
Develops department budget including purchasing and staffing expenditures. Routinely monitors budget compliance and recommends changes as needed to maintain operational quality and resident satisfaction.

Job Summary
Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, assists in providing person centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.
Essential Functions
Supports the implementation of a person centered program to meet individual needs and interests of the residents. Assists in leading and engaging residents in the 7 domains of well-being programs - purposeful, intellectual, social, spiritual, emotional and physical in a dementia friendly structured environment; including sensory focused small groups for residents who cannot participate in a large group setting.
Assists the Program Manager or Coordinator in the review of individual resident's service plans and completes program documentation, as required by Brookdale standards or state requirements.
Assists the program manager or coordinator in ensuring dementia care programs are in compliance with state, federal, and other regulations and meet Brookdale's mission, policies, procedures and standards.
Assists in providing guidance and resource to associates on the importance of programming/engagement and other areas regarding the quality of life for residents.
Coordinates and participates in special events at the community including family socials, marketing/sales and open houses.
Communicates positively with residents, families, and community professionals regarding the Brookdale dementia care program and its benefits. Communicates with families regarding residents' personal needs and successes.

Proven, solution-minded leader and change agent with entrepreneurial spirit responsible for creating and executing a strategic turnaround business plan, in partnership with the district team, to grow revenue and to materially impact sales and profitability and for developing the local team to become the preferred senior living community in the market. Leader is responsible for the community's daily operations, associate relations, resident and resident family engagement and connection, financial performance, and regulatory compliance. Works with sales to drive sales results and foster a strong sales performance culture. Creates an inclusive community culture that provides high quality resident experiences and care and engages residents, families, and associates.
Responsible for all operations within the community, interacting with staff and residents, prospects, or their family members, as necessary.
Assesses local market and develops and implements a turnaround business plan to become the highest performing senior living community in the area. Creates, analyzes, and executes annual operating and capital budgets and holds department leaders accountable for department budgets. Manages community performance against turnaround goals; sets and continuously assesses plans to improve performance to Company service and financial standards, including adjusting plans as needed in discussion with district leadership.
In partnership with District Director of Sales, provides strategy, support and vision to achieve sales and occupancy goals through effective business development efforts in the market. Engages in business development activities and events in the market while representing Brookdale as the leader in Senior Living to all referral sources. Drives sales and marketing efforts in collaboration with the community sales leader to meet or exceed occupancy or revenue targets by assisting to develop new business, generate leads, and build strategic relationships. Engages with prospects who tour the community. Identifies trends and empowers sales leader to implement approved sales and marketing activities and strategies to maximize revenue (RevPAR).
Builds and maintains strong working relationships with management team and encourages teamwork and collaboration; cultivates an inclusive community culture. Supervises, directs, and motivates community management while empowering department leaders to supervise, direct, and motivate staff; ensures department leaders proactively recognize and solve issues. Holds department leaders accountable for department performance. Provides assistance to leaders and staff as needed. Works with department leaders to ensure community maintains appropriate staffing levels to meet the needs of residents and in accordance with applicable legal requirements.
Encourages and supports department leaders to attract, develop, engage, and retain associates in accordance with Company policies. Understands the community's staffing needs and is responsible for overseeing the recruiting and onboarding process; ensures associates are appropriately trained and developed to meet the needs of residents. In consultation with department leaders, reviews promotions, development plans, disciplinary actions, and termination decisions to ensure consistency in the selection and retention of quality associates. Works with department leaders to analyze trends and implement strategies to reduce turnover and increase retention.
Builds high degree of resident satisfaction and retention. Is responsible for maintaining positive resident relations and is accessible and approachable to residents and their families. Empowers community management to proactively solve resident problems and resolve issues. Works with appropriate department leaders to leverage satisfied residents and families to grow community occupancy and to execute renewal program with existing residents. Partners with Resident Council as necessary. Works with community management to administer resident satisfaction and other surveys and to create action plans to address opportunities for improvement in resident satisfaction, experience, and engagement.
For assisted living or memory care communities, creates and maintains collaborative relationship with community clinical leader(s) to ensure community's care and services are appropriate to meet the needs of residents. Oversees resident services including the admission process, healthcare management, and maintenance of resident documentation to ensure high quality services and compliance with Company policy and applicable legal requirements. Empowers department leaders to demonstrate the same for their respected area. Ensures clinical leader(s) conduct service plan reviews, consistent with applicable legal requirements, with resident families to maintain the personal dignity of residents.
Demonstrates a high degree of financial acumen as it relates to community operations. Analyzes, develops, and executes annual operating and capital budgets; works to meet or exceed budgeted revenue, profitability, and occupancy goals. Works with department leaders and district team to proactively create plans to overcome unanticipated expenses or revenue shortfalls. Continually explores means of revenue enhancement and expense reduction, while meeting the needs of residents and adhering to Company policies and applicable regulations.
Identifies and builds positive relationships with local influencers and professionals to raise community profile. Becomes active in social and civic affairs of the local community that align with the Company's mission. Represents the community and the Company to governmental agencies (as appropriate), professional organizations, community groups, and other appropriate public agencies and groups.
Works with maintenance and other appropriate department leaders to ensure buildings, grounds, and property are up to Company standards; engages in active oversight of preventative maintenance systems and programs and frequent inspections that meet Company standards of excellence.

Qualifications
Education and Experience

Responsibilities
Responsible for maintaining a secure environment for all residents and associates within the building and on the community grounds. Greets visitors, monitors the inside and outside of the building, and responds to emergencies.
Greets and monitors visitors and residents coming and going from the building. Logs all residents and visitors in and out.
Monitors the inside and outside of the building and the parking lot; reports any abnormal findings to the Executive Director or to the proper authorities as the situation dictates.
Locks doors at designated time, according to community protocol. Checks the security of all doors regularly throughout the night.
Maintains familiarity with and monitors all emergency and safety equipment including security cameras, phones, and life safety systems.
Responds appropriately to emergencies by calling or contacting appropriate internal and external parties, according to established protocol. Assists with disaster management efforts and activities during evacuations (i.e. fires, hurricanes, etc.).
Answers the phone promptly. Responds to questions, directs calls, or takes messages as appropriate.
Escorts residents and visitors to the parking lot during evening hours, as requested.
May be required to handle leasing inquiries after hours and on weekends, making sure that accurate and timely information is provided to the Director of Sales & Marketing.
Drives the bus or community vehicle as needed or as scheduled.
Handles minor maintenance emergencies (i.e. plugged toilets, reset breakers, replace light bulbs, etc.) as needed.
Ensures the lobby and administrative offices are vacuumed, and kept clean and neat. Notifies housekeeping if cleaning is needed.

Responsibilities
Assists in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services department. Operates in accordance with current federal, state and local standards, guidelines and regulations, and established company policies and procedures to ensure that the medical-related emotional and social needs of the residents are met and maintained on an individual basis.
Assists in developing preliminary and comprehensive social service needs assessments and plan of care for residents to include identifying resident problems, needs and goals.
Encourages residents and family members to participate in the development and review of care. Interviews residents and families to obtain social history. Evaluates social and family information to assist in determining plans for social treatment. Interprets and relays the social, psychological, and emotional needs of the resident and family to the medical staff, attending physician, and other resident care team members.
Provides consultation to members of the facility, community agencies, etc. in an effort to solve the needs and problems of the residents through the development of social service programs. Meets with administrative, medical and nursing staff, as well as other related departments in planning, implementing, and assessing social service programs and activities.
Provides Medicare/Medicaid and financial assistance information to residents and families as appropriate. Assists in obtaining resources from community social, health and welfare agencies to meet the needs of the residents. Refers residents and family members to the appropriate social service agencies when the facility does not provide for the services or needs of the residents.
Completes all necessary forms, reports, etc. and submits to Director as required. Ensures that all charted progress notes are informative and descriptive of the services provided and documents resident responses to services provided. Provides oral and or written reports of the social services programs and activities as required. Participates in discharge planning.
Develops, implements, and maintains an ongoing quality assurance program for the social services department. Assists in the development, administration, and coordination of department policies and procedures. Reviews, documents and assists in resolving department complaints and grievances.
Keeps abreast of current federal/state regulations and professional standards, making recommendations on changes in policies and procedures to the Director. Maintains a reference library of written material and laws in order to comply with current standards and regulations. Develops quality social service programs.
Participates in facility surveys (inspections) made by authorized government agencies.
Serves on, participates in and attends various committee or department meetings as required and as appointed by the Administrator. Evaluates and implements recommendations from established committees as they may pertain to social services.
Schedules department work hours, personnel, work assignments etc. to expedite work. Conducts departmental performance evaluations in accordance with the facilities policies and procedures. Counsels/disciplines social services associates as requested or as needed. Terminates associates when necessary, documenting and coordinating such action with HR and or Administrator. Assists in recruiting, interviewing and selecting social services associates.
Develops and participates in the planning, conducting, and scheduling of timely in-service training classes. Develops, implements, and maintains an effective orientation program.
Forecasts the needs of the department. Assists in preparing and planning the social service's department budget and ensures adherence to the budget.

Our Receptionists sort, deliver and collect incoming/outgoing community and resident mail, and may be asked to do basic office/clerical functions, and maintain visitor logs. You may also respond to resident emergencies by calling appropriate internal and external parties based on community protocol.
