Responsibilities

Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change. Supervises licensed nurses and other direct care staff within the community. Strengthens clinical processes within community until a replacement is identified, then supports newly assigned Health and Wellness Director. Travels within the division as assigned to support management vacancy, start up, vacations, etc.

  • Responsible for the direct supervision of community-based licensed nursing staff (LPN/LVN, RN), the Med Techs/CMAs (if required by State Regulations) and/or Lead Resident Care Associates. May also supervise Resident Care Coordinators and Supervisor, Resident Care.

  • Assigns and directs work of subordinates; appraising performance; rewarding and disciplining associates; addressing complaints and resolving problems.

  • Provides training, supervision, and monitoring of associates in the administration of medications as described/allowed in Nurse Practice Act, to include auditing of medication administration records.

  • Provides training and education to resident care associates on an ongoing basis with classroom in-services, and situation-specific training.

  • Supervises the maintenance of resident charts and reviews documentation performed by care giving staff.

  • As described and allowed in the Nurse Practice Act, assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in their environment.

  • Manages the health care of residents, including the dissemination of information to families and associates. Ensures that family members are aware of resident's need for scheduled appointments.

  • Participates in pre-admission screening of prospective new residents. Assures that required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms. Updates assessments as required by policy and as described/allowed in Nurse Practice Act.

  • Performs on-site evaluations of residents admitted to alternate care environments for treatment, and maintains contact with resident families with the intent of returning resident to Brookdale.

  • Performs ongoing assessment/observation of residents' physical and psycho-social needs and coordinates with other departments to assure quality, proactive care.

  • Evaluates residents; documents changes in condition, and notifies executive director, physician, and resident's legally responsible party/family of resident's condition and reactions. Prepares Physician Visit Form and reviews and updates resident chart.

  • Facilitates continuity of care for those residents receiving home health care, hospice services, and other third party healthcare- related services.

  • Participates in or leads meetings relevant to resident care issues, such as Service Plan meetings, involving appropriate parties as needed.

  • Ensures in-house ancillary medical services such as podiatrist, doctor visits, dental visits, psych visits, lab, X-ray, ambulance, etc. are scheduled and followed through.

  • Participates in department quality improvement activities.

  • Assists in keeping the environment safe for associates to reduce the occurrence of Workers' Compensation claims by appropriately evaluating resident transfer and mobility needs and involving therapy services as indicated.

  • Shares on-call and manager on duty responsibilities/shifts as required.

  • Strengthens clinical process within the community until permanent replacement is identified.

  • Supports other requirements within the division as assigned.

Qualifications

Education and ExperienceEducation as required to obtain state nursing license (LPN/LVN or RN). Nurse management, senior living, or post-acute care experience preferred. Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment.

Certifications, Licenses, and Other Special RequirementsLPN/LVN or RN license.

Responsibilities

  • Our business office coordinators provide support to the community, which include accounts receivable, accounts payable, payroll and other human resources related duties.

  • You will also track community accounting changes, prepare and record all invoices for the community, assist with new hire documentation as well as organize and maintain personnel, resident, marketing and other files as needed.

  • An Associate's degree (A. A.) or equivalent from a two-year college or technical school; or minimum of one to two years related experience and/or training; or equivalent combination of education and experience is preferred.

Responsibilities

Provides direct supervision of care giving within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community.

  • Provides direct supervision of resident care staff, which includes monitoring job performance, collaborating with community leadership on associate performance evaluations, coaching and counseling associates and conducting and documenting corrective action as needed.

  • Coordinates training and education of resident care staff on an ongoing basis.

  • Verifies all associate hours on time cards and weekly hours report. Completes nursing schedules to assure proper coverage to meet resident needs and according to budget. This includes arranging replacement staffing when necessary and maintaining clear records of all changes, requests and irregularities.

  • Observes residents health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to Health and Wellness Director (or designee) in a timely manner.

  • Assists the Health and Wellness Director (or designee) in medication management, ordering medication from pharmacy and maintaining records as dictated in Brookdale Senior Living policies and procedures manual. Assures that medications are properly stored, medication carts/cupboards are cleaned and medication room is organized according to standards.

  • If within scope of practice, may administer medication to residents; may review capabilities of residents who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation).

  • Monitors staff in administration of medication, to include frequents audits of medication administration records (MAR).

  • Observes residents, records significant reactions, and notifies community leadership of resident's condition and reaction to drugs, treatments, and significant incidents. Responds promptly to all emergency calls from residents.

  • Communicates to families regarding resident's personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments. Assigns vital signs to associates and monitors compliance. Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through.

  • Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families.

  • Participates in the maintenance of resident charts including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately.

  • Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by community leadership.

  • Performs daily, weekly and monthly safety and infection control checks of the home and grounds, assisting the community leadership for resident health and safety purposes.

  • Participates in community functions including family socials and open houses. Communicates with community professionals.

  • Shares on-call duties as required.

Under limited supervision, receives walk-in/call-in visitors and uses software systems to identify and contact leads provided by the Business Development Coordinator and current community residents. Effectively explains and demonstrates to prospective residents and their families the benefits of residing and receiving care at the community. Communicates sales and contractual information and closes contract sales to maintain and improve occupancy levels.   • Routinely reviews software databases to identify and follow-up on leads from all sources. Calls a lead and obtains additional information on personal residential and care needs. Encourages prospective residents and families to make wait list deposits. Maintains long term contact with prospects to encourage continued familiarity. Encourages and promotes visits and tours of community(s). Meets established goals regarding contacts and tours to achieve desired sales goals. Enters updated prospective resident information and status of relationship into the lead management system.

  • Maintains positive and understanding relationships with current residents and their family. Encourages their positive promotion of community to friends, acquaintances and family. Identifies religious, civic and social groups to which current residents belong to expand referral network. Cultivate leads with current residents through positive interaction and interest in activities and concerns. Follows-up with resident referrals as identified. Explains and promotes the Resident Referral Program to current residents.

  • Assists in identifying innovative events and methods to increase visitation of targeted groups. Assists in planning, sponsoring, organizing, and executing special events to inform and attract referrals and prospects to visit community. Presents and demonstrates the value and benefits of the community for its residents as appropriate to stimulate interest and sales.

  • Obtains and maintains knowledge of competitors. Communicates community features to best compete with competitor's strengths and weaknesses. May visit competitors' communities to understand how to best refer and market services and products.

  • Conducts personal tours of communities for prospective residents and family. Schedules meetings with staff department heads as requested or as deemed necessary to effectively convince potential residents of abilities to meet their residential and care needs. Understands needs of potential residents to highlight community features that will satisfy them.

  • Explains fee structure in a manner that will be most meaningful and that will clearly reflect the benefits of prospective resident's choices. Explains the physical and financial assessment processes to prospects and coordinates required staff interviews and credit checks.

  • Closes contractual arrangements. Communicates availability of units and status of waiting lists. Obtains necessary personal information and prepares standard, legal documents required for signature. Communicates pricing structures and service rates. Obtains signatures, required deposits and authorizations for move-ins.

  • Requests assistance of marketing assistant when appropriate to coordinate needs relating to logistics of new resident moves including such things as transference of keys, furniture move-in coordination, unit preparation, new resident orientation and welcoming, etc. Maintains continued contact to reflect concern and interest for their continued wellbeing and satisfaction after relocation to the community.

  • May provide input regarding the development of promotional brochures, videos and mailings as used in networking referrals and marketing to prospective residents.

Provides residents with nursing services and administers medications and treatments in accordance with physicians' orders, within state licensure regulations and consistent with facility nursing standards. Monitors and manages the quality measures and indicators of the community. Plans, develops, directs and evaluates Educational and Clinical Training programs in accordance with current Federal, State and Local Regulation for SNF staff. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policies. Assists in implementing and revising service plans or resident care plans based on resident needs. Monitors responses to treatment plans. Possesses knowledge of medication interactions and notifies Resident Care Coordinator and Director of any occurrences.

  • Plans and organizes quality control programs and activities. Maintains library of quality control and assessment materials necessary for complying with current standards and regulations and maintaining quality resident care.

  • Oversees the Infection Control Program of the facility and devises infection control reports for submission to Director, Clinical Services and Quality Assurance Committee. Ensures adherence to OSHA regulations and infection control standards.

  • Directs and coordinates all in-service educational programs of the facility in accordance with current Federal, State and Local Regulations. Develops, maintains and directs orientation programs for skilled nursing personnel. Conducts regular or special in-services for staff to ensure they remain current on new procedures. Develops, maintains and secures records, reports and instructional manuals, reference materials, pertinent to in-service programs. Uses facility consultants and others for special in-services.

  • Plans, implements, evaluates, and directs the facility's risk management programs and activities in accordance with guidelines issued by the governing board and administrator. Investigates all allegations of misconduct. Notifies appropriate personnel of the investigation. Reports findings to the administrator and other appropriate personnel in accordance with state and federal law.

  • Ensures appropriate professional staff maintains adequate liability insurance. Ensures licensure and background checks are conducted in accordance with company policies.

  • Assists in developing and implementing plans of correction for cited deficiencies. Ensures such plans incorporate timetables and methods of monitoring to prevent such deficiencies from reoccurring.

  • Ensures all personnel attend and participate in annual in-service training programs relative to hazard communication. Provides leadership in formulating the goals and objectives of the in-service programs of the Department. Assists the staff in setting educational goals. Meets with supervisors on a regular basis to solicit advice concerning training needs and assists in identifying and correcting problem areas and/or the improvement of services.

  • Counts controlled substances with off going nurse or resident assistant; secures and corrects reconciliation of controlled substances during shift. Ensures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations and company policy.

  • Assists with discharge process relevant to medication/treatment orders.

  • Gathers and analyzes resident outcomes through tracking and trending to identify root cause and determines appropriate interventions.

  • Provides recommendations and reports to management, as necessary/required, concerning quality control programs and processes. Makes recommendations to facility management to reduce the frequency and severity of medical malpractice and resident/staff injury claims. Reports all accidents and incidents to appropriate agencies and facility management within established time frames.

  • Works with community management, public agencies, insurance adjusters, attorneys, and claimants to obtain and provide information necessary to resolve disputes.

Assists the Director, Clinical Services in planning, organizing, developing and directing the day-to-day operations of the Clinical Services department in accordance with current federal, state and local standards.   • Assesses the comprehensive nursing needs of each resident using acceptable long-term care assessment software tools and according to state and federal time frames.

  • Coordinates care plans for each resident. Oversees the day to day care of each resident to ensure care plans are followed.

  • Works with other team members to establish appropriate level of care in accordance with Medicare/Medicaid reimbursement levels.

  • Assists the Director, Clinical Services in ensuring comprehensive and accurate documentation by nursing service/direct personnel.

  • Plans, implements, evaluates, and directs the facility's Infection Prevention and Control Program.

  • Applies scientific principles and methods in conducting surveillance, using approved definitions of infection and standard methodologies for case identification, data collection, and reporting.

  • Plans and conducts educational programs based on prevention, trends, and outcomes.

  • Prepares reports and presentations for Quality Assurance Performance Improvement Committee, which should include Antibiotic Stewardship activities.

  • Investigates and reports outbreaks of infection and implements infection prevention interventions.

  • Assists in implementation of policy and procedures to support optimal associate compliance, infection prevention, and resident safety in adherence to OSHA regulations and infection control standards.Assists in developing and implementing a Clinical Services organizational structure.   • Participates in maintaining the policy and procedures that govern the day-to-day operations of the Clinical Services Department. Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, assists in providing person centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.

Essential Functions

  • Supports the implementation of a person centered program to meet individual needs and interests of the residents. Assists in leading and engaging residents in the 7 domains of well-being programs - purposeful, intellectual, social, spiritual, emotional and physical in a dementia friendly structured environment; including sensory focused small groups for residents who cannot participate in a large group setting.

  • Assists the Program Manager or Coordinator in the review of individual resident's service plans and completes program documentation, as required by Brookdale standards or state requirements.

  • Assists the program manager or coordinator in ensuring dementia care programs are in compliance with state, federal, and other regulations and meet Brookdale's mission, policies, procedures and standards.

To support our associates in their journey to become a U.S. citizen, Brookdale offers to advance fees for naturalization (Form N-400) application costs, up to $725, less applicable taxes and withholding, for qualified associates who have been with us for at least a year.

Responsibilities

  • Leader primarily responsible for the business office operations of the community.

  • Responsible for attracting, engaging, developing and retaining the community associates necessary to provide high quality care and personalized services to our residents, while minimizing the use of premium labor (in conjunction with HR department in a CCRC).

  • Supports Executive Director in driving profitable growth and complying with operational processes and regulatory requirements.

  • Ensures effective communication with associates, residents, families, vendors and other visitors to the community.

  • Ensures that residents are properly billed for services provided and leads the timely collection of receivables.

  • Responsible for overseeing payroll process to ensure that associates are paid correctly.

  • Ensures that vendor invoices for goods or services to the community are billed appropriately and are processed for payment on a timely basis. Supports an inclusive community culture.

  • Associates degree (A. A.) or equivalent from two-year college or technical school required; or additional years of experience can be substituted for the education requirement on a year-for-year basis. Minimum of three years related experience and/or training. Previous managerial and office setting experience preferred.

May also directly supervise another department.
