Responsibilities

Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, assists in providing person centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.

Essential Functions

  • Supports the implementation of a person centered program to meet individual needs and interests of the residents. Assists in leading and engaging residents in the 7 domains of well-being programs - purposeful, intellectual, social, spiritual, emotional and physical in a dementia friendly structured environment; including sensory focused small groups for residents who cannot participate in a large group setting.

  • Assists the Program Manager or Coordinator in the review of individual resident's service plans and completes program documentation, as required by Brookdale standards or state requirements.

  • Assists the program manager or coordinator in ensuring dementia care programs are in compliance with state, federal, and other regulations and meet Brookdale's mission, policies, procedures and standards.

  • Assists in providing guidance and resource to associates on the importance of programming/engagement and other areas regarding the quality of life for residents.

  • Coordinates and participates in special events at the community including family socials, marketing/sales and open houses.

  • Communicates positively with residents, families, and community professionals regarding the Brookdale dementia care program and its benefits. Communicates with families regarding residents' personal needs and successes.

  • Assists Manager or Coordinator with meeting the department's quality improvement initiatives including the Brookdale Excellence Standards Tool (BEST).

  • Supports plans and scheduled programming events outside the community, which may include driving a community motor vehicle.

  • Assists with volunteer program, including training and supervising volunteers.

  • Performs personal care, housekeeping and dietary responsibilities, as needed, to ensure service quality is maintained in accordance with Clare Bridge Program standards.

Qualifications

Education and ExperienceHigh School Diploma or equivalent required. Associates Degree preferred. Minimum one year of previous experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.

Certifications, Licenses, and other Special RequirementsMust have a valid driver's license and a current 3-year driving abstract with no more than one moving violation as a condition of employment.

Knowledge and SkillsHas a basic level of a skill and can follow written or verbal instructions. Has basic technical skills to complete a few repetitive and well-defined duties. Has a basic knowledge of the organization in order to answer basic questions. Basic knowledge of PC's and word processing software, preferably in a Microsoft Windows environment. Effective oral and written communications skills.

Responsibilities

Plans, directs, and coordinates activities of the dietary department at one or more locations, and provides food services for residents and employees. Responsible for providing a quality dietary experience by overseeing dietary staff and maintaining a pleasant and clean environment.

  • Establishes departmental regulations and procedures in conformance with administrative policies.

  • Develops standards for organization and supervision of dietary services.

  • Maintains quality dietary services within the prescribed budget.

  • Determines quality and quantity of food required; plans menus and controls food costs; may be scheduled to prepare meals.

  • Works closely with divisional dietary services specialist and consults with dietitian when necessary to ensure meals meet the nutritional requirements of the residents.

  • Oversees the food preparation and cooking, to ensure meals are prepared per Brookdale procedures.

  • Makes frequent inspections of all work, storage, and serving areas to determine that regulations governing food safety and sanitation are followed.

  • Prepares cleaning schedule, production worksheet and staff schedules.

  • Maintains inventory of food and nonfood items.

  • Selects, orients, and trains new employees in department; effectively delegates authority to dining services staff to ensure effective flow of materials and services.

  • Develops and directs cost control system; prepares and submits department budget to residence director/executive director.

  • Maintains appropriate documentation in resident's medical records.

  • Provides nutrition education and counseling for residents; presents dining services education program to dining services and other staff as needed.

  • Ensures safe work procedures are developed and followed, to include those relating to employee injuries, prevention, and infection control.

  • Ensures a continuous quality improvement program is in place and appropriate follow-up occurs.

Qualifications

Creates and implements effective community business development plans to support sales and occupancy goals and revenue targets throughout the community.   • Make sales calls to local professional and volunteer influencers to gain quality referrals and expand current referral network.

  • Partner with the Executive Director to ensure that ED is introduced to and engaged with key local influencers to maximize business development.

  • Partner with Brookdale community leaders of clinical, resident programing, culinary & memory care to ensure that their work is known in the greater community and that they are regarded as experts in creating experiences to better manage the challenges of aging.

  • Accurately maintain SMS database content with all sales activity and maximize utilization of Brookdale sales and marketing systems and resources.

  • Produce and manage a rolling 90 day sales plan to be used for goal setting, time and resource management, and updates to Brookdale leadership.

  • Attend stand-up in represented Brookdale community at least once a week to report on specifics of sales work and to get input/partnership to maximize the results of the sales plan.

  • With high professional standards, represent Brookdale well as measured in the growth of census and revenue for your represented community.

  • Build a productive network of local professionals who recognize that seniors can live better if they recommend and advocate a move to a Brookdale community.

Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, assists in providing person centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.

Essential Functions

  • Supports the implementation of a person centered program to meet individual needs and interests of the residents. Assists in leading and engaging residents in the 7 domains of well-being programs - purposeful, intellectual, social, spiritual, emotional and physical in a dementia friendly structured environment; including sensory focused small groups for residents who cannot participate in a large group setting.

  • Assists the Program Manager or Coordinator in the review of individual resident's service plans and completes program documentation, as required by Brookdale standards or state requirements.

  • Assists the program manager or coordinator in ensuring dementia care programs are in compliance with state, federal, and other regulations and meet Brookdale's mission, policies, procedures and standards.

  • Assists in providing guidance and resource to associates on the importance of programming/engagement and other areas regarding the quality of life for residents.

  • Coordinates and participates in special events at the community including family socials, marketing/sales and open houses.

  • Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met.

Responsibilities

  • Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met.

  • Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change.

  • Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Caregiver or Med Tech/Aide in order to meet the residents' needs and staffing requirements.

  • Assists in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and Caregivers and provides training and education to Caregivers on an ongoing basis to include classroom in-services and situation-specific training. In addition, they will also manage the associates schedules.

  • Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines .

  • Shares on call duties as required.

Responsibilities

Manages the associates responsible for the general operating maintenance and repair of the buildings and grounds to maintain proper care of the assigned community(s) in accordance with current federal, state, and local standards/regulations and company policies.

  • Supervises the community maintenance, housekeeping and laundry (if applicable) staff, to include hiring, training, evaluating performance, resolving disciplinary issues and firing.

  • Directs workers engaged in the maintenance and upkeep of the interior and exterior of the community. Inspects completed work for conformance to standards and policies.

  • Coordinates maintenance requests by residents for items needing repair in their rooms, including repairing plumbing, electrical, heating and replacing light bulbs.

  • Oversees repairs and maintenance of buildings including vacated rooms in a timely manner. This includes painting walls and cleaning carpets.

  • Ensures walls, floor coverings, doors, and woodwork in the common areas of the community are properly maintained.

  • Maintains positive communication with local fire Marshall, building inspector, state elevator inspector, and any other jurisdictions governing the community.

  • Assesses all plumbing and electrical problems within the community and determines the best course of action to correct the problem. Works within the legal scope of local and state codes.

  • Reviews safety data sheets (SDS) as requested for potential exposure to hazardous chemicals used and take necessary precautions.

  • Assess property damage and file property damage claims in accordance with company policy.

  • Responds appropriately to resident or community emergencies by assisting as needed.

Responsibilities

Oversees daily operations of the kitchen and ensures the highest quality food products. Directly supervises and trains kitchen personnel on all related culinary activities.

  • Oversees breakfast, lunch and dinner for quality assurance of meal presentation and portion control.

  • Selects recipes, prepares and coordinates all meals. Coordinates prep for the following day's menu.

  • Creates new dishes and develops recipes, standardizing recipes to ensure consistent quality. Establishes presentation technique and quality standards. Plans, modifies and develops menus.

  • Performs a pre-meal meeting with food servers to review detail of daily menu.

  • Ensures an adequate number of culinary associates each shift and ensures absences are covered.

  • Estimates food consumption and requisitions food purchases. Monitors and maintains inventory.

  • Assists in the adherence to maintaining budget compliance for daily food costs.

  • Assists in applying appropriate loss prevention procedures.

  • Assists in hiring, training, scheduling, disciplining and terminating associates. Provides training for all kitchen staff.

  • Assists in the coordination and execution of special events.

  • Maintains kitchen cleanliness and food preparation according to state and local health department code requirements. Ensures all kitchen equipment is in working order and kept clean at all times. Oversees the proper inventory, storage and use of cleaning chemicals. Keeps waste to a minimum by utilizing food storage and food recycling techniques.

Qualifications

Education and ExperienceAssociate's degree (A.A) or equivalent from two year college or technical school. Two to four years related experience and/or training; or equivalent combination of education and experience.

Certifications, Licenses, and Other Special RequirementsServSafe Certified. Obtain/hold any local, state and/or county required food handling/sanitation licenses and/or certificates.

Management and Decision MakingApplies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these.

Knowledge and SkillsPossesses extensive knowledge of a distinct skill or function and a thorough understanding of the organization and work environment. Has working knowledge of a functional discipline. Effective customer service skills and the ability to lead and supervise a diverse group of people. Ability to coordinate the daily production of menus, recipes and food preparation. Capable of managing multiple job tasks including the preparation of several foods at the same time. Excellent cooking skills and an understanding of food preparation principles, ingredients and culinary terminology. Ability to modify and develop creative recipes and menus that are healthy and satisfying to the residents. Ability to understand and apply sanitation principles.
