Responsibilities
Provides direct supervision of care giving within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community.
Provides direct supervision of resident care staff, which includes monitoring job performance, collaborating with community leadership on associate performance evaluations, coaching and counseling associates and conducting and documenting corrective action as needed.
Coordinates training and education of resident care staff on an ongoing basis.
Verifies all associate hours on time cards and weekly hours report. Completes nursing schedules to assure proper coverage to meet resident needs and according to budget. This includes arranging replacement staffing when necessary and maintaining clear records of all changes, requests and irregularities.
Observes residents health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to Health and Wellness Director (or designee) in a timely manner.
Assists the Health and Wellness Director (or designee) in medication management, ordering medication from pharmacy and maintaining records as dictated in Brookdale Senior Living policies and procedures manual. Assures that medications are properly stored, medication carts/cupboards are cleaned and medication room is organized according to standards.
If within scope of practice, may administer medication to residents; may review capabilities of residents who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation).
Monitors staff in administration of medication, to include frequents audits of medication administration records (MAR).
Observes residents, records significant reactions, and notifies community leadership of resident's condition and reaction to drugs, treatments, and significant incidents. Responds promptly to all emergency calls from residents.
Communicates to families regarding resident's personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments. Assigns vital signs to associates and monitors compliance. Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through.
Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families.
Participates in the maintenance of resident charts including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately.
Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by community leadership.
Performs daily, weekly and monthly safety and infection control checks of the home and grounds, assisting the community leadership for resident health and safety purposes.
Participates in community functions including family socials and open houses. Communicates with community professionals.
Shares on-call duties as required.

Responsibilities
Leader responsible for the community's daily operations, associate relations, resident and resident family engagement and connection, financial performance, and regulatory compliance. Creates and executes the business plan and develops the team to become a preferred senior living community in the market, while growing revenue and profitability in partnership with the district team. Works with sales to drive sales results. Creates an inclusive community culture that provides high quality resident experiences and care and engages residents, families, and associates.
Is responsible for all operations within the community, interacting with staff and residents, prospects, or their family members or representatives, as necessary; builds and maintains strong working relationships with management team.
In connection with the district team, develops and implements business plan to become a preferred senior living community in the local market and creates, analyzes, and executes annual operating and capital budgets; reviews and manages community performance against goals; sets plans to improve or maintain performance to Company service and financial standards.
Acts as a hands-on leader who supervises, directs, and motivates community staff; provides assistance as needed. Encourages teamwork and collaboration and cultivates an inclusive community culture. Ensures community maintains appropriate staffing levels to meet the needs of residents and in accordance with applicable legal requirements.
Attracts, develops, engages, and retains associates in accordance with Company policies. Responsible for recruiting and hiring high quality and engaged associates and ensuring they are appropriately trained and developed to meet the needs of residents. Reviews promotions, development plans, disciplinary actions, and termination decisions ensuring consistency in the selection and retention of quality associates. Analyzes trends in recruiting, turnover, and retention to continually improve community performance.
Builds high degree of resident satisfaction and retention. Is responsible for maintaining positive resident relations and is accessible and approachable to residents and their families. Is proactive in solving problems and resolving issues with support from district leaders. Leads efforts to leverage satisfied residents and families to grow community occupancy. Partners with the Resident Council as necessary. Administers resident satisfaction and other surveys and works with district team to create plan to address opportunities for improvement in resident satisfaction, experience, and engagement.
For assisted living or memory care communities, creates collaborative relationship with community clinical leader to ensure community's care and services are appropriate to meet the needs of residents. Oversees resident services including the admission process, healthcare management, and maintenance of resident documentation to ensure high quality services and compliance with Company policy and applicable legal requirements. Assists in developing and conducting service plan reviews, consistent with applicable legal requirements, with appropriate clinical care team members and resident families to maintain the personal dignity of residents.
Demonstrates a high degree of financial acumen as it relates to community operations; works to meet or exceed budgeted revenue and profitability; and proactively creates plans to overcome unanticipated expenses or revenue shortfalls. Continually explores means of revenue enhancement and expense reduction, while meeting the needs of residents and adhering to Company policies and applicable regulations.
Drives sales and marketing efforts in collaboration with community sales leader to meet or exceed occupancy or revenue targets by developing new business, generating leads, and building strategic relationships. Engages with prospects who tour the community. Identifies trends and implements approved sales and marketing activities and strategies to maximize revenue (RevPAR).
Identifies and builds positive relationships with local influencers and professionals to raise community profile. Becomes active in social and civic affairs of the local community that align with the Company's mission. Represents the community and the Company to governmental agencies (as appropriate), professional organizations, community groups, and other appropriate agencies and groups.
Ensures buildings, grounds, and property are up to Company standards through the oversight of preventative maintenance systems and programs and frequent inspections that meet Company standards of excellence.
Enforces current Company policies and procedures. Maintains applicable licenses in accordance with Company, Federal, State, and local requirements.

Qualifications
Education and ExperienceHigh school diploma or GED required. Bachelor's Degree preferred. Minimum of two years in operational leadership experience required. Additional years of experience can be substituted for the education requirement on a year-for-year basis.
Certifications, Licenses, and Other Special RequirementsMust have a valid administrator license in states where required. Must have a valid driver's license and access to a private vehicle for business use.

Responsibilities
Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met.

For assisted living or memory care communities, creates collaborative relationship with community clinical leader to ensure community's care and services are appropriate to meet the needs of residents. Oversees resident services including the admission process, healthcare management, and maintenance of resident documentation to ensure high quality services and compliance with Company policy and applicable legal requirements. Assists in developing and conducting service plan reviews, consistent with applicable legal requirements, with appropriate clinical care team members and resident families to maintain the personal dignity of residents.
Demonstrates a high degree of financial acumen as it relates to community operations; works to meet or exceed budgeted revenue and profitability; and proactively creates plans to overcome unanticipated expenses or revenue shortfalls. Continually explores means of revenue enhancement and expense reduction, while meeting the needs of residents and adhering to Company policies and applicable regulations.
Drives sales and marketing efforts in collaboration with community sales leader to meet or exceed occupancy or revenue targets by developing new business, generating leads, and building strategic relationships. Engages with prospects who tour the community. Identifies trends and implements approved sales and marketing activities and strategies to maximize revenue (RevPAR).
Identifies and builds positive relationships with local influencers and professionals to raise community profile. Becomes active in social and civic affairs of the local community that align with the Company's mission. Represents the community and the Company to governmental agencies (as appropriate), professional organizations, community groups, and other appropriate agencies and groups.
Ensures buildings, grounds, and property are up to Company standards through the oversight of preventative maintenance systems and programs and frequent inspections that meet Company standards of excellence.

Monitors residents within state licensure regulations; may supervise and direct resident assistants where allowed by state licensure regulations. Assists in maintaining a physical, social and psychological environment in the best interest of residents.
Obtains and administers medication and treatments, as prescribed by physician. Documents and reviews medication and treatment sheets for accuracy and compliance with physician orders, professional standards, federal and state regulations and company policies. Assists in implementing and revising service plans or resident care plans based on resident needs. Monitors responses to treatment plans. Possesses knowledge of medication interactions and notifies Resident Care Coordinator and Director of any occurrences.
Plans and organizes quality control programs and activities. Maintains library of quality control and assessment materials necessary for complying with current standards and regulations and maintaining quality resident care.
Oversees the Infection Control Program of the facility and devises infection control reports for submission to Director, Clinical Services and Quality Assurance Committee. Ensures adherence to OSHA regulations and infection control standards.
Directs and coordinates all in-service educational programs of the facility in accordance with current Federal, State and Local Regulations. Develops, maintains and directs orientation programs for skilled nursing personnel. Conducts regular or special in-services for staff to ensure they remain current on new procedures. Develops, maintains and secures records, reports and instructional manuals, reference materials, pertinent to in-service programs. Uses facility consultants and others for special in-services.
Plans, implements, evaluates, and directs the facility's risk management programs and activities in accordance with guidelines issued by the governing board and administrator. Investigates all allegations of misconduct. Notifies appropriate personnel of the investigation. Reports findings to the administrator and other appropriate personnel in accordance with state and federal law.
Ensures appropriate professional staff maintains adequate liability insurance. Ensures licensure and background checks are conducted in accordance with company policies.
Assists in developing and implementing plans of correction for cited deficiencies. Ensures such plans incorporate timetables and methods of monitoring to prevent such deficiencies from reoccurring.
Ensures all personnel attend and participate in annual in-service training programs relative to hazard communication. Provides leadership in formulating the goals and objectives of the in-service programs of the Department. Assists the staff in setting educational goals. Meets with supervisors on a regular basis to solicit advice concerning training needs and assists in identifying and correcting problem areas and/or the improvement of services.
Counts controlled substances with off going nurse or resident assistant; secures and corrects reconciliation of controlled substances during shift. Ensures cleanliness and organization of medicine supply room and security of discontinued medicine in accordance with federal and state regulations and company policy.
Assists with discharge process relevant to medication/treatment orders.
Gathers and analyzes resident outcomes through tracking and trending to identify root cause and determines appropriate interventions.
Provides recommendations and reports to management, as necessary/required, concerning quality control programs and processes. Makes recommendations to facility management to reduce the frequency and severity of medical malpractice and resident/staff injury claims. Reports all accidents and incidents to appropriate agencies and facility management within established time frames.
Works with community management, public agencies, insurance adjusters, attorneys, and claimants to obtain and provide information necessary to resolve disputes.

Qualifications & Skills
Education as required to obtain state nursing license and state nursing license (LPN/LVN or RN)
Driver's license
Minimum of 3 years relevant experience, and Clinical leadership experience preferred.
Strong working knowledge of technology, proficiency in Microsoft office suite and electronic documentation.

Responsibilities
Manages the day-to-day clinical services of the community to ensure residents' healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as residents' healthcare needs change. Supervises and provides leadership, as well as coaching, to licensed nurses and other direct care staff within the community. The HWD level for each community is determined based on the total complexity of the role. Complexity criteria include, but are not limited to, factors such as size, type of product lines, medication management regulations, 90-day assessment requirements, multiple licensure requirements, state regulatory complexity, and skilled services requiring an RN.

Requirements to be a Maintenance Technician
Maintenance Technicians will maintain scheduled and unscheduled maintenance on, but not limited to: HVAC, elevator, fire and sprinkler equipment, generators, landscape, kitchen equipment and emergency pull cord equipment. Maintenance Technicians will also perform general maintenance projects as scheduled or assigned and respond to maintenance requests by community directors and residents in both private rooms and public living areas. They will also maintain positive communication with local fire marshal, building inspector and any other jurisdictions governing the community.

Leads the day-to-day operations of a larger community of 60-65 units and two product lines or 65-175 units with up to two product lines. Is responsible for the community's associate relations, resident and resident family engagement and connection, financial performance, and regulatory compliance. Creates and executes the business plan and develops the team to become a preferred senior living community in the market, while growing revenue and profitability in partnership with the district team. Works with sales to drive sales results. Creates an inclusive community culture that provides high quality resident experiences and care and engages residents, families, and associates.
Responsible for all operations within the community, interacting with staff and residents, prospects, or their family members or representatives, as necessary.
Develops and implements business plan to become a preferred senior living community in the local market and creates, analyzes, and executes annual operating and capital budgets; manages community performance against goals; sets plans to improve or maintain performance to Company service and financial standards.
Builds and maintains strong working relationships with management team and encourages teamwork and collaboration; cultivates an inclusive community culture. Empowers department leaders to supervise, direct, and motivate staff and to proactively recognize and solve issues. Holds department leaders accountable for department performance. Provides assistance to leaders and staff as needed. Ensures community maintains appropriate staffing levels to meet the needs of residents and in accordance with applicable legal requirements.
Supports department leaders to attract, develop, engage, and retain associates in accordance with Company policies. Responsible for recruiting and hiring high quality and engaged associates and ensuring they are appropriately trained and developed to meet the needs of residents. Reviews promotions, development plans, disciplinary actions, and termination decisions ensuring consistency in the selection and retention of quality associates. Builds high degree of resident satisfaction and retention. Is responsible for maintaining positive resident relations and is accessible and approachable to residents and their families. Is proactive in solving resident problems and resolving issues. Leads effort to leverage satisfied residents and families to grow community occupancy; executes renewal program with existing residents. Partners with Resident Council as necessary. Administers resident satisfaction and other surveys and works with department leaders and district team to create plan to address opportunities for improvement in resident satisfaction, experience, and engagement.
For assisted living or memory care communities, ensures collaborative relationship with community clinical leader to ensure community's care and services are appropriate to meet the needs of residents. Oversees resident services including the admission process, healthcare management, and maintenance of resident documentation to ensure high quality services and compliance with Company policy and applicable legal requirements. Empowers department leaders to demonstrate the same for their area. Ensures service plan reviews are conducted, consistent with applicable legal requirements, with appropriate clinical care team members and resident families to maintain the personal dignity of residents.
Demonstrates a high degree of financial acumen as it relates to community operations. Analyzes, develops, and executes annual operating and capital budgets; works to meet or exceed budgeted revenue, profitability, and occupancy goals. Proactively creates plans to overcome unanticipated expenses or revenue shortfalls. Continually explores means of revenue enhancement and expense reduction, while meeting the needs of residents and adhering to Company policies and applicable regulations.
Drives sales and marketing efforts in collaboration with the community sales leader to meet or exceed occupancy or revenue targets by developing new business, generating leads, and building strategic relationships. Engages with prospects who tour the community. Identifies trends and works with sales leader to implement approved sales and marketing activities and strategies to maximize revenue (RevPAR).
Identifies and builds positive relationships with local influencers and professionals to raise community profile. Becomes active in social and civic affairs of the local community that align with the Company's mission. Represents the community and the Company to governmental agencies (as appropriate), professional organizations, community groups, and other appropriate public agencies and groups.
Ensures buildings, grounds, and property are up to Company standards through the oversight of preventative maintenance systems and programs and frequent inspections that meet Company standards of excellence.
Enforces current Company policies and procedures. Maintains applicable licenses in accordance with Company, Federal, State, and local requirements.
