Position Summary
CVS Health is seeking a Nurse Practitioner (NP) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.
PRIMARY DUTIES AND RESPONSIBILITIES:
Responsibilities include but are not limited to
Patient-Centered Quality and Safety
Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
Counsels and educates patients and families on health maintenance and respond to patient care inquiries
Evaluates and treats patients' medical needs with safe and high-quality treatment
Documents all patient care within an electronic health record in accordance with current policies and procedures
Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients
Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease
Refers to appropriate specialists and patient care components as appropriate
Maintain patient confidentiality in accordance with PHI and HIPAA standards
Communication and Interdisciplinary Collaboration
Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care
Participates in and in some cases leads clinical care team huddles

On call rotation shared with providers in market for after hours patient clinical needs or shift coverage

Required Qualifications
A minimum of 1 year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.
Internal applicants with
Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
Current National Board Certification in Family Practice

While performing the duties of the job, the Primary Care NP is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment.

Required Qualifications
A minimum of one year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred.
Active Family Nurse practitioner license in good standing
Provide holistic, evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. This includes education and treatment for pregnancy prevention, sexually transmitted infection (STI) Prevention and safer sex practices.
Order and administer immunizations and injections (to include but not limited to COVID19, influenza, TB, Depo) and provides evidence-based patient/parent vaccine counseling and education.
Minimum of two years of medically relevant experience or equivalent
Complete the DOT certification exam within 30 days of employment
Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish
Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).
Pass a FIT test and wear the provided respirator mask when protocol requires.

The Medicaid senior manager, network management:
Negotiates, executes, conducts high level review and analysis of dispute resolution and/or settlement negotiations of contracts with national providers including, but not limited to labs, Home Health, Home Infusion, Urgent Care Centers, vendor, in accordance with company standards to maintain and enhance provider networks while meeting and exceeding accessibility, quality and financial goals and cost initiatives.

You will:
Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.

Position Summary
This is a great opportunity to join our prescription entry team while working from home. As a pharmacy technician, you will be responsible for accurately and efficiently reading, interpreting, and entering prescriptions into our system. You will be essential to ensure patients receive their correct prescriptions in a timely manner, directing questionable prescriptions to the prescriber for clarification, and entering complete prescription information once clarification is received. The Medicaid senior manager, network management:
Negotiates, executes, conducts high level review and analysis of dispute resolution and/or settlement negotiations of contracts with national providers including, but not limited to labs, Home Health, Home Infusion, Urgent Care Centers, vendor, in accordance with company standards to maintain and enhance provider networks while meeting and exceeding accessibility, quality and financial goals and cost initiatives.

The Retail Store Associate position provides an opportunity, in a leading retail setting, to excel in a growing, high impact, customer focused role, working both independently and as a member of a team, to positively impact the lives of others.
Essential Functions:
Providing differentiated customer service by anticipating customer needs, demonstrating compassion and care in all interactions, and actively identifying and resolving potential service issues
Focusing on the customer by giving a warm and friendly greeting, maintaining eye contact and offering help locating additional items, when needed
Accurately perform cashier duties - handling cash, checks and credit card transactions with precision while following company policies and procedures
Maintaining the sales floor by restocking shelves, checking in vendors, updating pricing information and completing inventory management tasks as directed by store manager
Supporting opening and closing store activities, when needed
Providing customer support to all departments, including photo and beauty, ensuring departments are fully stocked and operational while remaining current with all updated services and tools
Assisting pharmacy personnel when needed, including working regular shifts in the pharmacy as part of opportunities for growth and career development
Embracing and advocating for new CVS services and loyalty programs that support our purpose of helping people on their path to better health
Required Qualifications
At least 16 years of agePhysical Requirements:
Remaining upright on the feet, particularly for sustained periods of time
Lifting and exerting up to 35 lbs of force occasionally, up to 10 lbs of force frequently, and a negligible amount of force regularly to move objects to and from, including overhead lifting
Visual Acuity - Having close visual acuity to perform activities such as: viewing a computer terminal, reading, visual inspection involving small parts/details

Pay Range
The typical pay range for this role is:
$15.00 - $18.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
