The Home Care Registered Nurse (RN) role provide services in the comfort of the patient's own home. RN will be trained in both Home Health and Hospital Care at Home so that patients experience the full continuum of care with one integrated care team. RN will work with an effective team to communicate, assess, plan, implement, evaluate, supervise, and coordinate services in accordance with professional standards of nursing and procedures of Essentia Health. Home Health is a model of care allowing patients to benefit from nursing, therapy, social services, home health aide, infusion therapy, and nutrition therapy. This role offers the unique opportunity to work directly with the patient and their providers for either short-term recovery or ongoing care needs. The RN develops deep relationships with patients, successfully engaging them with the resources for improved health and quality of life.
Hospital Care at Home brings the essential elements of inpatient hospital care to the comfort and convenience of the patient's home. Like a hospital RN, this role will be responsible for the patient's care from admission to discharge. The RN performs a comprehensive nursing assessment, medication reconciliation, safety assessments, and implements standardized protocols. The RN will utilize a variety of remote technology options. Required Work Experience:1-year clinical nursing experience
Education Qualifications:
Successfully graduated from an accredited nursing program.
Licensure/Certification Qualifications:
Must be licensed Registered nurse in ND or MN or be able to obtain within 3 months.
Basic Cardiac Life Support (BCLS) certification or ability to become certified within 1 month from date of hire.
Possession of a valid motor vehicle operator's license as verified through an appropriate licensing agency.
Proof of current motor vehicle insurance.
Must complete and pass a state required background check
FTE:
Casual (as needed) up to 40 hours per week

Shift Rotation:
Days and Evening Options- 4, 8, or 12-hour shifts
Premium pay for evening and weekend positions
Shift Start Time:
0700-0900
Shift End Time:
1630-1930
Weekends: Every 3rd to 6th Weekend
Holidays: Every 3rd to 6th Holiday
Call Obligation: On a rotating basis

Compensation:
Credit given for prior nursing experience. Mileage reimbursed at the IRS rate.
Licensure/Certification Qualifications:
West Market:
Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency.
Proof of current motor vehicle insurance.
Detroit Lakes and Graceville: Must meet the driving requirements and criteria acceptable to Essentia Health's insurer.
Fargo Location Only: Current Registered Nurse license in the state of North Dakota and Minnesota upon hire.
