Maxim Healthcare Services in Carlsbad is looking for a part-time Occupational Therapist to take on 6 visits per week with 6 different patients (Adults/Pediatrics) located throughout North County of San Diego.
Compensation: $ 70 per hour - $9 5 per hour
The Occupational Therapist (OT) is responsible for performing client evaluations, developing and providing therapy services, and documenting services in accordance with the plan of care developed for each patient.
Essential Duties and Responsibilities:
Provides skilled occupational therapy services/interventions in accordance with physician orders
Regularly reevaluates client for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriate
Assesses patient needs, plans for, evaluates and modifies care to meet goals of Occupational
Therapy Interventions
Collaborates with all disciplines to plan and evaluate team goals for each patient
Instructs patient/family/caregivers on patient's total occupational therapy program
Advises, consults and instructs members of the multi-disciplinary team in specific occupational therapy modalities
Observes and reports patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team members
Directs OT treatment and develops occupational therapy plan of care; determines priority need for OT
Refers individual to other disciplines as deemed appropriate and as approved by the physician
Reports adverse signs and symptoms to the physician
Reports client's reaction to treatment or changes in condition to physician/Director of Clinical
Services or designee
Teaches new skills or retrains clients in once-familiar daily activities that have become difficult or unfamiliar due to illness or injury
Establishes equipment needs to facilitate independent living
Instructs individual and/or family/caregiver in the use of various types of adaptive equipment to\ improve function; ensures equipment is in good working order
Trains individual and/or family/caregiver in use of prosthetic and/or orthotic devices
When client is planning to return to work, OT performs work capacity evaluations and may refer individual to specialized vocational training in accordance with policy and regulations
Provides accurate and timely documentation according to the plan of care
Participates in care conferences or other meetings as required to ensure coordination of home health services (if applicable)
Participates in quarterly medical record reviews (if applicable)
Prepares and submits clinical progress-summary based on the progress of goals
Provides in-services/education programs for various disciplines as needed
Participates in peer consultation process
Supervises Occupational Therapy Assistants according to company policies/procedures and
State regulations
Supervises Certified Nursing Assistant (CNA)/Certified Home Health Aide (CHHA) according to company policies/procedures and State regulations (homecare setting only)
Completes recertification and comprehensive assessments according to company's policy/procedures and state regulations
Participates in discharge planning
Complies with applicable local, state, and federal laws and regulations regarding professional standards and practice standards
May fabricate splints and instruct individual and/or family/caregiver in the use, application and benefits
Performs other duties as assigned/necessary
Minimum Requirements:
Current licensure as an Occupational Therapist in the State of practice
Must be a graduate of an accredited school or program of Occupational Therapy
Maintains certification through the American Occupational Therapy Certification Board preferred
One (1) year of prior professional Occupational Therapy experience preferred; home care and/or facility experience preferred
Must possess and maintain current CPR certification in accordance with State regulation(s)
Current record of PPD or chest x-ray results
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required
Computer proficiency required
Must be at least 18 years of age
