Job Description:
Responsible to provide collaboration and coordination of care between providers, staff and patients for a defined population. Acts as a primary liaison between the MOUD providers, clinic staff, and patients. In conjunction with internal departments and disciplines, coordinates with community outreach programs, including screening and health promotion activities and education. The position will include direct patient care in the role of care coordination for a defined population. Ongoing collaboration with providers, staff, quality, regulatory, and reimbursing agencies is required for optimal care coordination across the continuum. Position requires knowledge and ability to research evidence-based practices, to analyze and interpret data to make program recommendations, and improve population outcomes in collaboration with system leadership. Is a professional practicing in a multi-faceted domain of specialty nursing that focuses on health care of individuals, families, groups, communities and entire populations. Provides care that occurs in primary and specialty care outpatient facilities, non-acute community out -patient settings; during tele health nursing encounters that occur in medical offices or by individual nurses in the home. Have authority, accountability and responsibility for nursing practice; makes decisions; and takes action consistent with the obligation to promote health and to provide optimal care recognizing that health is a universal right. Interacts with patients in concert with other health professionals, skillfully uses the nursing process to assess primary concerns, identify problems, analyze and integrate subjective and objective data, decide on a plan of action, apply the appropriate intervention and evaluate the outcome.

Licensure/Certification Qualifications:
Preferred previous Behavioral Health and/or Substance Use Disorder treatment experience.
Current Registered Nurse license with MN State Board of Nursing.Current Basic Cardiac Life Support (BCLS) certification or ability to become certified within 3 months from date of hire.
