Job Description:
In cooperation with the patient and in coordination with the patient's other health care providers, this position works to optimize a patient's drug therapy to improve the quality of life and health outcomes of the patient. The standard of care includes ensuring each patient's medications are assessed to determine that each is appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. Position requires high level of clinical skills, and the ability to build positive relationships with patients and colleagues, including other health care providers. Work Experience: Evidence of ongoing clinical practice by proof of 25 MTM patient visits, reflective of the scope and complexity of privileges requested in the previous 12 months or obtain within 3 months of hire.
Education Qualifications:
Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction.
If graduated prior to 1996 or from a College of Pharmacy other than the University of MN, North Dakota State University, or South Dakota State University, the pharmacist clinician must demonstrate curriculum commensurate Medication Therapy Management education, have completed a PGY1 or PGY2 ambulatory care residency or an ACPE accredited Medication Therapy Management training program. Copy of your training program certificate of completion is required.
Licensure/Certification Qualifications:
Active license to practice pharmacy in MN and ND.
Ambulatory - Immunization Delivery Certification completed within 1 year from hire date.
Ambulatory - BLS certified within 90 days from hire date.
