Job Description:
This position is responsible for the coordination of services for patients and families who are referred to the Hospice/Palliative Care Program. The coordinator receives the program referrals, assesses patient/family need and develops the initial plan of care in collaboration with the Hospice Interdisciplinary Team. The Referral/Provider Relations Coordinator is also responsible for the provision of philosophy and end of life care through education to referral sources, health care providers and community programs. Key Responsibilities - Responsible for assessment and coordination of services for patients/families referred to the hospice/palliative care program. - Establishes networking opportunities with provider groups, associates and other professional organizations which ensure continued contact with referral sources. - Provides education to health care professionals and community on end-of -life issues to raise awareness - Completes patient admissions as determined by the program needs. - Coordinates general inpatient care in hospice contracted facilities and Solvay Hospice House.

Qualifications:
Current nursing licensure in state(s) of employment. Ability to obtain Wisconsin licensure within 3 months of employment required to work in the Duluth Office.
Basic Cardiac Life Support (BCLS) required or the ability to obtain within 3 months of hire.
Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency.
Proof of current motor vehicle insurance.
Preferred Qualifications:
Current or recent (within 5 years) hospice RN experience.
Background in Marketing, Business, Sales, or customer service
Minimum of one year of recent adult ICU, Emergency Department or acute care obstetrics experience.
Minimum of two years inpatient acute care, long term care, nursing home and/or home care experience.
