Job Description:
Develop connections with various community individuals and organizations to introduce them to the long-term care environment. Assist new employees to develop skills and relationships in the long-term care facility. Assist employees to identify and utilize resources to reduce social-economic barriers to employment. Develop plans to provide residents with access to life-long learning and new relationships with employees and community organizations.
Education Qualifications:
Key Responsibilities:
Coordinate the individual experience for the volunteer/student employee/staff/resident, including the introduction and celebration of new residents and staff
Guide and support nursing assistant students through onboarding and training
Coach newly hired nursing assistants through self-discovery and basic workplace skills
Facilitate career development
Make appropriate resource connections to enable nursing assistants to achieve acceptable employment performance if experiencing socioeconomic barriers
Establish relationships with community organizations to provide residents with events and activities with community groups
Collaborate with facility leaders and the facility project team to assure various project goals are met
Coordinate celebrations for project milestones
Educational Requirements:
Associate degree in behavioral science, education, social work, or related health care field
OR
1 year of experience with career counseling, case management, community relationships, social work, workforce development or similar work

Ability to demonstrate empathy and active listening skills
Excellent communication and organizational skills
Completion of the nursing assistant on-line curriculum within 3 months

The approximate base compensation range is posted, but the actual offer will reflect our total compensation and benefits package. The offer will be determined by a number of factors including the selected candidate's experience, knowledge, skills, abilities, FTE, as well as internal equity among our team.
