Job Description:
Plans, coordinates, and implements Social Services for patients of all ages, patients' families, and other person significant to patients at the Medical Center. Anticipates and attends to patient's specific psychosocial needs, financial needs/concerns, equipment needs, educational needs, and community resources from admission through transfer to the next appropriate level of care. Assists with discharge planning, grief counsel, financial application, and access to community resources as needed. Enhances patient care, thus contributing to the goals and mission of the Medical Center.
Education Qualifications:
A bachelors degree in social work (BSW).
If Social Service Department (45325 or 45320), a bachelor's degree in social work (BSW) or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling and psychology. Licensure not required if degree is in a human services field.
Licensure/Certification Qualifications:
Current State Social Work Licensure.
