Provides therapeutic services and diagnostic services to patients and families. This is to include but is not limited to individual, group, and family therapy, mental health intakes and assessments, diagnostics, and coordination of treatment services for the patient. May be required to provide Minnesota DHS Clinical Supervision requirements as required for Partial Hospitalization Programs.

Education Qualifications:

Master degree in social work or psychology.

Seeking a full-time Licensed Independent Clinical Social Worker (LICSW) to become an integrated component of our primary care team based in Wahpeton, ND by increasing access for mental health and its support services. Our team of primary care physicians and advanced practice professionals utilize best practice methods to provide an environment that is engaged, innovative, value-driven and proactive in the care of our patients.   • 0.75-1.0 FTE, full-time position with FT benefits (will consider part-time interest also)

  • Monday-Friday from 8a-5p

  • Based in Wahpeton, ND, with outreach to Graceville, MN clinic.

  • 8-12 outpatient (no inpatient responsibilities)

  • Patient population all ages

  • Mental Health Training, MSW required

  • Management of psychosocial aspects of chronic and acute diseases.

  • Application of behavioral principles to address lifestyle and health risk issues.

  • Consultation and co-management in the treatment of mental disorders and psychosocial issues

FUNCTIONS:

  • Assists the primary care provider in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the primary care team;

  • Conducts client intakes, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care provider concerning the clients' treatment goals and plan;

  • Provides consultation and training to the primary care providers to enhance their skill and effectiveness in treating mental health problems;

  • Gives primary care providers timely feedback about the client's care, treatment recommendations and progress via documentation in the client's record and verbal feedback;

  • Assists, to the extent feasible, in the clients' community functioning by helping with public benefits, vocational rehabilitation, social support, housing, etc.

  • Appropriately documents the clients' progress and diagnostic information in the treatment chart to sync with EMR interface and billing

    QUALIFICATIONS:

  • Has excellent working knowledge of evidence based treatments for medical and mental health conditions.

  • Has ability to work through brief client contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions.

  • Is comfortable with the pace of primary care, working with an interdisciplinary team, and has strong communication skills.

  • Has the ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.

LICENSURE/CERTIFICATIONS REQUIRED:

  • Graduate of an accredited LICSW program and licensed/ability to obtain license in state of ND & MN.

  • Successful completion of Essentia Health credentialing process prior to practice.
