Job Description:

Responsible to provide leadership, program development and coordination between providers, staff and patients for a defined population. Acts as a primary liaison between the operations team, internal stakeholders and key community partners. In conjunction with internal departments and disciplines, coordinates community outreach programs, including screening and health promotion activities, and education. The position may include direct patient care in the role of care coordination for a defined population. Ongoing collaboration with providers, staff, quality, regulatory and reimbursing agencies are required for optimal care coordination across the continuum. Position requires knowledge and ability to research evidence-based practices analyze and interpret data to make program recommendations and improve population outcomes. Work Experience: Minimum of 3 years of nursing healthcare experience.

Education Qualifications:

Bachelor of Nursing degree preferred. Prior experience with MAT/MOUD, Chemical Dependency or Behavioral Health programs preferred.

Licensure/Certification Qualifications:

Current nursing license with the appropriate State Board of Nursing.
