POSITION SUMMARY
This position is co-located within the Crisis Response Center and assists in facilitating urgent engagements/enrollments for the Whole Health Clinic in the community for members not currently enrolled with ongoing outpatient behavioral health services. Assists members in securing personal, organizational, and community resources including initial appointments at the Whole Health Clinic. This position assists with enrolling members with AHCCCS and/or the Healthcare Marketplace as needed
CORE FUNCTIONS
Responds to urgent engagement requests within the Crisis Response Center and other community settings to complete intake assessments, including full ICC Demographic or 834 enrollment.
Completes AHCCCS and Healthcare Marketplace screenings as needed with members.
Follows up with coordination of care as needed to ensure appropriate first appointments are scheduled.
Completes progress notes, other patient record documents and writes comprehensive and timely reports/assessments in accordance with agency and funding source guidelines.
Assists in coordinating and planning out of home placements and discharges.
Accurately reads, records and interprets information, including assessments, measures, and diagnostic criteria.
Completes all required training by licensing, agency, and funding source or as requested by supervisor and participates in individual and team clinical supervision.
This position requires autonomous work; working independently in the community. Confers with a supervisor for routine staffing, general guidance and any additional support required. This position will engage in weekly supervision for additional support.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.
Must have 18 credit hours of post-high school education in a field related to behavioral health completed no more than four years before the date the individual begins providing behavioral health services and two years of full time behavioral health work experience; or four years of full time behavioral work experience.
Employees working at BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.
PREFERRED QUALIFICATIONS
Bachelor's degree and at least one year of full time behavioral health experience or has an Associate's degree and at least two years of full time behavioral health work experience preferred.
Additional related education and/or experience preferred.
