Job Description:
In collaboration with Interdisciplinary team plans, coordinates, and implements Social Services for patients of all ages, patients' families, and other person significant to patient at Essentia Hospice and Palliative Care. Anticipates and attends to patient's specific psychosocial needs, financial needs/concerns, equipment needs, educational needs, and community resources from admission through end of life, or transfer to the next appropriate level of care. Assists with grief counsel, discharge planning, financial applications, and access to community resources as needed. Enhances patient care, thus contributing to the goals and mission of the Essentia Hospice and Palliative Care.
Education Qualifications:
Master's degree in social work preferred,
Bachelor's degree in social work and at least one year of social work experience in health care setting
Bachelor's degree in psychology, sociology, or other field related to social and at least one year of social work experience in health care setting
compensation is based on candidate's social work degree
Licensure/Certification Qualifications:
Current State Social Work Licensure.
