The Community Health Worker (CHW) serves as a mission-aligned bridge connecting Community Health and residents and patients of the community. This role helps transform lives by providing services as a Community Health Worker helping communities improve access to basic/better health services and contributing to community development and improved quality of life. The position entails helping individuals and families by providing informal chronic disease and mental health counseling with (if need be) referral to healthcare providers, local health agencies, and community organizations.
General Responsibilities
CHWs are embedded in the community, fostering trust, and building on-going relationships
CHWs participate in local events, connect with residents in the community, and promote programs awareness
CHWs engage and mobilize residents, patients businesses, and organizations in ways that existing staff and volunteers may not have been able to do so
CHWs facilitate partnerships among organizations, agencies, and stakeholders, fostering collaborative spirit and leveraging resources for greater collective impact
CHWs track interactions with residents and patients in EPIC, UniteUs, and other data systems to ensure quality care and appropriate referrals.
