ProMedica Bay Park Hospital caters to communities east of the Maumee River. Our campus includes a state-of-the-art community hospital with 91 private beds and a professional office building dedicated to doctors' offices and outpatient rehabilitation, as well as a treatment and diagnostic center. The Registered Nurse assesses, plans, implements and evaluates the nursing care provided to a group of patients. This nurse leads the nursing team members in the care as directed by the attending physician and in collaboration with other health care providers in accordance with the philosophy and policies of ProMedica Acute Care Hospitals. Actively participates in the creation of an environment that fosters patient, family, physician and employee satisfaction. Scope of Service:

The Registered Nurse provides nursing care to individuals and groups which require specialized knowledge, judgment, and skill derived from the principles of biological, physical, behavioral, social, spiritual/cultural, and nursing sciences. The Registered Nurse functions within the full scope of nursing practice as noted by the Board of Nursing of their designated state for which they are practicing in addition to compliance with any of the hospitals accrediting bodies. At least 1 hour of contact hours must be related to Chapter 4723 of the Ohio nurse practice code and rules.

Personal Protective Equipment (PPE) - Demonstrates knowledge and use of standard precautions and PPE. - Demonstrates knowledge and use of safe patient handling equipment.

Physical Demands - Must be able to tolerate exposure to dust, fumes, chemicals, temperature changes, and exposure to blood borne pathogens and bodily fluids. - Must be able to attain all health requirements as identified by the organization (i.e., PPD, respirator training, vaccines, as appropriate). - Must be able to move about hospital and between workstations; and prolonged periods of standing. * - Must be able to frequently move, lift or carry light to heavy patients or equipment. * - Must be able to lift medium materials, non-material handling; continuous standing, walking, bending, stooping and reaching; frequent pushing, pulling and squatting.

POSITION SUMMARY Provide technical support to Ophthalmologist/Optometrist. Patient-care duties may include obtaining and recording of medical histories and patient ocular history, as well as performing patient specific ophthalmic testing/exams and administering eye drops prior to the patient seeing the provider.

ACCOUNTABILITIES

Follows up with pharmacies with renewals and prior authorizations as instructed by physician. Escorts patients from exam room to appropriate area (i.e.: reception area, dilating room, additional testing, or scheduling). Clean and sterilize equipment and maintain autoclave in accordance with established guidelines and procedures.

POSITION SUMMARY

This position is responsible for assessing potential risks and directing, developing, and coordinating programs, policies, and procedures to ensure HRI's compliance with federal, state, and local laws and regulatory requirements.

ACCOUNTABILITIES

Manage the day-to-day operation of the HRI Compliance and Delegation Oversight Program. Identify potential areas of non-compliance, FWA issues, areas of potential risk, potential strategic advancements, and regulatory changes, and develop and implement corresponding action plans including but not limited to reporting any areas of excessive risk directly to the Board of Directors. Coordinate responses to government investigations and other inquiries or demands. Identify, investigate, and provide necessary reporting of violations/potential violations of rules, regulations, policies, procedures, and standards of conduct to leadership and/or regulatory agencies Monitor proposed federal and state regulatory and policy changes and identify potential risks to the organization. Represent HRI with various state and federal trade organizations. Research and provide guidance on federal and state health care program statutes, regulations, rules, and guidance regarding reimbursement, conditions of payments, new offerings, coverage, and other material requirements. Research and prepare responses to complaints (provider and/or member) filed with the state Departments of Insurance, attorney generals' offices, Health and Human Services, and other state and federal administrative entities; identify trends/findings with significant policy/operational impact and address same. Proactively assess risks and execute the Compliance Plan, resulting in timely deliverables, and resolving and communicating issues/problems. Lead HRI's response to privacy and confidentiality concerns, authorization requirements, and informational notices and materials reflecting appropriate regulatory practices. Develop department-specific compliance training and education, as needed. Respond to Health Plan Partner Sponsor inquiries as needed. Chair the Health Resources, Inc. Compliance Committee and participate in various operational committees to ensure risk mitigation. Manage Company Compliance calendar ensuring all regulatory timelines are met. Manage Company Plan Filings process and monitor utilization of only filed products. Work in conjunction with the Legal and Compliance departments to assist with corporate regulatory requirements. Under the direct supervision of the Police Supervisor and Security Services Administrative Director, the police officer is responsible for protecting patients, visitors, employees and hospital property. The ProMedica Police Officer will perform patrols either on foot, vehicle or other means of ProMedica owned properties within the City of Toledo with a primary emphasis on ProMedica Toledo Hospital. The ProMedica Police Officer works to ensure a safe and therapeutic environment, working in partnership with clinical staff. The ProMedica Police Officer will enforce criminal and traffic laws, parking violations, engage in crime prevention initiatives, apprehend criminals and answer calls for service or safety issues. Duties will be performed in compliance with agency directives, standard operating procedures, and special orders or as otherwise directed by supervision.

Accountabilities

Patrols assigned areas. Provides law enforcement services for all ProMedica Toledo properties, employees, patients and visitors by patrolling assigned areas and taking action when necessary. Monitoring conduct of visitors on ProMedica property; questioning suspicious individuals when necessary. Investigates and reports all accidents, safety/fire hazards and security incidents involving loss, theft, damage, injury and recovered property. Prepares appropriate reports to document these incidents. Offers to care and assistance to visitors, guests, employees and patients. Must promote customer service and satisfaction at all times, treating all people in a courteous and professional manner. Must successfully complete all training required by ProMedica Health Systems and/or ProMedica Security Services as defined in organizational policies, procedures or by assignment. Must remain current in any certifications and knowledgeable in infection control and hazardous material measures. Must maintain annual fit testing requirements, BLS certification, and all other in-services required by JCAHO. Appears in court to testify. Enforces parking regulations and assist with traffic and pedestrian traffic on ProMedica properties. Enforces the law at a level that is consistent with organizational expectations, arresting individuals that are in violation of the law on ProMedica property. Works in partnership with the ProMedica community to create a safe environment for all.   3. Prepares intravenous admixtures and other sterile preparations (including chemotherapy and parenteral nutrition where applicable) safely and accurately using appropriate techniques. Only Pharmacy Technicians who have completed all required competencies will prepare intravenous admixtures

  4. Assists in ordering, receiving, unpacking, and storing medications and supplies in appropriate locations; Repackages bulk medications in unit-dose packages safely and accurately per established procedures using appropriate techniques

  5. Assists in completing all medication inspections and identifies and removes all outdated and unusable medications.

  6. Collaborates with other health care professionals in meeting departmental goals. Establishes and maintains mechanisms for communication and constructive problem solving with staff. Utilizes a team approach in decision-making and problem solving. Build trust and mutual respect.

  7. Performs training of new employees and in-services current employees in areas of specialization and/or expertise.

Position Summary:

The Behavior Technician assists in delivering behavior analysis services to children with autism under the direction and close supervision of the behavior analyst. The Behavior Technician is dedicated to providing the highest quality of safe care with compassion in partnership with each individual child, their team and their family. Primary duties include:

Implement approved ABA treatment plans including both skill acquisition programs and behavior reduction programs. Implement all programs and interventions using behavior technology appropriately, neutrally and consistently. Track and record child's progress by maintaining daily records including daily data collection on all goals and writing objective session notes. Participate equally with other team members in supervising and maintaining safety of children in all activities, both in and out of the center. Maintain professional communication with families through authorized daily notes. Maintain cleanliness and organization of learning environment daily. When children are not present assist with program needs that will enhance performance (creating new materials, schedules and reading pertinent research regarding applied behavior analysis, etc.)

The Document Imaging Coordinator reports to the Supervisor, HIM and has no direct reports.

POSITION SUMMARY

The HIM Document Imaging Coordinator ensures all patient health information is accurately stored in the electronic medical record imaging system. This includes transporting, prepping and quality checking paper documentation for scanning and indexing into the electronic medical record and may also include completing data integrity validation and correction requiring proficiency in multiple software applications, and ultimately preserving the integrity of the health record. Other responsibilities include reviewing documents sent to HIM for processing to ensure they are current and following standard guidelines for forms management. HIM Document Imaging Coordinator sorts/process all incoming mail for requests of information and ensures requests are uploaded according to HIPAA requirements. Promotes positive customer relations through prompt and courteous service in this fast-paced, high volume and customer-focused environment. Local travel between facilities is required.

ACCOUNTABILITIES

Retrieve patient information in support of patient care, ensuring prompt service at all times. Ensure each medical record document is correctly identified and labeled with the patient's name, medical record number and account number; take action when the information is not correct. Remains current with HIPAA privacy policies and procedures as they pertain to information contained in the medical record. Logs requests, request status and information released into the electronic Release of Information system and as necessary, any other secondary systems. Prepares documents for conversion, verifying that all documents contain the accurate patient identifiers, taking action when the information is not correct. Utilize scanning equipment to convert documents into the Electronic Health Record including the calibration and cleaning of the scanning equipment, correction of double feeds and poor images, operate all aspects of scanning software application, and joined with the quality processing of items through the scanning process. Electronically index documents in the imaging system while conducting a quality and accuracy process. Makes corrections at the time of indexing or reporting any issues to limit risk of data integrity concerns. Perform and monitor QA process for all documents scanned on the front end, meeting established quality and productivity standards. Utilizing EHR tools, reports all discrepancies to ensure corrections are made and integrity of the record is accurate. Reviews all documents scanned to the LMR and verifies that they are following standard guidelines. Reports any form issues, including bar code integrity, standardization, and content. This includes recognition and reporting of new or unapproved forms and documents. Prepare scanned documents and other department records for off-site storage vendor ensuring that boxes are correctly labeled according to policy and procedure. Maintains record storage areas so the folders and/or boxes are neat, orderly, and accessible. Performs all other duties as assigned.

POSITION SUMMARY

Key member of a patient-centric care delivery model for high-quality service within the hospital setting. Performs at or above the standards of care set for the superior patient experience in clinical and non-clinical settings. Will learn how to proficiently and effectively navigate through multiple information technology platforms/applications, equipment, and processes to provide the best support, practices/protocols and value-based care.

ACCOUNTABILITIES

  1. Maintains departmental equipment and facilities in designated areas in a clean, orderly condition and in compliance with infection control guidelines.

  2. Provides excellent patient service by documenting information accurately, promptly, and legibly in order to meet regulatory or medical record requirements. Utilizes medical charts, forms, practice management systems and all forms of electronic communication efficiently.

  3. Provides for a safe patient experience through adherence to clinical protocols, regulatory requirements, manufacturer standards and reports any potential equipment malfunctions or safety violations.

  4. Prepares equipment and supplies under the direction of clinical staff

  5. Demonstrates proficiency and skill in EKGs and other activities as directed by a licensed respiratory therapist or radiology technologist

  6. Transportation of patients

  7. Maintains clean and stocked exams or store rooms

  8. Assist clinical staff in procedures and task as directed

  9. Promotes a strong commitment towards achieving outstanding internal and external customer satisfaction and superior patient experience.

MICHIGAN: Licensed Pharmacy Technician or Pharmacy Intern License with Michigan Board of Pharmacy. Purpose:

Ensures coordination and continuity of care through effective transitions of care management as patients move in and out of the acute care environment.

Primary Duties:

Identify high risk patients by utilizing a standardized risk stratification process Develop preliminary transition plan with anticipated disposition, including contingency plan Collaborate with hospitalist and other key team members to manage transition of care activities and communicate vital information Actively participates in daily transition rounds Identify transition barriers and collaborate in comprehensive, patient-centered care plan development. Submitting nursing orders as needed, to assure timely transitions. Coordinate patient access to necessary services, including community and public health resources. Drives the transition process. Ask probing questions on progress (e.g. facility search, patient teaching, facilitate testing) Collaborate with care team to provide discharge/transition education. Serve as an essential link for patients without a primary physician. Work in partnership with acute and ambulatory care team to follow patient through care continuum and provide thorough hand-over Coach for self-management utilizing motivational interviewing Facilitate post-acute appointments as applicable Ensure timely documentation of all patient encounters Maintain positive working relationships with all key customers Attend applicable conferences, trainings and meetings Participate in quality improvement and strategic initiatives Complete special projects and other duties, as assigned

Must have active Registered Nurse licensure in state of assigned practices with an Associate degree or BSN (preferred) in nursing, At least 2 years of applicable nursing experience. Develops and administers physical therapy treatment programs to facilitate rehabilitation of physically disabled patients and to maximize function and independence. Job Description

ProMedica Toledo Hospital is one of the largest acute-care facilities in the region. This position performs diversified administrative support functions vital to the day-to-day clinical operations and revenue integrity for the department. In this position, one must possess and display strong interpersonal skills, communicate effectively to internal and external customers and respond to all inquiries in a timely, professional manner.

ACCOUNTABILITIES

Performs customer service functions of answering phones, recording and forwarding messages and greeting customers in a polite and timely manner. Registers patients in the system and rehab specific EMRs (Epic and WellSky) to ensure accurate billing. Task includes pre-registration and re-registration every 30 days. Verifies insurance coverage for all rehabilitation services and documents detailed coverage in the EMR. Must access various electronic insurance portals and phone trees to determine insurance coverage. Maintains accurate clinical schedules, scheduling patients for initial and recurring therapy visits in the rehab EMR. Ensures therapy plans of care are completed and routed in a timely fashion to comply with regulatory requirements and system-wide quality standards. Responsible for point-of-service collections, including co-pays and deductibles. Records and processes collection in accordance with system policies. Processes daily billing charges, completes charge reconciliation to ensure billing accuracy, and identifies and assists with correction of billing errors. Performs general office support including but not limited to typing, data entry, filing, copying and scanning. Assist with reports and data collection as directed. Performs other duties as assigned.

Education: High school diploma or GED.

Skills: Proficient in keyboard and data entry (requires 95 keystrokes/minute corrected). Knowledge and experience in use of computers. Experience with Microsoft Office software including Word, Excel, Power Point and Teams. Good interpersonal skills in order to positively represent both the department an the system to callers and visitors. Must be able to work calmly and efficiently under stress in a busy environment. Must be self-directed and detail oriented, with attention to accuracy.
