Manage a caseload of a minimum of 10 participants in treatment
Required: 10 hours per week within the hours of 9a-8pm local time. The 10 hours may be completed during the week or on weekends. Candidate can choose the set schedule that works for them, but it needs to be consistent once chosen.
$40-$60 per session hour (dependent on state of residence)
Primary Responsibilities:
Deliver remote care throughout an 8-week evidence-based program
Monitor participant progress, provide follow-up communications and care coordination recommendations, and document session notes
Participate as a member of the care team and assist participants as needed with accessing resources, as well as obtaining consent and authorization forms
Identify and respond to high-risk participant encounters with elevated safety concerns, following company protocols
Participate in regularly scheduled meetings with supervisors, monthly team meetings, and Town Halls
Maintain the highest standards of confidentiality as described in HIPAA Privacy & Security policies, and ensure compliance with annual training requirements

Required Qualifications:
One or more of the following active, unrestricted, North Carolina licenses:
Licensed Clinical Social Worker (LCSW)
Licensed Professional Counselor (LPC)
Licensed Mental Health Counselors (LCMHC) or state equivalent
Ability to maintain a consistent schedule for patients 10+ hrs/week
Preferred Qualifications:
Clinical experience conducting psychotherapy utilizing Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and other solution-focused approaches
Interest in obtaining licensure in other states
Proven passion about impacting lives and the future of behavioral care

Prescription requests
Entry, billing, and submittal of the customer's order to fulfillment for delivery assist with operations support, training, and process improvements
Contribute to updating Standard Operating Procedures (SOPs) and team content
Creation/ routing of pharmacy orders through receipt
Demonstrate handling both inbound and outbound member/patient calls
Prep and scan documents, verify member/patient data submit prescription orders in computer system
May contact members/patients/providers to validate data, request payment information and establish priority of order or delivery details

Position in this function works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
Primary Responsibilities:
Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
Completes follow-up and post-discharge assessments according to documented standard operating procedure
Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
Provides counsel, support and education to staff as appropriate
Actively participates in ongoing meetings pertaining to patient care and clinical excellence
Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
On-call for caseload during business hours. Participation in clinical after hours on-call program on rotational basis
Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
Perform related duties as necessary and other duties as assigned

Position in this function works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
Primary Responsibilities:
Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
Completes follow-up and post-discharge assessments according to documented standard operating procedure
Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
Provides counsel, support and education to staff as appropriate

This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 7:00am - 7:00pm CST and Saturdays (on rotation) between 7:00am - 5:00pm CST. It may be necessary, given the business need, to work occasional overtime or weekends.
We offer 3 weeks of paid training. The hours during training will be 9:00am to 5:30pm EST. Training will be conducted virtually from your home.

Primary Responsibilities:
Handle outbound calls to health care providers (i.e. physician offices, clinics) and insurance companies to identify the type of assistance the provider needs (EG. benefit and eligibility, billing and payments, authorizations for treatment, explanation of benefits)
Focus on resolving issues on the first call, navigating through complex computer systems to identify the status of the issue and provide appropriate response to caller
Deliver information and answer questions in a positive manner to facilitate strong relationships with providers and their staff
Complete the documentation necessary to track provider issues and facilitate the reporting of overall trends
This role is equally challenging and rewarding. You'll interact with providers with the intent to develop a relationship with them. Within a high volume setting, you'll need to develop knowledge of our various products and multiple levels of benefits within each product in order to best assist our providers/customers.

Required Qualifications:
High School Diploma / GED OR equivalent work experience
Ability in using computer and Windows PC applications, which includes strong navigation skills and learning new and complex computer system applications
Ability to successfully complete Customer Service training classes, complete all class assessments with a passing score, and demonstrate proficiency of the material
Ability to make outbound calls as needed
Must be 18 years of age OR older
Ability to work Monday-Friday, 7:00am - 7:00pm CST and Saturdays (on rotation) between 7:00am - 5:00pm CST
Preferred Qualifications:
1+ years of experience in a related environment (i.e. office, administrative, clerical, customer service, etc.) using phones and computers as the primary job tools
Previous experience handling incoming and outgoing phone calls
Prior pharmacy experience

Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

Position in this function works as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography. Dedicated to improving the health and well- being of patients, this position collaborates with the PCP, HBMC interdisciplinary teammates and consultants, and as needed, the health plan Medical Director and other health professionals, to develop and carry out patients' goals of care in all phases of the patient journey.
Primary Responsibilities:
Practices in accordance with the respective state laws and regulations governing the practice of advanced practice nursing or physician assistants
Establishes and maintains effective relationships with patients, care givers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff when applicable
Performs an initial comprehensive assessment on all newly enrolled patients and provides ongoing care thereafter. Develops a patient management care plan upon enrollment and updates it as needed when changes in condition warrant or following hospitalization. Meets with patients and/or their legal representatives to review newly developed or modified care plans; involves the PCP or supervising physician in these meetings, when applicable
Completes follow-up and post-discharge assessments according to documented standard operating procedure
Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions
Comfortable with basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc. New hire and annual skills check/training provided
Consults with hospital, emergency or post-acute clinical team following notification of patient transfer
Educates patients and/or their legal representatives in disease processes affecting patients and ways to manage them effectively, as well as to promote wellness
Provides counsel, support and education to staff as appropriate
Actively participates in ongoing meetings pertaining to patient care and clinical excellence
Implements HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care and to promote the improvement of care management and delivery
On-call for caseload during business hours. Participation in clinical after hours on-call program on rotational basis
Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews
Perform related duties as necessary and other duties as assigned
