The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with plan of care developed for each individual patient and physician's orders when applicable.

  • Performs patient reassessments and regularly reevaluates patient for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriate

    Responsibilities:

  • Responsible for assisting patient with and per plan of care, the following;

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient's laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

  • Must follow patients plan of care and complete all tasks assigned

  • Communicates in a constant, clear, and effective way with the client, family/caregiver and office

  • Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

    Requirements:

  • Meets all training requirements in accordance with state and federal laws when applicable

  • Ability to read, follow written instructions and document care given

  • Possess and maintains current CPR certification in accordance with state regulation

  • PPD or chest x-ray if applicable

  • Successful completion of Maxim screening examinations

  • Participation and completion of Maxim's competency program

  • Must meet all federal, state, and local requirements

The Entry Level Non-Medical Caregiver provides services to patients/clients in their places of residence that includes, but is not limited to, certain -hands-off- care. The provision of -hands-off- care as described above is contingent upon the licensing requirements of each state.

Responsibilities while on the job include the following:

Provides -hands off- care such as: providing and encouraging socialization, encouraging a mild exercise program, encouraging medication compliance through timely reminders to take medications, supervising the safety of the patient/client when bathing, toileting, and dressing, and assisting with grocery-shopping; and performing light housekeeping chores

Arrives at the client's home on time and leaves when shift schedule is complete

Maintains confidentiality of all client information, in accordance with HIPAA standards

Safeguards the client/patient by observing appropriate infection control procedures including but not limited to universal precautions

Notifies the office of any abnormal findings or conditions

Adheres to Companion Caregiver standards and procedures of the Company
