GENERAL SUMMARY
The Admission/Discharge/Transfer (ADT) Nurse is responsible for facilitating the admission/discharge/transfer of patients throughout the hospital. Reports to department leadership.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Completes forms and databases pertinent to patient admission.
Coordinates movement/transfers of patients between units, and transports patients, when necessary.
Assists with discharges of patients, including paper work, and patient care education.
Provides ongoing discharge education to patient families and staff, in order to support patient care and nursing staff.
Utilizes Teletracking software to communicate with units/departments across the health system to ensure optimum patient movement.
Works closely with Patient Placement Coordinator to ensure optimal patient care during transfers.
Provides developmentally appropriate psychosocial and clinical care respective to the assigned patient population.
Performs other duties as assigned.
LICENSES AND/OR CERTIFICATIONS
Current Virginia state license as a Registered Nurse or Registered Nurse holding a valid Compact State license.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
Bachelor's degree in nursing or related field with healthcare/management experience preferred.
Professional knowledge of areas of responsibility in order to assist in the planning and implementation of patient flow and bed placement activities.
Demonstrates excellent leadership, communication, and analytical skills with the ability to plan, supervise, develop, coordinate and maintain a professional work environment.
Previous nursing administrative supervisor, operation coordinator, or education coordinator experience a plus.
Critical thinking abilities a must.
Previous experience with patient tracking software preferred.
Demonstrates knowledge of integrated software applications and mobile technology preferred.
Demonstrates the ability to identify problems and implement solutions for operational and organizational functions.
Interpersonal skills necessary to direct subordinates and delegate tasks.
RN certification preferred.
Pediatric experience preferred.
CPR Training required and/or must be obtained within 45 days of hire.
WORKING CONDITIONS
Normal office environment with little exposure to excessive noise, dust, temperature and the like.
