    Base pay in range will be determined by applicant's skills and experience. Full-time associates in role are eligible for an annual bonus incentive and sales referral bonuses.

Responsibilities

Works with an assigned group of individual BAH agencies in a specific geographical area (limited to only one state) on agency development, referral sources and improved operations. Facilitates training and education opportunities for Brookdale at Home (BAH) associates in order to improve the delivery, volume and quality of Brookdale at Home operations. Implements BAH operations to ensure the integrity of clinical programs, systems, policies and procedures. Promotes commitment to corporate standards, codes of conduct and ensures compliance with local, federal and Medicare laws and requirements.

  • Collaborates with each assigned agency's Executive Director and Area Director/Manager BAH and/or Divisional Director, BAH for agency development. Assesses the needs of individual assigned agencies and sister Brookdale Communities that can be served by the host agencies to improve agency/network operations.

  • Will assist in the financial oversight of the assigned Brookdale at Home agencies-specifically dedicated to BAH revenue per day, expense control and development of continuous PRN assistance at the local level, to assist the communities in reaching all of their BAH financial goals.

  • Assures all assigned agencies quality improvement process is maintained.

  • Attends state surveys of assigned agencies and assists in response and/or plans of correction.

  • Assures personnel files of BAH associates in all assigned agencies meet compliance standards.

  • Available to agency leaders in assigned agencies working after normal business hours as a resource.

  • Will serve as an interim agency leader in the absence of a permanent agency leader whenever necessary.

  • Assists in recruiting and new hire training of agency leaders in assigned agencies.

  • Partners with each agency leader to execute marketing plans to achieve caseload goals.

  • Represents Brookdale's private duty service line and increases awareness through participation in outside events, professional groups, and community involvement in the local market. Uses relevant knowledge and research to plan, coordinate, and implement monthly prospect and/or referral source activities and events as specified by management and the agency marketing plan. Follows up and executes sales process with all leads from events.

  • Trains BAH staff in the following activities: initiating referrals; complying with CCR procedures; effectively participating in the SNF DC care planning process; appropriate marketing strategies to develop the BAH product.

  • Implements screening protocols that will identify Brookdale at Home candidates and any changes in the residents. Conducts trainings on screenings of residents at regular intervals to determine need for intervention/treatment.

  • Conducts reviews of operations including quality of delivered care and established clinical programs to ensure that the agencies are fully implemented within program settings. Implements plans for areas of improvement with clinical staff and makes recommendations for enhanced productivity and efficiency of program settings.

  • Implements state-of-the-art training programs to remove deficiencies improve care and educate staff on current and new reporting and clinical requirements or techniques. May develop training classes and materials or utilize existing training materials and classes to improve performance. May work with other Q1 staff to develop and deliver company-training initiatives.

  • Monitors current and changing regulations (CMS, FI and local guidelines) and recommends changes to company practice and policies to ensure compliance. Assist with developing and maintaining all policies relating to documentation standards.

  • Participates in special projects and initiatives as requested by Home Office and supervisors.

  • Participates in internal relationship building and marketing activities as they relate to clinic and agency development.

  • Fosters a positive working environment, works a flexible schedule to meet the needs of clinics.

  • Maintains positive relationships and rapport with coworkers, residents, patients, family members and community personnel.

  • Performs other duties as assigned.

Qualifications

Education and ExperienceBachelor's Degree preferred. Minimum of two years management or supervisory experience in home care or health related setting required.

Certifications, Licenses, and Other Special RequirementsMust have a valid driver's license and access to a private vehicle for business travel.

Management/Decision MakingApplies existing guidelines and procedures to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent, example, reasonableness or a combination of these.

Knowledge and SkillsAbility to effectively train BAH staff and coach them to improvement. Basic computer software skills including Word and Excel. Ability and willingness to be timely and manage travel and work schedules efficiently and effectively. Ability to maintain the highest standards of ethical professional and clinical judgment. Ability to address staff, residents and visitors in a courteous and friendly manner as well as accept constructive criticism. Ability to respect the privacy of residents and to refrain from discussing their personal situations and problems with other residents, particularly as it may relate to their health. Must be willing to communicate with residents of various levels of functioning and various types of personalities. Ability to speak and write English fluently in order to converse with residents as needed. Willingness and ability to assist in planning and executing the evacuation of residents during emergency situations. Willingness to communicate perceived physical and emotional needs of residents to other skilled staff to assist with addressing their care needs.
