The Registered Nurse assesses, plans, implements and evaluates the nursing care provided to a group of patients. This nurse leads the nursing team members in the care as directed by the attending physician and in collaboration with other health care providers in accordance with the philosophy and policies of ProMedica Acute Care Hospitals. Actively participates in the creation of an environment that fosters patient, family, physician, and employee satisfaction. Ensures decisions made are based on patient and family centered care philosophy utilizing evidenced based practices, and focused on safety, customer satisfaction, and quality outcomes
ACCOUNTABILITIES

Recognize the patient or designee as the source of control and full partner in providing compassionate and coordinated care based on respect for patient preference, values, and needs. Elicit and communicate patient values, preferences, and expressed needs. Provide patient-centered care with sensitivity and respect for the diversity of human experience in collaboration with the interdisciplinary team. Engage patients or designated surrogates in active partnerships that promote health, safety and well-being, and self-care management. Addresses patient/family education needs considering patient level of comprehension, style of learning from the point of entry through discharge. Communicate care provided and needed at each transition in care. Evaluates patient's response and intervenes to ensure optimal patient outcomes. 2. Integrate best current evidence with clinical expertise and patient/family preferences and values for delivery of optimal health care. Base individualized care plan on patient values, clinical expertise, and evidence 3. Formulates, manages, implements, updates, and evaluates the effectiveness of the individualized written nursing care plan and delivery of nursing care for each patient assigned. Demonstrates the ability to analyze problems and make sound judgments based on logic and/or critical thinking skills. 4. Use data to monitor the outcomes of care processes and use improvement methods to continuously improve the quality and safety of healthcare systems thus minimizing the risk of harm to patients and providers through both system effectiveness and individual performance. Administers medications and executes regimens as authorized by the state licensure. Uses quality measures to understand performance. Demonstrates effective use of technology and standardized practices that support safety and quality. Demonstrate effective use of strategies to reduce risk of harm to self or others. Use organizational error reporting systems for near miss and error reporting. Participates in unit-based quality improvement, evidence-based practice projects or research. 5. Use information and technology to communicate, manage knowledge, mitigate errors, and support decision making. Navigate, plan patient care and document ongoing status, interventions, patient responses and outcomes in accurate, timely manner in the electronic health record. Respond appropriately to clinical decision-making supports and alerts. Advocate protecting the Health Performance Improvement (HPI) of each patient. 6. Interact effectively with patient and families and within nursing and inter-professional teams to foster open communication, mutual respect, and shared decision-making to achieve quality patient care and positive health outcomes, and to enhance patient satisfaction. Functions as the coordinator of the patient's entire plan of care through interaction and communication with inter-professional teams. Act with integrity, consistency, and respect for differing views. Function completely within own scope of practice as member of the health care team; initiate requests for help when appropriate. 7. Influences the behavior of individuals or groups of individuals within their environment in a way that will facilitate the establishment and acquisition/achievement of shared goals. Demonstrates accountability for the delivery of standard-based nursing care consistent with the core values, mission, philosophy, and policies of ProMedica. Integrates leadership skills of systems thinking, communication, and facilitating change in meetings patients' needs. Participates in the change process to improve patient care, the work environment, and patient and staff satisfaction. Demonstrates accountability for own nursing practice. Assigns, directs, delegates, and supervises ancillary personnel and support staff in carrying out particular roles/functions aimed at achieving patient care goals. 9. Demonstrate awareness of and responsiveness to the larger context of health care system and will demonstrate the ability to effectively call on microsystem resources to provide care that is of optimal quality and value. Plans, organizes, and delivers patient care in the context of the work unit. Rounds with providers, and practices bedside report/hand-offs. 10. Escalates concerns utilizing appropriate chain of command to maintain safety, quality, and patient satisfaction. 11. May involve the physician, patient/family, and health care team in collaborative decision-making to modify the patient care plan. Serves the following age populations: infant, pediatric, adolescent, adult, and geriatric. Talks with physician, anesthesia prior to surgery to discuss needs of the patient, physician, and anesthesia. Utilizes nursing process in making nursing care decisions. Demonstrates use of the nursing process as a tool for problem-solving. Considers age related needs in determining plan of care. 12. Assesses patient status systematically to anticipate and respond to change in clinical condition. Includes age related needs in responding to clinical changes. 13. Performs and documents thorough assessment and initiates appropriate action in response to assessment data. Identifies the patient, surgeon, procedure, and site. 14. Participates in planning, implementing, and evaluating nursing care based on defined patient care plan, patient care policies, standards of care and age-related needs. 15. Prepares the operating room and equipment for each case. Responsible for establishing priorities and organizing assignments to provide flexible individualized patient care. 16. Orients patients to the operating room environment and assures someone is with the patient at all times. Assists anesthesia personnel throughout the procedure. 17. Recognizes and initiates appropriate action in critical situations. Able to summon assistance, as necessary. Stays with patient and initiates appropriate interventions as determined by situations. Readily locates and utilizes emergency equipment and supplies. 18. Able to properly assist the physician with the plan of care. Adapts to changes of pace and remains functional in emergencies. Demonstrates improved performance in critical situations with successive experiences. 19. Demonstrates skill competence in performing nursing procedures. Provides for patient safety in the O.R. by accurately carrying out nursing procedures. Able to perform work effectively on major/minor cases. Able to circulate, scrub or basic level assist in the O.R. as assigned. Maintains mastery in age related competencies. 20. Contributes to the development of unit policies, procedures, and standards of care. 21. Participates in staff meetings or on committees if asked. Ensures unit policy and procedures and standards are being adhered to. Assists in QA data collection. 22. Maintains effective interpersonal relationships with other departments, physicians, management, and other staff members. Motivated, takes self-initiative Communicates effectively both verbally and non-verbally. Acts on suggestions/criticisms positively. 23. Demonstrates sensitivity to the needs of patient/peers. Self-directs continuing related to the operating room/nursing. Begins development of beginning managerial role (i.e., wing leading). Works on CNOR certification. 27. Accepts responsibility for achievement of own professional goals and complies with the policies of ProMedica Toledo Hospital. 28. Performs other duties as assigned. Education: Current CPR training.

License: Current State license as a Registered Nurse.
Certification: Specialty certifications as required
WORKING CONDITIONS Personal Protective Equipment: Demonstrates knowledge and use of standard precautions and PPE. Demonstrates knowledge and use of safe patient handling equipment.
Physical Demands: Must be able to tolerate frequent exposures to latex and latex products. Must be able to move about the hospital and between workstations. Must be able to tolerate exposures to blood borne pathogens. Must be able to frequently move, lift and/carry medium heavy to heavy weight objects and materials.
