Titleiiiiiiiiiiiiiiiiiiiiiiiiiiii LifeHouse Family Strengthening CoordinatorReportsiiiiiiiiiiiiiiiiiiiiii Director of LifeHouseStatusiiiiiiiiiiiiiiiiiiiiiiiii PRN, Non-ExemptLocationiiiiiiiiiiiiiiiiiiiii Springfield, LifeHouse Crisis Maternity HomeApply Online:i https://www.ccsomo.org/employmentPosition SummaryThe Family Strengthening Coordinator is responsible for the case management of the Residents of LifeHouse, as well as the LifeHouse Aftercare clients.i Working with residents while at LifeHouse and Aftercare clients, to provide comprehensive case management, resource referrals and general support, the Family Strengthening Coordinator works within the family strengthening framework which emphasizes a strengths-based approach focusing on the empowerment of each individual.i The Family Strengthening Coordinator must have a commitment to the agency mission.Duties and ResponsibilitiesDuties will include but are not limited to:Commitment to Mission, Vision, and Values of Catholic Charities of Southern MO.Strict adherence to resident/client and program rules of confidentiality.Maintain professional staff/client boundaries.Communicate effectively, respectfully, and on an ongoing basis with LifeHouse Care Management Team, residents, volunteers, and other staff members.Provide comprehensive case management to LifeHouse residents and LifeHouse Aftercare clients.Work closely with the Director of LifeHouse to identify and develop programming and resources, on-site and off-site, for residents.Orient new residents and staff to the LifeHouse program.Supervise and coordinate resident class schedules and monitor attendance.Work with each assigned resident on establishing, completing, and documenting a self-sufficiency plan, including short and long term goals.Coordinate and oversee resident attendance at school, work, and other goal oriented programs.Establish and maintain a therapeutic relationship with residents.Provide support and guidance to residents on working towards and completing goals.Provide input to the Director regarding resident plans, including progress and recommendations.Conduct telephone and on-site intake assessments of new residents and case management assessments at program entrance.Record case activity on each resident in the client file/chart in a timely manner.Record resident data in agency database CaseWorthy, LifeHouse HMIS, and any grant data base collection requirements.Assists with outcome measurements and quality improvement initiatives.Assess and monitor parenting skills with residents with appropriate intervention, counseling, education, and support.Work with families to assess infant/children's needs and establish a parenting plan to ensure that physical, emotional, behavioral, and social needs of infants/children are being achieved.Oversee compliance with and completion of residence program responsibilities including daily chores.Attend community networking meetings as LifeHouse representative to develop relationships and resources within the community.Assist with transportation of resident to and from appointments as needed.Orient, train and supervise staff, student interns, and volunteers as needed.Attend staff meetings and house meetings (as required).Enforce LifeHouse standards for cleanliness.Represent LifeHouse professionally at all times.On-call time required, as well as emergency coverage when needed.Additional tasks and duties as instructed.Minimum QualificationsEducation, Licensure, and CertificationBachelor's Degree in Social Work or related field required. Must be at least 21 years of age and possess a valid driver's license and proof of current car insurance. Current CPR certification or the ability to obtain within the first 30 days of employment.
