GENERAL SUMMARY The Community Care Liaison is responsible for assisting patients and families during inpatient stays. The Community Care Liaison coordinates disposition meetings with appropriate stakeholders and manages commitment and detainment hearings. Provides support, resources, and case management services to families. Collaborates with multidisciplinary teams as necessary. Functions in an on-call capacity as necessary to meet the needs of the department and organization. Performs other duties as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES - Assist families with resources during their child's inpatient stay. - Coordinate disposition meetings and community care team meetings with family and providers - Coordinates commitment and detainment hearings. - Provide responsive collaboration with providers, guardians, and court representatives. - Participate in team meetings - Collaborate and maintain working relationship with regional Community Service Boards. - Supports parent/family/school communication as needed. - Consistently meets documentation standards for quality and timeliness. Maintain confidentiality of patients/families medical information in accordance with department and hospital policies. Adhere to established productivity measures. - Performs additional responsibilires as assigned by management.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS - Bachelor's degree in social work, human services, or related field, required. Master's degree in social work or related field preferred. - Understanding of TDO processed, preferred. - At least 1 year of applicable work experience with acute or crisis mental health care, required. - Background check required.
WORKING CONDITIONS Normal office environment with little exposure to excessive noise, dust and temperature.
