As an organization, we are leaders in excellence, we have a steadfast commitment to solutions, we respect all differences, abilities, and similarities in our employees and clients, and we advocate autonomy!The Community Living Mentor develops and conducts training programs for the Community Living staff including staff participating in the apprenticeship program through the Department of Mental Health (DMH).In conjunction with Program Supervisor, Program Manager and Program Director, develops, implements, and facilitates a comprehensive mentoring and training program for all new and current employees in the Community Living Department, incorporating the DMH Apprenticeship to develop community living staff to their fullest potential.Measures mentee progress and maintains documentation on progress of mentees during their DMH Apprenticeship and will provide a report to the mentees supervisors and to other management members as needed.Responsible for completing appropriate DMH Apprenticeship paperwork for each mentee and works with mentees at least weekly on a scheduled shift and meets with mentees at least bi-weekly outside of the mentees scheduled shift.Responsible for immediately reporting to Program Manager or Program Supervisors any staff performance or disciplinary issues.Actively works with staff to problem solve issues or concerns as they ariseInteracts with staff, agencies, parents/guardians, and providers in a manner that promotes the highest level of professionalism and teamwork.Presents a positive attitude, image, and demeanor when working with staff and when representing the Center in the community.Maintains regular, effective communication with Program Supervisors, Program Manager and Program Director.Keeps Program Supervisors, Program Manager and Program Director informed of developments, concerns, and program participant concerns.Advises Community Living RN of individuals medical concerns.Administers prescriptions and PRN medications as prescribed by physician.Determines when independent action is appropriate and when consultation with Program Supervisor, Program Manager or Program Director is appropriate before acting.Meets with staff teams to provide coaching and training.Completes Quality Assurance duties as assigned by reviewing competencies through the DMH Apprenticeship Program as well as CARF and DMH Waiver requirements.Maintains confidentiality of personnel issues as needed.May be required to work shifts in homes to ensure required staffing ratios and individuals served needs are met.May be assigned to work at other work locations based on staffing and individuals needs.Responsible for completing service notes at time-of-service delivery.Completes training as stated by CHS policy HRD-P-022 Training.Responsible for promoting a positive Safety Culture is expected and necessary to make CHS a safe. place in which to work. Safe work practices and policies for this position shall be reviewed and strictly followed.QUALIFICATIONS:AA/AS + 2-year related experience, 4-year degree, 4 years related experience or the combination of experience and education to equal 4 years.Strong communication skills both written and verbal required.OPENING DATE: August 11, 2022CLOSING DATE: Until FilledSHIFTS: FT (40 hrs.)
