Job Description

GENERAL SUMMARY The Clinical Care Access Coordinator is responsible for facilitating transitions of care, assessing patient needs, expediting utilization requests, connecting patients with CHKD and community resources, and supporting timely patient transitions to appropriate levels of care. Clinical Care Access Coordinators operate as part of a continuum of care to ensure patient and family access when transitioning levels of care. This position reports to the Practice Manager - Partial Hospitalization Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES - Assesses and coordinates care based upon patient need and provider recommendations. - Assesses patient presentation, triage biopsychosocial needs, and evaluates medical necessity criteria for identified programs. - Coordinates with family and providers prior to program enrollment, to assess need, provide resources, schedule assessments, and facilitate utilization review. - Provides responsive collaboration with providers, families, and managed care utilization review staff as needed. - Provides feedback to treatment team regarding access and barriers to resources. - Participates in team meetings and supervision. - Collaborates and maintains working relationship with referral sources, physician partners and managed care staff. - Consistently meets documentation standards for quality and timeliness. Maintains confidentiality of patients/families medical information in accordance with department and hospital policies. Adheres to established productivity measures. - Performs additional responsibilities as assigned by management.

LICENSES AND/OR CERTIFICATIONS - None required.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS - Master's degree in Social Work or Counseling required. - Minimum two years of clinical work experience preferred. - Minimum one year of work experience with pediatric population or families and intensive mental health care programs preferred. - Must possess an understanding of current community resources. - Increased Security Clearance may be required: FBI fingerprinting, criminal background check, and Child Protective Services registry search.

WORKING CONDITIONS Normal office environment with little exposure to excessive noise, dust, temperature and the like.
