Responsibilities
Assists the Director of Social Services in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of the community.
Meets with administration, medical, nursing staff, and other related departments in planning social services, as directed. Involves the residents/families in planning social service programs when possible.
Interviews resident/families in a private and confidential setting regarding personal information. Information obtained can include but is not limited to: personal and family problems, and past illnesses.
Provides consultation to members of our staff and community agencies, in efforts to solve the needs and problems of the residents through the development of social service programs.
Performs administrative requirements (completing necessary forms, reports, etc.), and submits completed forms to the Director.
Records and maintains regular Social Service progress notes indicating response to the treatment plan and/or adjustment to the new living setting.
Provides information to residents/families about Medicare, Medicaid, and other financial assistance programs available. Participates in community planning related to the interests and needs of the residents and families.
Refers residents/families to appropriate social service agencies when the community cannot provide for the services or needs of the resident.
Participates in discharge planning, development, implementation of social care plans, and resident assessments.
Compiles and maintains a social service directory listing available for community resources.
Compiles information on discharge plans and presents it to appropriate committee, as required.
Assists in making appointments and arranging transportation for the resident/family as requested or appropriate.
Assists in the development, administration, and coordination of department policies and procedures.
