As an organization, we are leaders in excellence, we have a steadfast commitment to solutions, we respect all differences, abilities, and similarities in our employees and clients, and we advocate autonomy!iProfessional service coordination work, which on a daily basis provides Targeted Case Management services to eligible children, adults, and their families.iSome duties include:Provides professional service coordination servicesApplies and exercises professional judgment and discretion in determining and securing Medicaid and non-Medicaid habilitative services to ensure both active treatment and maintenance of skills aimed at preventing or reducing out-of-home placement and/or institutionalization of individuals served.Supports individuals served and families to navigate complex requirements and regulations to receive appropriate program and/or residential services.Manages a caseload of moderate size and complexity, normally involving a limited number of disability areas and age ranges.Secures information such as medical, psychological, and social factors contributing to individualis situation to assist in completing the Individual Support Plan (ISP); secures supplementary information such as employment, medical and/or school records.
