Responsibilities
Provides strategy support and vision to the Brookdale at Home agency to achieve sales goals through effective business development efforts. Maintains and improves upon caseload and revenue production of the Brookdale at Home agency in accordance with marketing and business plans to include managing the sales process and completing all activities required for a sale. Focuses on business development for Brookdale at Home in a geographic cluster or major market (M3). Represents the Brookdale at Home agency and increases referrals through business development. Accountable for building the lead pipeline by developing referral relationships. Responsible for quarterly development and execution of market specific business development plan to achieve client caseload goals. Works directly with National Director of Sales & Marketing, Brookdale at Home.
Develops the overall strategy in the assigned market to generate new business and grow strong referral partnerships. Educates area medical professionals on Brookdale at Home as an essential part of the recovery process and facilitates partnership with both Hospice and Home Health to coordinate marketing efforts and streamline business-to-business sales calls.
Creates quarterly market specific business development plan to meet or exceed the established goals for professional leads. Contacts local sources including legal and financial professionals, senior organizations, appropriate special interest groups, hospital discharge planners, skilled nursing facilities, retirement communities, clergy, medical insurance providers, and other local community contacts.
Coordinates and executes outside networking presentations on behalf of the M3 market, with the primary focus of generating referrals for Brookdale at Home, but representing the full continuum of services available through Brookdale entities in the market
Provides strategy, support and vision for the BAH agency to achieve sales goals through effective business development. Utilizes established sales processes, systems, and forms for sales to perform job duties, track information, compile data and reports, and achieve desired agency caseload goals.
Develops and maintains relationships with and generates leads through clients, family, and professional referral sources on a weekly basis. Provides information and conducts presentations about Brookdale services and programs, market advantages, availability, and other relevant information to meet the needs of prospective referral partners.
Provides business development strategy and guidance in the M3 market to coordinate outside marketing efforts designed to spotlight Brookdale at Home, while representing the area Brookdale continuum to drive referral business for all Brookdale product lines.
Represents Brookdale's private duty service line and increases awareness through participation in outside events, professional groups, and community involvement in the local market. Uses relevant knowledge and research to plan, coordinate, and implement monthly prospect and/or referral source activities and events as specified by management and the agency marketing plan. Follows up and executes sales process with all leads from events.
Assists BAH management with client retention through new client welcome events, referral programs, outside visits to current hospitalized clients, and other programs as outlines in the marketing plan or by the National Director of Sales & Marketing BAH.
Partners with management to develop and execute marketing plans and achieve caseload goals. Researches and provides recommendations for content and delivery of brochures, flyers, press releases and other forms of media that promote services.
Visits with competitors on a quarterly basis. Creates and updates competitive analysis on a quarterly basis. Serves as a main contact for all competitive issues that would affect agency.

Qualifications
Education and ExperienceBachelor's Degree in Marketing, Business, or related field from an accredited college or university is preferred. A minimum of two to five years sales experience, preferably in the retirement industry or medical or pharmaceutical sales, with a proven track record of generating and closing a high percentage of qualified leads required; or equivalent combination of education and experience.

Ability to effectively listen and communicate verbally and in writing is essential.

The RN Care Manager serves as a liaison among Brookdale teams, residents, families, and healthcare providers within our communities, promoting coordination, communication, and collaboration at Brookdale's HealthPlus communities. This role encompasses the identification, coordination, communication, and implementation of services essential for enhancing residents' quality of life and achieving an optimal level of wellness. Works closely with primary and specialty care providers to formulate resident-specific care plans, ensuring that the delivery of medically necessary services aligns with established care plans. Serving as a resident advocate, they actively contribute to identifying and improving service delivery, closing gaps in care, and understanding key interventions and care protocols relevant to treating residents in place.
Educates and explains Brookdale's care pathways as determined by individual diagnosis and healthcare needs.
Monitors and communicates quality performance measures, and assists with training and education of teams to ensure quality outcomes for residents. This involves effective communication and collaboration with providers and other healthcare entities, such as managed care coordinators, insurance providers, and external healthcare partners responsible for coordinating resources and supplies pertinent to resident needs.
Reviews the current population's medical diagnoses to identify care needs. Assists in assessing the health, functional, and psycho-social status of residents.
Coordinates services required to enhance optimum wellness and manage chronic conditions.
Collaborates with hospice, palliative, and home health services/therapy as indicated.
Provides advanced care planning education to residents and families/responsible parties. Helps ensure each resident has a completed Advanced Care Plan, inclusive of goals of care. Communicates with healthcare providers to ensure care delivery is consistent with the resident's Advanced Care Plan(s) Participates in collaborative care reviews (CCR) and care conferences.
Provides resources for residents and families to make informed decisions regarding choices in meeting healthcare needs and effectively champions the benefits of residing and receiving care and services at the community. This includes informing them about opportunities to enhance care and the benefits of an IE-SNP plan or other benefit opportunities.
Applies quality improvement methodology to analyze, enhance, and manage outcomes, striving to achieve quality measure goals. Collaborates with Health and Wellness Directors (HWD) to ensure compliance with service plans, while working with providers to attain value-based outcomes for our residents. This includes coordinating and communicating care for each resident.

Responsibilities
Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met.
Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change.
Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Caregiver or Med Tech/Aide in order to meet the residents' needs and staffing requirements.

Responsibilities
Supervises the general operating maintenance and repair of the buildings and grounds to maintain proper care of the assigned community(s) in accordance with current federal, state, and local standards/regulations and company policies.
Supervises scheduled maintenance on: H.V.A.C., elevator, fire and sprinkler equipment, generator, landscape, kitchen equipment, and emergency pull cord equipment.
Responds to maintenance requests by residents for items needing repair in their rooms, including repairing plumbing, electrical, heating and replacing light bulbs.
Supervises repairs and maintains vacated rooms in a timely manner. This includes painting walls and cleaning carpets. Inspects completed work for conformance to standards and policies.
Ensures walls, floor coverings, doors, and woodwork in the common areas of the community are properly maintained.
Assists in the training of maintenance staff.
Maintains positive communication with local fire Marshall, building inspector, state elevator inspector, and any other jurisdictions governing the residence.
Conducts in-service fire safety training for all employees.
Repairs walls, floor coverings, doors, and wood work in the common areas of the community(s).
Assesses all plumbing and electrical problems within the community(s) and determines the best course of action to correct the problem. Works within the legal scope of local and state codes.
Reviews safety data sheets (SDS) as requested for potential exposure to hazardous chemicals used and takes necessary precautions.
Assess property damage and file property damage claims in accordance with company policy.
May directly supervise the maintenance technicians and building engineers, to include delegating tasks, hiring, training, evaluating performance, resolving disciplinary issues and firing.

Oversees and directs the sales efforts for multiple communities and products within district, major markets, networks and stand-alone communities. We are an upscale independent living retirement community located in North-West Houston (near Tomball). This position involves some security, but only on the inside of the building. This position will sit at our front desk to monitor cameras and life safety systems, this position will respond to resident emergencies by dialing 911, staying with the resident and then contacting the appropriate emergency contact. The security guard will conduct building rounds to ensure all is quiet, during the rounds the security guard will pick up any garbage and check all perimeter doors, making sure they are closed and locked. The security guard will contact the appropriate authorities in the event of suspicious activity, while also communicating to management any incidences and completing an incident report. The security guard will handle any minor maintenance issues, such as unstopping toilets, resetting breakers and helping the residents with tv troubleshooting, they will also contact the director of maintenance in the event of a major maintenance emergency. The position will complete a nightly shift log and note all issues and concerns. There is also the task of placing dinner orders through our POS system and answering the phone.
This position is full time, Monday through Friday from 4pm-12am. We are seeking a strong, reliable individual that can work independently and enjoys working with seniors.

Drives the bus or community vehicle as needed or as scheduled.
Handles minor maintenance emergencies (i.e. plugged toilets, reset breakers, replace light bulbs, etc.) as needed.

Responsibilities
Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met.
Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change.
Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Caregiver or Med Tech/Aide in order to meet the residents' needs and staffing requirements.
Assists in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and Caregivers and provides training and education to Caregivers on an ongoing basis to include classroom in-services and situation-specific training. In addition, they will also manage the associates schedules.
Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines .
Shares on call duties as required.
Education as required to obtain state nursing license (LPN/LVN or RN) and a minimum of 1-2 years relevant experience.
LPN or LVN license.
