Responsibilities

Provides strategy support and vision to the Brookdale at Home agency to achieve sales goals through effective business development efforts. Maintains and improves upon caseload and revenue production of the Brookdale at Home agency in accordance with marketing and business plans to include managing the sales process and completing all activities required for a sale. Focuses on business development for Brookdale at Home in a geographic cluster or major market (M3). Represents the Brookdale at Home agency and increases referrals through business development. Accountable for building the lead pipeline by developing referral relationships. Responsible for quarterly development and execution of market specific business development plan to achieve client caseload goals. Works directly with National Director of Sales & Marketing, Brookdale at Home.

• Develops the overall strategy in the assigned market to generate new business and grow strong referral partnerships. Educates area medical professionals on Brookdale at Home as an essential part of the recovery process and facilitates partnership with both Hospice and Home Health to coordinate marketing efforts and streamline business-to-business sales calls.

• Creates quarterly market specific business development plan to meet or exceed the established goals for professional leads. Contacts local sources including legal and financial professionals, senior organizations, appropriate special interest groups, hospital discharge planners, skilled nursing facilities, retirement communities, clergy, medical insurance providers, and other local community contacts.

• Coordinates and executes outside networking presentations on behalf of the M3 market, with the primary focus of generating referrals for Brookdale at Home, but representing the full continuum of services available through Brookdale entities in the market

• Provides strategy, support and vision for the BAH agency to achieve sales goals through effective business development. Utilizes established sales processes, systems, and forms for sales to perform job duties, track information, compile data and reports, and achieve desired agency caseload goals.

• Develops and maintains relationships with and generates leads through clients, family, and professional referral sources on a weekly basis. Provides information and conducts presentations about Brookdale services and programs, market advantages, availability, and other relevant information to meet the needs of prospective referral partners.

• Provides business development strategy and guidance in the M3 market to coordinate outside marketing efforts designed to spotlight Brookdale at Home, while representing the area Brookdale continuum to drive referral business for all Brookdale product lines.

• Represents Brookdale's private duty service line and increases awareness through participation in outside events, professional groups, and community involvement in the local market. Uses relevant knowledge and research to plan, coordinate, and implement monthly prospect and/or referral source activities and events as specified by management and the agency marketing plan. Follows up and executes sales process with all leads from events.

• Assists BAH management with client retention through new client welcome events, referral programs, outside visits to current hospitalized clients, and other programs as outlines in the marketing plan or by the National Director of Sales & Marketing BAH.

• Partners with management to develop and execute marketing plans and achieve caseload goals. Researches and provides recommendations for content and delivery of brochures, flyers, press releases and other forms of media that promote services.

• Visits with competitors on a quarterly basis. Creates and updates competitive analysis on a quarterly basis. Serves as a main contact for all competitive issues that would affect agency.

Qualifications

Education and ExperienceBachelor's Degree in Marketing, Business, or related field from an accredited college or university is preferred. A minimum of two to five years sales experience, preferably in the retirement industry or medical or pharmaceutical sales, with a proven track record of generating and closing a high percentage of qualified leads required; or equivalent combination of education and experience.

Responsibilities

The RN Care Manager serves as a liaison among Brookdale teams, residents, families, and healthcare providers within our communities, promoting coordination, communication, and collaboration at Brookdale's HealthPlus communities. This role encompasses the identification, coordination, communication, and implementation of services essential for enhancing residents' quality of life and achieving an optimal level of wellness. Works closely with primary and specialty care providers to formulate resident-specific care plans, ensuring that the delivery of medically necessary services aligns with established care plans. Serving as a resident advocate, they actively contribute to identifying and improving service delivery, closing gaps in care, and understanding key interventions and care protocols relevant to treating residents in place.

• Educates and explains Brookdale's care pathways as determined by individual diagnosis and healthcare needs.

• Monitors and communicates quality performance measures, and assists with training and education of teams to ensure quality outcomes for residents. This involves effective communication and collaboration with providers and other healthcare entities, such as managed care coordinators, insurance providers, and external healthcare partners responsible for coordinating resources and supplies pertinent to resident needs.

• Reviews the current population's medical diagnoses to identify care needs. Assists in assessing the health, functional, and psycho-social status of residents.

• Coordinates services required to enhance optimum wellness and manage chronic conditions.

• Collaborates with hospice, palliative, and home health services/therapy as indicated.

• Provides advanced care planning education to residents and families/responsible parties. Helps ensure each resident has a completed Advanced Care Plan, inclusive of goals of care. Communicates with healthcare providers to ensure care delivery is consistent with the resident's Advanced Care Plan(s) Participates in collaborative care reviews (CCR) and care conferences.

• Provides resources for residents and families to make informed decisions regarding choices in meeting healthcare needs and effectively champions the benefits of residing and receiving care and services at the community. This includes informing them about opportunities to enhance care and the benefits of an IE-SNP plan or other benefit opportunities.

• Applies quality improvement methodology to analyze, enhance, and manage outcomes, striving to achieve quality measure goals. Collaborates with Health and Wellness Directors (HWD) to ensure compliance with service plans, while working with providers to attain value-based outcomes for our residents. This includes coordinating and communicating care for each resident.

• Provides oversight, coordination, and family communication during resident hospitalization and rehab stays. Ensures return to the community where appropriate.

• Communicates information promptly to the appropriate care provider regarding changes in residents' health/well-being, personal needs, risk management issues, customer service issues, and family, or outside health care provider concerns.

• Facilitates continuity of care for those residents receiving home health care, hospice services, and other third-party healthcare-related services.

• Completes documentation as required by Brookdale or state regulatory agencies.

Responsibilities

Job Summary

Assists Clare Bridge Program Manager or Coordinator in leading and implementing a calendar of person centered dementia friendly programs that meet specific needs of each and all residents within the dementia care community. In alignment with Brookdale's Resident and Family Engagement philosophy, assists in providing person centered programs while engaging residents in meaningful and purposeful dementia care programs within the Brookdale community enhancing their quality of life. Recognizes individual abilities and needs while encouraging independence and ensuring resident habits, historical routines, interests and preferences are honored in the provision of care and programming.

Essential Functions

• Supports the implementation of a person centered program to meet individual needs and interests of the residents. Assists in leading and engaging residents in the 7 domains of well-being programs - purposeful, intellectual, social, spiritual, emotional and physical in a dementia friendly structured environment; including sensory focused small groups for residents who cannot participate in a large group setting.

• Assists the Program Manager or Coordinator in the review of individual resident's service plans and completes program documentation, as required by Brookdale standards or state requirements.

• Assists the program manager or coordinator in ensuring dementia care programs are in compliance with state, federal, and other regulations and meet Brookdale's mission, policies, procedures and standards.

• Assists in providing guidance and resource to associates on the importance of programming/engagement and other areas regarding the quality of life for residents.

• Coordinates and participates in special events at the community including family socials, marketing/sales and open houses.

• Communicates positively with residents, families, and community professionals regarding the Brookdale dementia care program and its benefits. Communicates with families regarding residents' personal needs and successes.

• Assists Manager or Coordinator with meeting the department's quality improvement initiatives including the Brookdale Excellence Standards Tool (BEST).

• Supports plans and scheduled programming events outside the community, which may include driving a community motor vehicle.

• Assists with volunteer program, including training and supervising volunteers.

• Performs personal care, housekeeping and dietary responsibilities, as needed, to ensure service quality is maintained in accordance with Clare Bridge Program standards.

Responsibilities

Responsible for developing and providing short-term tactical direction and oversight. Develops short-term improvements impacting an area. Directs the overall operations of an assigned group of communities within a geographic region. Oversees a revenue of $20-60 Million.

• Develops long-range objectives and strategies for each community within the region.

• Develops annual operating budget, including census, payroll, staffing and capital forecasts. Monitors and assesses compliance with financial forecasts, productivity and goal achievement. Determines areas needing cost reduction and program improvement and implements changes as needed through the Executive Directors at each community.

• Implements company-wide programs at communities in the areas of dining, asset management, finance, human resources, and other operational areas. Enhances appropriate communication between the communities and the divisional/corporate offices.

• Works with community management to implement sales and marketing activities and strategies to maximize occupancy.

• Assures that resident services reflect the highest quality in conformance with federal and state law and company standards. Enhances the quality of resident care by working with the functional team members in nursing, life enrichment, dining services, housekeeping and maintenance as needed.

• Evaluates, monitors, and enhances public relations efforts at each community.

• Assists all regional offices with preparation for state survey and licensing.

• Assures that all regional communities conform to qualitative and quantitative operating standards.

• Directly supervises and evaluates performance of Executive Directors within region. Updates, trains, and coaches staff as necessary to maintain high standards of quality care, financial accountability and business growth.

• Influences change in a positive manner within the assisted living industry through involvement with trade associations, regulatory agencies, and state executive and legislative officials.

• Partners with regional and community operations teams to oversee the overall health and safety program and practices for the region and maintain a safe environment for the residents, associates and families.

Qualifications

Education and ExperienceBachelor's Degree in Healthcare, Gerontology, Business or other related field required. Minimum of seven to ten years experience in Operations, Marketing, Financial Planning and Human Resources Management in a management role; or equivalent combination of education and experience.

The Financial Analyst role provides support to Financial Planning & Analysis that facilitates the successful achievement of business objectives. We are an upscale independent living retirement community located in North-West Houston (near Tomball). This position involves some security, but only on the inside of the building. This position will sit at our front desk to monitor cameras and life safety systems, this position will respond to resident emergencies by dialing 911, staying with the resident and then contacting the appropriate emergency contact. The security guard will conduct building rounds to ensure all is quiet, during the rounds the security guard will pick up any garbage and check all perimeter doors, making sure they are closed and locked. The security guard will contact the appropriate authorities in the event of suspicious activity, while also communicating to management any incidences and completing an incident report. The security guard will handle any minor maintenance issues, such as unstopping toilets, resetting breakers and helping the residents with tv troubleshooting, they will also contact the director of maintenance in the event of a major maintenance emergency. The position will complete a nightly shift log and note all issues and concerns. There is also the task of placing dinner orders through our POS system and answering the phone.

This position is full time, Monday through Friday from 4pm-12am. We are seeking a strong, reliable individual that can work independently and enjoys working with seniors.
