Applies knowledge and skills necessary to provide care appropriate to the age of the patient served (infant, pediatric, adolescent, adult and/or geriatric). Demonstrates competence, awareness and sensitivity of the physical, emotional and socio-psychological needs of infants through geriatics. Conducts occupational therapy evaluations. Demonstrates skill in appropriate selection, administration and interpretation of standardized and non-standardized assessments using age specific criteria for each age population served. Develops and implements an occupational therapy treatment plan according to a valid prescription and in accordance with policies and procedures.

POSITION SUMMARY Provides and interprets evaluations for hearing and balance disorders. The audiologist will perform diagnostic testing with the realm of the scope of practice of Audiology, including but not limited to: Auditory evoked Potentials, ECOG, VEMP, hearing evaluations, immittance testing, fitting and dispensing of hearing aids with verification methods as well as mapping/programming of cochlear and temporal bone implants for the pediatric and adult populations. These tests will be performed along with all subsequent reports.

ACCOUNTABILITIES Provides support and diagnostic testing for ENT physicians and advanced practice providers

Provides diagnostic evaluations, treatment and interpretation for auditory disorders Performs all modalities of evoked potentials. Provides interpretation of all BAER studies done for hearing purposes. Interprets vestibular studies (ECOG/VEMP) and provides formal report to referring physician. Provides referrals to other allied health professionals as deemed necessary. Provides recommendations regarding need for further testing or treatment as needed. Counsels patients regarding test results, referrals and recommendations. Participates in all staff meetings and conferences concerning Audiology. Assist in the selection of supplies, materials and equipment to be purchased for the Audiology section. Assists in establishing goals, procedures, and services within the section as appropriate. Participates in the development of new programs Conducts in-service training programs for other audiologists and staff members. Supervises audiology doctoral students as necessary. Acts as a resource for patients, other medical professionals and other area audiologists for referrals, recommendations and advice on hearing and balance needs. Appropriate recommendations and verification of amplification including temporal bone implanted devices. Responsible for mapping/programming of cochlear and temporal bone implant devices. Maintain required minimum continuing education units required by certifying organization and/or State licensure. Maintain Basic Life Support Provider certificate with timely renewals. Travel to and from satellite sites.

  1. Interprets and processes all medication orders. Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques.

  2. Sustains the formulary by minimizing nonformulary procurements, utilizing the therapeutic substitution policy and promoting rational medication therapy selection.

  3. Monitors all new medication orders against the patient's medication profile for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, allergies, and appropriateness of drug, dose route, frequency, and therapeutic duplications. Resolves identified problems with the physician or nurse; and documents these activities as appropriate.

  4. Answers all drug information questions accurately using drug information resources available in the department and documents these activities as appropriate.

  5. Reads, extracts, and interprets information in patient charts accurately.

Provides professional review, quality control checks, and/or direct provision of service for: all unit dose packaging program, all admixtures and sterile products including formulation, stability and sterility consideration, extemporaneous compounds, medication order entry, floor stock orders and controlled substances.

  8. Obtains required continuing education for state licensure.

  9. Provides clinical consultation and clarification to practitioners; recommends evidence-based medication therapy regimens and monitoring plans; and suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Under the direction of the Manager of Jobst Medication Therapy Management, performs ambulatory care clinical pharmacist functions necessary for the daily operations. ACCOUNTABILITIES

In collaboration with providers, manages referred patients' medication therapy as outlined in the Collaborative Practice Consult Agreement. Communicates politely, effectively, and professionally with patients, other pharmacists, and all other healthcare providers verbally in-person and via phone, as well as written through mail and electronic medical record (EMR). Clarifies any medication questions with any of the patient's providers/specialists when needed. Cooperates with management and peers to promote an environment of teamwork and collaboration. Performs point-of-care tests and/or orders disease related tests based on referred patients' medication therapy; documents results and communicates results following policies and standards of ProMedica clinical laboratory. Performs outpatient chart review for appropriate medication usage and provides patient counseling about related drug therapies. REQUIRED QUALIFICATIONS

Education: High School diploma or Equivalent.

License/Certification/Experience:

A diploma from an accredited phlebotomy training institute OR one year phlebotomy experience.

Successful completion of an academic program which included substantial practical clinical phlebotomy experience (i.e.: 3+ week clinical student rotation program in a hospital setting).

PREFERRED QUALIFICATIONS:

Professional registration preferred [i.e., PBT(ASCP)]

1 year experience in a high-volume phlebotomy environment

Position Summary:

The Registered Nurse assesses, plans, implements and evaluates the nursing care provided to a group of patients. The Northwest Ohio Hemophilia Treatment Center is a hospital outpatient department that specializes in the care of patients with bleeding disorders, clotting disorders and other benign hematological disorders. We are a comprehensive care center that specializes in the care of patients across the lifespan. Hemophilia Treatment Centers are federally designated and emphasize preventative services to help reduce or eliminate complications. We are also a Center for Excellence in the care of women/girls with bleeding.

Position Summary:

The Office Manager for the Hemophilia Center is responsible for efficiency in daily clinic operations to ensure safety, quality, patient experience, staff engagement and fiscal responsibility. Promotes a culture of inclusion and team building to create a productive workflow within the clinic. Other duties include, but are not limited to:

Maintains operations of daily clinic function, safety and quality for patients/families, providers, and staff. Assures IT operations and equipment are functioning. Maintains direct communication with providers and staff via email, huddles, and weekly clinic and MD-OM meetings. Manages operations of specialty clinics. Creates and manages physician schedules serving as a liaison between multiple business units. Manages staff and patient scheduling to enhance efficiency, patient flow and economic stability, while improving access to care. Creates clinic budgets, monitors, and approves expenses to maintain productivity targets. Uses EPIC and STRATA to assure timely and accurate data entry, appropriate completion of billing and the billing process, and completion of the work queues. Assure compliance with ProMedica Policy, state / federal agencies (HIPAA, ODH, OSHA, CLIA, Joint Commission) and maintain a continuous state of readiness. Manages Hemophilia Foundation requirements by continuous data collection, evaluation, and Action Planning to meet target goals. Manages clinic grants as Project Director assuring guidelines and timelines are followed; assures completion of grant annual review, executed official assurances, IRB approval, estimated budget, expense submission to grant department, and report of expenditures. Maintaining direct communication with the grants department and grant funders. Interviews and hires new staff members. Designs orientation of new staff via EPIC, MyLearning and use of experienced staff. Assures all staff understand new education, system, and clinic updates, and continue to follow policy and procedure. Oversees responsibilities and duties of staff to ensure productivity, efficiency and workflow using the audit process. Ensures employees job description is consistent with the duties performed and holds staff accountable for performance and service, including annual evaluations and the performance improvement process.

Education: Bachelor's Degree in Nursing. Skills: Medical terminology, excellent computer skills, organizational skills. Must have excellent time management skills, ability to multi-task and prioritize work. Attention to detail, problem solving skills, and critical thinking. Must have a strong commitment to customer satisfaction and be able to balance the needs of providers and organizational initiatives. POSITION SUMMARY

The Respiratory Care Assistant will be competent to care for patients of all ages to include infants, children, adolescents, adult, and geriatric population, dependent on the area assigned. The Respiratory Care Assistant is able to perform the skills/procedures validated by an accredited respiratory care program and approved by the state issuing the limited practice license, required by the assignment with supervision, according to regulatory standards, clinical practice guidelines, policies and procedures approved by the Medical Directors and Administration. This individual performs the duties and responsibilities outlined below as assigned, under supervision from the Respiratory Care Department leadership team and the Medical Directors of Respiratory Care. Standards of performance and skills for the areas to which the individual may be assigned must be maintained according to department policy. The individual must be committed to providing patients and families with a positive experience through compassionate care and excellent service, safely and efficiently. This individual will display professionalism at all times and maintain patient confidentiality.

ACCOUNTABILITIES

  1. Achieves and maintains a level of competency to perform prescribed therapeutic and diagnostic procedures within their scope of practice per business unit, in accordance to departmental competency standards, which include general care, critical care, emergency care, and diagnostic procedures, including, but not limited to, the following: - General Care: patient identification and assessment, medical gas delivery, medication administration via nebulizers and MDI/DPI, bronchopulmonary hygiene, and hyperinflation therapy - Critical Care: initiation, management and weaning of invasive and non-invasive ventilation; airway management and suction, including tracheostomy, endotracheal and pharyngeal; cuff and ETCO2 monitoring, intubation assist and extubation; set-up, calibration, and maintenance of arterial and central venous pressure lines - Emergency Care: responds to rapid responses and emergency codes, as required; performs cardiopulmonary resuscitation (CPR), airway management, and actively participates in patient stabilization and internal transports - Diagnostic Care: blood sampling and analysis, specimen collection, continuous and/or noninvasive monitoring, bedside pulmonary function screens, spontaneous breathing parameters, electrocardiograms (EKG) o Note: critical tests and values will be reported in a timely manner per Hospital Policy.

  2. Safely assembles, operates and trouble shoots therapeutic and diagnostic equipment required for patient care, including, but not limited to, the following: - Gas administration, monitoring and analyzing devices, including flow meters and regulators, blenders, cylinders and bulk systems, compressors, analyzers, and pulse oximeters - Mechanical devices, including mechanical ventilators, manometers, gauges, spirometers and EKG machines. - Age specific patient care equipment, including resuscitation equipment, oral and nasal airways, endotracheal and tracheal tubes - Equipment disinfection and sterilization

  3. Understands and applies Respiratory Care Clinical Practice Guidelines, to provide safe, highly efficient, and cost effective care, using evidence based best practice.

  4. Provide educational needs of patients on disease specific process, tobacco cessation and respiratory care services provided, including instruction for home care equipment and medications.

  5. Assist with clinical instruction of respiratory care, nursing, and medical students and residents, as needed. Actively participates in the orientation of new staff and contributes to the growth of others

  6. Understands and acts in accordance with all Respiratory Care and Hospital Policy and Procedures for those areas assigned and is able to with minimal supervision in those areas.

  7. Completes accurate patient documentation, including billing and productivity standards. Participates in creating a plan of care, evaluate achievement of identified goals, record results and communicate pertinent information to appropriate members of the health care team

  8. Performs other related duties as assigned or requested, within the scope of training and educational documentation as per Limited Permit

Education: Must be actively enrolled and in good standing, or a recent graduate of an accredited respiratory therapy program Skills: Must be able to input and retrieve computer information License: If working in an Ohio facility, a current Limited Permit License to render respiratory care services by the Ohio Respiratory Care Board in the State of Ohio. Membership in the American Association of Respiratory Care and other professional organizations is desired. Certification: Must hold current BLS certification
