The Charles and Virginia Hickman Hospital will give our community its first-ever wellness destination. Our families and friends will have improved access to nutrition education, exercise resources and state-of-the-art care like they never have before. We offer over 200,000 square feet and has 58 acute cate beds in our new state-of-the-art regional hospital. Our campus also features an organic farm with access to nutrition, education and therapies. Position Summary:
Provides professional nursing care appropriate to the age of the patients being served. Participates in patient/family teaching and provides leadership by working collaboratively with physicians and ancillary health care personnel. Communicates patient information in a complete, accurate manner while maintaining confidentiality. Maintains and improves personal and professional learning and development through literature, seminars, workshops, and affiliations.

Current State of Michigan license as a Registered Nurse.
Current BCLS.
ACLS required within one year of hire for CCU staff.

Facilitates continuity of care through flexible scheduling and willingness to work additional shifts when needed.
Graduate of an approved school of professional nursing.
Previous experience in an acute care setting preferred.

Position Summary
Our Environmental Services Technicians are responsible for executing the daily duties required to establish and maintain a safe, hygienic, and aesthetically pleasing care environment. While executing the assigned tasks, the EVS Tech will have contact with patients, visitors, and clinical and non-clinical staff. Great customer service and a friendly can-do attitude will ensure your success. Our techs are responsible for ensuring the cleanliness and safety of patient rooms and common areas throughout the Toledo Hospital campus within the basic principles of environmental hygiene and infection prevention. The environmental services technician, in accordance with organizational standards of quality, is an essential contributor to cleaning, disinfection, infection prevention and a positive patient experience.
Primary Responsibilities
Responsible for daily occupied and discharge/transfer cleaning and disinfection of patient rooms, as well as other common areas of their job assignment such as rest rooms, nurses' stations, corridors, conference rooms, utility rooms, treatment areas, kitchens, offices, lobbies, lounges, stairwells and waiting rooms. Other assigned areas may include surgery OR's, labor and delivery, emergency department and other procedural areas.
Performs duties while demonstrating working knowledge of the health care cleaning and disinfecting principles and requirements as well as proper and safe chemical use.
Adheres to all organization safety, risk management and infection control guidelines and standards while promoting a culture of safety.
Actively participates in departmental performance improvement initiatives, including but not limited to initiatives to improve patient experience and cleanliness metrics.
Maintains cleaning cart(s), supplies and equipment evidence by properly stocking and restocking environmental services cart in a neat and orderly fashion.
Removes trash and other waste and transports to the appropriate disposal area while identifying proper waste streams handling.
Follows all infection prevention practices (i.e. proper cleaning and disinfection protocols and process, hand hygiene, standard & isolation precautions, as well as proper donning and doffing of personal protective equipment (PPE).
Recognizes and adheres to patient privacy requirements (HIPAA).
Practices critical thinking skills.
Demonstrates personal accountability and shows appropriate respect to co-workers, patients and visitors.
Identifies and utilizes appropriate resources to ensure problems or concerns are followed through and resolved.
Demonstrates knowledge of facility safety requirements.
Demonstrates the ability to operate and properly maintain all EVS tools and equipment needed to clean and disinfect the environment.
Demonstrates flexibility with regards to daily work assignment and assigned tasks.
May be asked to assist with on-boarding and peer-training of new Environmental Technicians.
Requires attention to detail, ability to follow written and verbal directions and the ability to work with minimal supervision.
Ability to move about the hospital and between workstations.
Provide excellent customer service and respond to inquiries.
Effective interpersonal skills.
Ability to operate and utilize pagers, vacuums, putty knives, carts and trash mobiles.
Ability to tolerate unpleasant odors and possible exposure to communicable diseases and body fluids; must be able to tolerate exposure to dust and chemicals.
Ability to constantly move, lift or carry light to medium weight equipment furniture, etc. and occasionally move or lift heavy equipment or furniture.
Must be able to wear N95 or PAPR hood.
Stoop bend, reach, stretch, push and pull. Must be able to move and/or lift material and damp mops or towels weighing up to 10 lbs., 70% of the time; move carts weighing up to 40 lbs., 10% of the time; and move carts weighing up to 50 lbs., 20% of the time.
Performs other duties as directed.
Education/Certifications/Licenses
High School Diploma desired/preferred.
Certified Health Care Environmental Services Technician (CHEST) and/or Certified Surgical Cleaning Technician (CSCT) desired/preferred.
Special Skills/ Equipment:
Can demonstrate effective use of computers and/or mobile devices for daily job performance activities.
Ability to read/write English at 6th-9th grade level preferred.

Position Summary
Our Environmental Services Technicians are responsible for executing the daily duties required to establish and maintain a safe, hygienic, and aesthetically pleasing care environment. While executing the assigned tasks, the EVS Tech will have contact with patients, visitors, and clinical and non-clinical staff. Our techs are responsible for ensuring the cleanliness and safety of patient rooms and common areas throughout the Toledo Hospital campus within the basic principles of environmental hygiene and infection prevention. Primary Responsibilities
Responsible for daily occupied and discharge/transfer cleaning and disinfection of patient rooms, as well as other common areas of their job assignment such as rest rooms, nurses' stations, corridors, conference rooms, utility rooms, treatment areas, kitchens, offices, lobbies, lounges, stairwells and waiting rooms. Other assigned areas may include surgery OR's, labor and delivery, emergency department and other procedural areas.
Performs duties while demonstrating working knowledge of the health care cleaning and disinfecting principles and requirements as well as proper and safe chemical use.
Adheres to all organization safety, risk management and infection control guidelines and standards while promoting a culture of safety.
Actively participates in departmental performance improvement initiatives, including but not limited to initiatives to improve patient experience and cleanliness metrics.
Maintains cleaning cart(s), supplies and equipment evidence by properly stocking and restocking environmental services cart in a neat and orderly fashion.

REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

POSITION SUMMARY
The UM Coordinator reviews submitted clinical information for prior authorization requests, acute care initial, concurrent and post-discharge, LTAC, SNF, home health authorizations, and outpatient/ambulatory services. Assist with transition of care needs including referrals to case management, behavioral health, population health, and disease management.
ACCOUNTABILITIES
Perform effective preadmission, initial, concurrent, and post-discharge medical management for outpatient/ambulatory, inpatient, LTAC, SNF, and home health authorizations. Identify members for potential case management and referral needs. Reviews clinical submitted by providers to identify any transition of care needs and assists with this process.
Authorize and facilitate ambulatory services, acute care/post-acute care, and home health care as well as out-of-plan services, considering the member's individual needs, and utilizing panel providers whenever possible for cost-effectiveness.
Precertify/certify and concurrently review requests for care utilizing InterQualR level of care criteria to ensure appropriate medical care in the most cost-effective setting. Utilize nursing education and experience to evaluate the appropriateness of requests relative to the member's diagnosis(es)/condition.
Assess all members receiving care for post-discharge or Case Management needs. Refer members directly to Case, Behavioral, or Disease Management (depending on the product) to ensure that the member receives appropriate medical care and services, in the most cost-effective setting, to maintain or improve health status when possible, and prevent avoidable readmissions.
As appropriate, collaborate with hospital discharge planning staff to identify an appropriate post-discharge plan of care for hospitalized members. Confer with physician offices, ancillary providers, Plan Case Managers, and Medical Directors on pertinent case questions or situations when evaluation or decision is beyond the scope of the Coordinator. Send post-discharge letters, as appropriate, to follow up and further assess the member's situation, functioning, and compliance with the follow-up plan.
Document ambulatory and inpatient medical management, in the appropriate system applications, according to current Company and Department policies and procedures. Complete all related reporting documentation according to established timeframes. Comply with all Federal, State, accreditation, professional and corporate regulations, standards, and policies pertaining to privacy and confidentiality.
Identify potential high-dollar cases and collaborate with the Finance Department on case-specific expected risks;
Report any quality issues identified during clinical documentation review. Identify and report any concerns for fraud, waste, and abuse to the internal investigation unit.
Enforce prior authorization contract language and regulatory compliance requirements for network providers. Enforce regulatory compliance requirements for out-of-network providers.
Initiate single case agreement requests with out-of-network providers and initiate sanction/exclusion investigation review for out-of-network provider requests.
Assist in the investigation of provider complaints and regulatory complaints when warranted.
Perform other duties as directed.

Education: Associates Degree in Nursing

Position Summary:
The Phlebotomist collects products or specimens for basic procedures, such as: analytical testing of glucose, perform procedures, such as: bleeding times, sweat chlorides, D-Xylose tolerance and point-of-care testing using proper techniques and performs and documents quality control procedures utilizing laboratory computer information system. They follow strict principles of asepsis and antisepsis & assist in the care of the patient prior to, during and after specimen collection. The phlebotomist determines, from like samples, combinations of tests and the associated acceptable minimum volumes. They are knowledgeable of pre-analytic variables, which may affect specimen analysis and are familiar with test synonyms and abbreviations, and cross-referencing tests to specimen types.
Must have a high school diploma or equivalent. A diploma from an accredited phlebotomy training institute OR one year phlebotomy experience.

Experience must be either: Six months' experience in a high volume phlebotomy environment, or successful completion of an academic program which included substantial, practical clinical phlebotomy experience (EX: 8+ week clinical student rotation program in a hospital setting)

Position Summary
Our Environmental Services Technicians are responsible for executing the daily duties required to establish and maintain a safe, hygienic, and aesthetically pleasing care environment. While executing the assigned tasks, the EVS Tech will have contact with patients, visitors, and clinical and non-clinical staff. Primary Responsibilities
Responsible for daily occupied and discharge/transfer cleaning and disinfection of patient rooms, as well as other common areas of their job assignment such as rest rooms, nurses' stations, corridors, conference rooms, utility rooms, treatment areas, kitchens, offices, lobbies, lounges, stairwells and waiting rooms. Other assigned areas may include surgery OR's, labor and delivery, emergency department and other procedural areas.
Performs duties while demonstrating working knowledge of the health care cleaning and disinfecting principles and requirements as well as proper and safe chemical use.
Adheres to all organization safety, risk management and infection control guidelines and standards while promoting a culture of safety.
Actively participates in departmental performance improvement initiatives, including but not limited to initiatives to improve patient experience and cleanliness metrics.
Maintains cleaning cart(s), supplies and equipment evidence by properly stocking and restocking environmental services cart in a neat and orderly fashion.
Removes trash and other waste and transports to the appropriate disposal area while identifying proper waste streams handling.
Follows all infection prevention practices (i.e. proper cleaning and disinfection protocols and process, hand hygiene, standard & isolation precautions, as well as proper donning and doffing of personal protective equipment (PPE).

The Registered Nurse assesses, plans, implements and evaluates the nursing care provided to a group of patients. Serve as spiritual caregiver to patients, families and staff.
ACCOUNTABILITIES
Must be available to serve as a back-up for the on-call chaplains. Needs to be available by telephone or pager at all times during back-up on-call shifts and able to respond immediately when contacted by a hospital. If the situation is an emergency, must arrive at the hospital within 45-60 minutes.
Shares responsibility within the department for on-call shifts, being available for a maximum of 3 on-call shifts per month when no contingent chaplain is available.
Shares responsibility within the department to cover one holiday on call shift per calendar year.
Attends monthly spiritual care meetings.
Must complete all MyLearning assignments in a timely fashion.
Completes appropriate documentation, electronically and using appropriate forms, and maintains accurate statistics for the department.
Must coordinate communication between patient/family and community clergy as needed.

Education: Master's degree in related field required or working towards such degree. Must have four (4) units of Clinical Pastoral Education (CPE) for full-time positions and two (2) for part-time positions.

POSITION SUMMARY
Under the direction of the radiologist, performs complex professional procedures for the diagnosis and treatments of patients. Obtains high quality images, which are interpreted by a radiologist in the evaluation of disease or abnormality. Assists radiologists or other staff physicians in the performance for diagnostic radiologic procedures whenever required.
Must have completed formal radiologic technology training at an AMA-approved school.
Must have met ARRT (American Registry of Radiologic Technologists) registry requirements and be currently registered and hold an active state license or be registry and state licensure eligible. POSITION SUMMARY
Pharmacy technicians assist pharmacists in preparing and distributing medications, maintaining the drug inventory, and maintaining patient records. Pharmacy technicians work only under the supervision of a registered pharmacist. ACCOUNTABILITIES
Under the direction of a registered pharmacist, procures and distributes medication orders per physician request, pharmacist request, or prearranged work assignment according to established policies, procedures, and protocols.
Fills physician orders. Replenish medication surgical kits, automated dispensing machines, and stock requisitions accurately.
Prepares intravenous admixtures and other sterile preparations (including chemotherapy and parenteral nutrition where applicable) safely and accurately using appropriate techniques. Only Pharmacy Technicians who have completed all required competencies will prepare intravenous admixtures
Assists in ordering, receiving, unpacking, and storing medications and supplies in appropriate locations; Repackages bulk medications in unit-dose packages safely and accurately per established procedures using appropriate techniques
Assists in completing all medication inspections and identifies and removes all outdated and unusable medications.
Collaborates with other health care professionals in meeting departmental goals. Establishes and maintains mechanisms for communication and constructive problem solving with staff. Utilizes a team approach in decision-making and problem solving. Build trust and mutual respects.
Performs training of new employees and in-services current employees in areas of specialization and/or expertise.

Education: High school diploma or GED
License: Certified Pharmacy Technician with Ohio Board of Pharmacy. May be hired with a Technician Trainee License (expires 1 year from receipt) with restrictions in duties and expectation of Certified License being obtained.
Certification: Certified Pharmacy Technician (either PTCB or ExCPT (NHA) exam permitted)
