  • GENERAL SUMMARY

  • The Medical Social Worker is responsible for assisting patients with biopsychosocial problems resulting from acute or chronic illness, disability and/or trauma. The Medical Social Worker completes comprehensive assessment with patients and/or caregivers to obtain psychosocial history and develop individualized treatment plan. Provides support, resources, and case management services. Collaborates with multidisciplinary teams to implement treatment plan and provides follow up as necessary. ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provision of clinical care-

  • Provides evidence-based skills coaching, behavioral modification and direct observation to pediatric patients with mental health diagnoses.

  • Implements and reinforces strategies included in patient's treatment or behavioral plan. Relays ineffective aspects of plan back to broader team for review and adjustment.

  • Completes coping cards with each patient and encourages their use.

  • Completes nursing tasks, as delegated by the RN, for medically involved clients.

  • Maintains a collaborative and supportive stance with patients as a means of maintaining unit calm.

  • Displays expertise in de-escalation cycle and educates patients and families daily on its use.

  • Crisis Management-

  • Maintains consistent awareness of where each assigned patient is on de-escalation cycle; utilizes skills appropriate for observed de-escalation level.

  • Adopts a collaborative and reflective stance with patients with the goal of de-escalation in mind.

  • Provides emotional first aid and de-briefing to individual patients and groups after behavioral escalations.

  • Ensure safe and compliant treatment setting-

  • Manages patient's room, unit environment and team schedule, ensuring consistent structure and a priority towards safety.

  • Ensures work areas are organized, safe and ready for the next activity or shift.

  • Maintains constant awareness of patient location and immediately alerts nurse to changes in mental status.

Experience, Education and Training Minimum:

Current State license as a Registered Nurse (RN) or Registered Nurse holding a valid Compact State license. BSN preferred. Candidates must complete a BSN program within three years from their date of hire (if hired after July 1, 2012 must be actively enrolled in a BSN program with completion within three years). Pediatric experience preferred. RN specialty certification preferred (CPN, CCRN etc). CPR Training required and/or must be obtained within 45 days of hire. PEARS training required within 90 days of employment. PALS training required within 6 months of hire. Nurses who assist with cardiac procedures are required to obtain ACLS training within 12 months of hire. Job Description

  • GENERAL SUMMARY

  • The Mental Health Technician is responsible for assisting in the delivery of continuous individual patient care, interaction, supervision and role modeling to pediatric patients and families under the supervision of a registered nurse. Reports to the RN - C/C Staff Nurse Manager.

  • ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Performs basic patient care, clinical and non-clinical procedures appropriately under the direction of the registered nurse.

  • Maintains the safety of the patient and the environment.

  • Assists with the activities of daily living.

  • Collects, documents, and reports data to the registered nurse.

  • Performs other duties as assigned.

  • LICENSES AND/OR CERTIFICATIONS

  • Current certification as a nurse assistant, medical technician, medical assistant, emergency medical technician, or completion of a hospital-based training program for unlicensed assistive roles with similar clinical competencies. Nursing students can qualify if they are actively enrolled in an accredited Registered Nurse program with completion of at least two semesters of clinical rotation.

  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

  • High School Graduate or equivalent.

  • CPR Training required and/or must be obtained within 45 days of hire.

  • Crisis Prevention (CPI) certification must be obtained within 90 days of hire.

  • Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing.

  • Preferred:

  • Degree from an accredited college or university in social work, psychology, recreational therapy or related therapeutic discipline.

  • One year of related experience, with understanding and knowledge of mental illness.

  • Work experience in the treatment of children and adolescents seeking psychiatric treatment.

  • Increased Security Clearance Required for Job Roles in Medical Tower II: FBI fingerprinting, criminal background check, and Child Protective Services registry search.

  • WORKING CONDITIONS

  • Normal patient care environment with little exposure to excessive noise, dust, temperature and thelike. Frequent exposure to communicable disease and moderately adverse working condition due to performance ofcertain patient care activities. Frequent interactions with patients that display poor impulse control and behavior outside of social norms.

Centralized Nurse Triage - LPN - Childrens Hospital of The King's Daughters

Job Description

  • GENERAL SUMMARY

  • The Centralized Nurse Triage - LPN is responsible for performing successful telephone assessment of a pediatric patient's condition over the phone using established guidelines/protocols within the scope of practice and under the clinical guidance/supervision of the RN and/or Medical Director. Reports to designated RN Manager.

  • ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Obtains an accurate medical history, determines chief complaint, and provides appropriate disposition of the patient.

  • Answers the caller's questions, and provides pertinent advice and education.

  • Provides reassurance and direction to the caller.

  • Completes follow up as needed per policy.

  • Follows laws, policies, and regulatory requirements governing healthcare services and practices.

  • Performs other duties as assigned.

  • LICENSES AND/OR CERTIFICATIONS

  • Current Virginia state license as a Licensed Practical Nurse (LPN) or Licensed Practical Nurse holding a valid Compact State license required.

  • CPR required or must be obtained within 45 days of hire.

  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

  • One to two years of clinical experience required.

  • Medical office or ED experience preferred.

  • Pediatric experience preferred.

  • Professional knowledge of nursing theory and practice.

  • Knowledge of telephone-based clinical assessment techniques and telephone triage protocols.

  • Ability to effectively gather and exchange information in both oral and written communications to coordinate services and multiple schedules based on physician preferences and guidelines.

  • Proficient computer and data entry skills required.

  • Ability to utilize scheduling software (e.g. EPIC) and maintain updated schedules.

  • Superior time management and organization skills.

  • Basic computer knowledge, comfortable with different programs, private home office with adequate cable modem/broadband access required.

  • Ability to work flexible shifts.

  • Ability to travel.

  • Submission and approval of Telecommuting request form required for designated telecommuting departments.

  • WORKING CONDITIONS

  • Clinical patient care environment with minimal exposure to hazards such as excess noise, dust, or extremes in temperature. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, body fluids and other conditions common to the healthcare environment. Moderate adverse working conditions due to the need to perform certain patient care activities.
