This position is responsible for the coordination of services for patients and families who are referred to the Hospice/Palliative Care Program. The coordinator receives the program referrals, assesses patient/family need and develops the initial plan of care in collaboration with the Hospice Interdisciplinary Team. The Referral/Provider Relations Coordinator is also responsible for the provision of philosophy and end of life care through education to referral sources, health care providers and community programs. Key Responsibilities - Responsible for assessment and coordination of services for patients/families referred to the hospice/palliative care program. - Establishes networking opportunities with provider groups, associates and other professional organizations which ensure continued contact with referral sources. - Provides education to health care professionals and community on end-of -life issues to raise awareness - Completes patient admissions as determined by the program needs. - Coordinates general inpatient care in hospice contracted facilities and Solvay Hospice House.
Education Qualifications:

Current nursing licensure in state(s) of employment. Ability to obtain Wisconsin licensure within 3 months of employment required to work in the Duluth Office.
Basic Cardiac Life Support (BCLS) required or the ability to obtain within 3 months of hire.
Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency.
Proof of current motor vehicle insurance.
Preferred Qualifications:
Current or recent (within 5 years) hospice RN experience.
Background in Marketing, Business, Sales, or customer service

Job Description:
Plan, organize, control, and supervise clinical activities in the hospital pharmacy according to hospital policies, standards of practice of the profession, and State and Federal laws. Recommends innovation in the practice and function of the hospital pharmacy to the Sr. Director of Acute Care Pharmacy and carries out mutually agreed upon programs. Responsible for oversight and management of clinical practice and ongoing programs related to drug therapy, such as anticoagulation program, drug utilization review, medication use and adverse drug reaction monitoring. Partners with Sr. Operations manager in supporting pharmacy services. In addition to local site support for clinical practice, contributes, leads and oversees system clinical pharmacy initiatives and functions as needed. Serves on committees as deemed necessary. Work Experience: Minimum five years clinical pharmacy experience. Minimum two years experiences as a staff pharmacist or residency in an ASHP accredited program. Minimum one-year experiences as a supervisory pharmacist involved with program development.
Education Qualifications:
Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (BS or PharmD degree required)
Licensure/Certification Qualifications:
Licensed as a Registered Pharmacist by the Board of Pharmacy of the state in which they will be working (Minnesota, Wisconsin, North Dakota.
If currently registered as a Pharmacist in another state, must be registered with the Board of Pharmacy of the state in which they will be working (Minnesota, Wisconsin, North Dakota) as a Pharmacist-Intern and actively pursuing licensure as a Registered Pharmacist, obtaining licensure within 90 days of hire.

Job Description:
This position greets customers and facilitates the patient registration process in a timely, professional, and courteous manner. Interviews and obtains demographic and financial information from patients or their representatives through face-to-face and virtual interactions to provide Essentia Health with the documents and data necessary to ensure appropriate care and compliant, accurate patient registration and billing. Responsibilities and training include: Assisting in case and operating room preparation, patient positioning, and assisting surgeon with procedures as required; demonstrating organizational skills to ensure instrumentation, equipment, and supplies, etc., are in place; maintaining availability for on-call rotation and emergency coverage; and maintaining current knowledge on surgical procedures and technology. May perform other designated technical care relative to specialty.
Education Qualifications:
Required Qualifications Experience as a Surgical Assistant or two years experience as an Operating Room/Surgical Tech.
Graduate of an accredited program in Surgery Technology or Licensed Practical Nursing.
